[Basic diagnostic program in dizziness and syncope symptoms: value of electrophysiologic studies of the heart].

Syncopes and preceding signs and symptoms, particularly also in form of appearances of giddiness, are, including specialists of various branches, among others ENT, neurology, orthopaedics and ophthalmology, always to be classified according to the fact whether or not a cardiovascular disease is to be regarded as cause. If there is the suspicion, both special non-invasive investigations and such ones which possibly are to be performed invasively are indicated. The latter come after the non-invasive investigations partly on account of the expenditure (technique of heart catheterization) and the

The velocity step test. Its use in the evaluation of the effects of drugs in dizzy patients.

The effect of flunarizine (10 mg/day) was studied in open and double-blind clinical trials. Patients presenting the symptom of dizziness were diagnosed as having the hyperventilation syndrome. They showed vestibular hyperreactivity in the velocity step test. This test was performed before and after 4-6 weeks of treatment. The results of treatment were compared with a control or placebo group. In an open trial, 12 of 14 patients reported alleviation of dizziness during treatment and showed a significant decrease in right-left asymmetry of their velocity step responses. In a double-blind trial, …

Electron microscopic observations of the utricle and ampullae in a case of dizziness of suspected saccular origin.

Electron microscopic observation was reported on the utricle and ampullae of a case, a 36-year-old male, with dizziness attack due to suspected saccular origin. After the removal of the utricle and ampullae, the dizziness recurred, and finally the saccular nerve section relieved the dizziness attack of this case. So the actual lesion was localized in the saccule and was not in the specimens obtained at the first surgery. Though most of hair cells appeared normal, there were a few with many vacuoles or shrinkage of the cytoplasm distributing sparsely throughout the sensory epithelia. The distri…

Self-Reported Dizziness Handicap is Related to Functional Capacity in People with Vestibular Hypofunction.

BACKGROUND AND PURPOSE: Individuals with vestibular hypofunction (VH) report low levels of physical activity (PA), corresponding with restrictions when performing activities of daily living. However, a clear association between physical function and self-reported dizziness handicap and measures of vestibular function has not been established. Our purpose is to investigate whether functional aerobic capacity and PA are related to dizziness handicap and objective assessments of the vestibulo-ocular reflex in participants with VH. METHODS: A total of 36 participants (14 males, 22 females; 70.11 …

Joint Hypermobility: An Under-Recognised Cause of Palpitations, Dizziness, and Syncope in Young Females.

Background: Symptoms of dizziness, syncope, and palpitations are common presentations in outpatient and emergency care, frequently attributed to stress and anxiety when conventional neurological and cardiac evaluations are normal. Joint hypermobility (JH) syndromes including hypermobile Ehlers-Danlos syndrome (hEDS), and hypermobility spectrum disorders (HSD) are under-recognised as potential causes. Methods: Our retrospective cohort study examined the clinical features, diagnostic findings, and responses to treatment in patients with JH syndromes, who are referred to a specialised syncope cli…

Postural Orthostatic Tachycardia Syndrome (POTS) as a Cause of Dizziness - Expanding the Etiological Spectrum.

BACKGROUND AND OBJECTIVES: Dizziness is a frequent complaint encountered in neurology clinics. Dizziness can be spontaneous or triggered, which includes orthostatic dizziness. Orthostatic dizziness can be acute (reflex/vasovagal syncope), chronic (orthostatic hypotension (OH), or postural orthostatic tachycardia syndrome (POTS). Since dizziness has numerous causes, these patients undergo extensive investigations before a diagnosis is made. Here, we describe five patients who presented with dizziness and were diagnosed to have POTS on evaluation. MATERIALS AND METHODS: We conducted a retrospec…

Clinical criteria to exclude acute vascular pathology on CT angiogram in patients with dizziness.

BACKGROUND: Patients presenting to the emergency department (ED) with dizziness may be imaged via CTA head and neck to detect acute vascular pathology including large vessel occlusion. We identify commonly documented clinical variables which could delineate dizzy patients with near zero risk of acute vascular abnormality on CTA. METHODS: We performed a cross-sectional analysis of adult ED encounters with chief complaint of dizziness and CTA head and neck imaging at three EDs between 1/1/2014-12/31/2017. A decision rule was derived to exclude acute vascular pathology tested on a separate valid…
