[Ileus of the small intestine as sequela of intraduodenal diverticulum. Contribution of roentgen diagnosis to pathophysiology of diverticulum formation].

The case of a 31-year-old male is reported who had recurrent, unexplained abdominal complaints. After hospitalization, an ileus was localized by plain abdominal film in the ileocecal region. A contrast meal revealed a typical intraluminal (duodenal) diverticulum with an enlarged prestenotic duodenum that was undetected during endoscopy. At operation, the cause of the ileus was revealed to be a phytobezoar near the ileocolic valve. After removal and after transluminal resection of the diverticulum, the postoperative course was uneventful. The authors discuss the importance of early roentgen cin…

Meckel's diverticulum Imaging: In and Out.

Meckels diverticulum (MD) is the commonest among the omphalomesenteric duct anomaly spectrum with an overall incidence of 2-3% . It is a true diverticulum arising on the antimesenteric side of distal ileum approximately 2 feet from the ileocecal junction. Heterotopic gastric and pancreatic mucosa are commonly found within the diverticulum predisposing to bleeding and inflammation. Complications are more common in male and the type of complication differs based on the age of occurrence and include hemorrhage, inflammation, obstruction, ischemia and tumor formation. Due to overlapping clinical p…

A Presentation of Meckel's Diverticulum With Malignancy.

INTRODUCTION: Meckel's diverticulum (MD) is the most common congenital gastrointestinal anomaly resulting from incomplete obliteration of the omphalomesenteric duct. Although often asymptomatic, MD may rarely harbor malignancies. The management of incidentally discovered MD in adults remains controversial. CASE PRESENTATION: A man in his 70s presented with abdominal pain and iron deficiency anemia. Computed tomography demonstrated a 13 × 14 cm heterogeneously enhancing right lower quadrant mass concerning for a primary mesenteric neoplasm. The patient underwent surgical resection, and intraop…

[Urethral diverticulum in the pregnant woman].

Urethral diverticulum is a pathological lesion characterized by a sac-shaped protrusion of the urethra. Diverticulum is a rare condition in the urological practice. Clinical case. The diagnosis, conservative management of diverticulum during pregnancy and surgical treatment on the 7th day after natural childbirth is presented in the article. A review of similar cases showed that urethral diverticulum in pregnant women should be managed conservatively, using antibiotic therapy, anesthesia, puncture aspiration, and cystoscopic drainage. Diverticulectomy during pregnancy is not recommended, as it…

Treatment of Achalasia and Epiphrenic Diverticulum.

BACKGROUND: Epiphrenic diverticulum (ED) is a pulsion pseudodiverticulum found in the distal 10 cm of the esophagus. Motility disorders are present in the majority of patients with ED explaining the pathophysiology of this rare disease. Achalasia is the most common underlying disorder. We present a review on the diagnosis and management of ED in the setting of achalasia. METHODS: Literature review. RESULTS: Symptoms are most related to the underlying motility disorder. The diagnostic workup should always include an upper digestive endoscopy and a barium esophagogram. Esophageal manometry ide…

Perforation of a mesenteric Meckel's diverticulum.

Meckel's diverticulum is a remnant of the embryologic omphalomeseteric duct and is a common congenital anomaly found in ~2% of the population. The clinical significance of this anomaly is that the persistent diverticulum can lead to intestinal obstruction or diverticulitis and may contain ectopic tissue which can lead to bleeding, ulceration or perforation. The classic location of a Meckel's diverticulum has been described ~40 cm from the ileocecal valve on the antimesenteric side of the distal ileum. There have only been a few documented cases of a Meckel's diverticulum found on the mesenteri…

[Meckel's diverticulum : state of the art].

The persistence of the embryonic vitelline duct can lead to omphalomesenteric duct anomalies. A wide variety of anomalies, depending on the remnant segment and its degree of involution, may occur as a result of the omphalomesenteric duct failing to obliterate completely. The most frequent omphalomesenteric duct anomaly is Meckel's diverticulum, which is present in approximately 2-3 % of the population. Despite the progress in medical imaging, conventional abdominal computed tomography and endoscopy have limitations for the diagnosis of Meckel's diverticulum. The laparoscopic exploratory remain…

Toothpick impaction in a sigmoid diverticulum.

A 70-year-old woman presented to our emergency department with recurrent episodes of left lower quadrant pain. She subsequently underwent high anterior resection with working diagnosis of acute diverticulitis, but intraoperatively was found to have perforated sigmoid diverticulum due to toothpick impaction. A high index of suspicion is required for diagnosis of toothpick ingestion. Appropriate preoperative investigations such as endoscopy and CT, despite their fairly low sensitivity, should be performed to increase the ability to accurately diagnose this condition and therefore optimise subseq…

A curious case of Meckel's diverticulum.

Meckel's diverticulitis or Meckel's associated pathology frequently presents in childhood with gastrointestinal bleeding. It is rarely seen in adults. It is a congenital abnormality that commonly goes undetected. We present a case of a perforated Meckel's diverticulum due to fishbone ingestion in an elderly gentleman. The aim of this case report is to highlight the rare presentation of a perforation in a Meckel's diverticulum due to an extrinsic pathology and to outline diagnostic and management options in cases of Meckel's diverticulum.

[Giant Meckel's diverticulum in an adult].

Meckel's diverticulum results from a partial persistence of the omphalomesenteric duct and is the most common congenital anomaly of the gastrointestinal tract, affecting about 2% of the general population. Its presentation as a giant Meckel's diverticulum (>5 cm) is rare and is associated with major complications. We report a case of a 53 year-old woman with constipation for at least ten years. A colonoscopy from eight years ago suggested megacolon. The patient consults in the last month for abdominal pain associated with anorexia. The computed tomography scan image suggested an ileal megadive…

Ventricular diverticulum: a review of the literature.

BACKGROUND: Ventricular diverticulum (VD) is a rare cardiac malformation. The surgical indications for VD remain controversial. This review is designed to determine the demographic characteristics, diagnosis, and surgical indications of this disease. METHODS: Using PubMed and the Chinese electronic databases CNKI, WANFANG, and VIP, a computerized search was performed of the literature from China published between March 1965 and July 2012. Major risk factors for developing VD complications were confirmed by logistic regression analysis in case-control studies. RESULTS: Ninety-three articles a…

Resection of urethral diverticulum in pregnancy.

BACKGROUND: Female urethral diverticulum is a rare finding, less often diagnosed during pregnancy. There are few reports in the literature on urethral diverticulum in pregnancy with most undergoing expectant management. CASE: A young woman presented during the third trimester of pregnancy with pelvic and vaginal pain and a vaginal bulge. On examination, she was noted to have a 5-cm periurethral mass consistent with a urethral diverticulum. We describe the successful excision of this diverticulum during pregnancy. CONCLUSION: The procedure was performed without adverse consequences to the pre…

An unusual presentation of Meckel's diverticulum.

Meckel's diverticulum can present with unusual symptoms and signs and therefore delay diagnosis. We present the case of a 63 year old lady who presented to the gynaecologists with a huge right sided ovarian cyst. She had required 13 units blood transfusion previously (5 years ago) for massive gastrointestinal bleed with no cause found despite extensive investigations. At operation she was found to have a Meckel's diverticulum in the left lumbar region containing a large calculus. She had an unremarkable post-operative course.

Perforation of Meckel's diverticulum with enteroliths.

Perforation of Meckel's diverticulum with enteroliths is a rare complication. Here, we report a case of perforation of Meckel's diverticulum with enteroliths, which could be accurately diagnosed by the preoperative computed tomography (CT). A 46-year-old man with acute onset of severe abdominal pain, and a localized muscle guarding in the right hypochondrium, had a solitary stone detected in the right abdomen by the radiography. The abdominal CT revealed a saclike outpouching of the small intestine, containing air/fluid levels and an enterolith, with surrounding free air and mesenteric inflamm…
