Factors Associated With Poor Outcome in Childhood Swimming Pool Submersions.

OBJECTIVES: The aims of the study were to determine factors associated with poor outcome in childhood swimming pool submersions and to study the association of bystander resuscitation with clinical outcome. METHODS: This was a retrospective study of swimming pool submersion victims younger than 18 years in a metropolitan area from 2003 to 2007. Submersion, prehospital, and victim data were obtained from hospital, Emergency Medical Services, and fatality records. Outcome based on survival at hospital discharge was favorable (baseline/mild impairment) or poor (death/severe impairment). Logistic

A 10-month-old infant with reversible findings of brain death.

Death has occurred when there is irreversible loss of integration of the organism as a whole, and brain death is said to be a criterion for death. In the present case, a 10-month-old boy was found submerged in a bathtub and was given cardiopulmonary resuscitation for 37 minutes. He had received therapeutic dosing of phenobarbital and midazolam up to 5 hours prior to a brain death examination. He fulfilled all criteria for brain death according to Canadian Neurological Determination of Death Forum recommendations on an examination 42 hours after the drowning event, but started breathing another…

Advanced Cardiac Life Support.

In brief Changes in the advanced car-brief diac life support guidelines in-elude a new classification system for interventions, training in the use of automated defibrillators, and required early defibrillation for ventricular fibrillation. The algorithms for treating ventricular fibrillation and pulseless ventricular tachycardia, pulseless electrical activity, and asystole are reviewed. Cardiac arrest caused by special circumstances-hypothermia, near drowning, trauma, and electrical injuries-presents additional treatment considerations.

Snorkelling and Breath-Hold Diving Fatalities in Australia-A Review of 317 Deaths.

As snorkelling and breath-hold diving are conducted in a potentially hostile environment by participants with varying skills and health, fat … rd of victims were likely disabled by cardiac arrhythmias and at least 137 deaths were from primary drowning, with 34 following apnoeic hypoxia. Pre-existing health conditions, particularly IHD and LVH, predispose to many snorkelling deaths in older participants and may be somewhat mitigated by targeted health screening. Drownings from apnoeic hypoxia pers …

Sand aspiration managed with extracorporeal membrane oxygenation and a mechanical insufflation-exsufflation device: A case report.

We present a case of a class 3 drowning event with severe sand aspiration, successfully managed with mechanical ventilation, bronchoalveolar lavage, ECMO, and the use of a mechanical insufflation-exsufflation device (MI-E). Sand aspiration is a potentially lethal consequence of near-drowning events. It may cause respiratory failure by several mechanisms, including laryngospasm, upper or lower airway obstruction and an intrinsic acute respiratory distress syndrome (ARDS) related to the sand itself. Its standard management involves lung-protective mechanical ventilation strategies, bronchoalveol…

"Immunohistochemical analysis of Sigma-1 receptor (σ-1R) expression in human pineal gland in relation to different causes of death".

Sigma-1 receptor (σ-1R) modulates cellular signaling pathways, probably acting as a ligand operated chaperone. When activated, the receptor … rom forensic autopsies were divided into five groups according to the cause of death: sudden death, drowning, fire fatality, hanging, and hemorrhagic shock, and examined with hematoxylin-eosin stain and immunohistochemistry for σ-1R. Both pinealocytes and perivascular spaces were evaluated. The pineal glands from sudden death were only mildly positive for …

Death in a bathtub of an adolescent with neurofibromatosis type 2 exhibiting meningioangiomatosis with white matter involvement.

Neurofibromatosis type 2 (NF2) is a neurocutaneous syndrome characterized by the development of multiple benign tumors, including vestibular … under the dura mater (meningioma), and in the upper cervical cord (neurofibroma). Typical signs of drowning, such as foam in the airways, were not present. Upon histological examination, meningioangiomatosis (MA) was observed in the cerebellum and the cerebral cortex, specifically in the frontal lobe, temporal lobe, and insula. The MA extended into the wh …

Violent deaths among elderly in Attica, Greece: A 5-year survey (2011-2015).

According to World Health Organization (WHO) estimations, approximately 6 million people lose their lives yearly due to some kind of injury, while one-third of these incidents are due to violent acts. Violent deaths include accidents (road traffic accidents, drownings, falls, heat effects etc), suicides as well as homicides. Elderly people seem to be at greater risk due to their special characteristics as a vulnerable social group. The aim of this study was to investigate violent deaths (accidents, suicides and homicides) among elderly people in Attica, Greece, during the period 2011-2015 by e…

Multiple Scedosporium apiospermum abscesses in a woman survivor of a tsunami in northeastern Japan: a case report.

INTRODUCTION: Scedosporium apiospermum is increasingly recognized as a cause of localized and disseminated mycotic infections in near-drowning victims. CASE PRESENTATION: We report the case of a 59-year-old Japanese woman who was a survivor of a tsunami in northeastern Japan and who had lung and brain abscesses caused by S. apiospermum. Initially, an aspergillus infection was suspected, so she was treated with micafungin. However, computed tomography scans of her chest revealed lung abscesses, and magnetic resonance images demonstrated multiple abscesses in her brain. S. apiospermum was cultu…

Temporal and regional trends in fatal childhood injuries in Norway 1971-1989.

The paper focuses on how mortality due to injuries among Norwegian children has varied over time and throughout the country over the last tw … pes of injuries except bicyclists and passengers, but with the sharpest decline for pedestrians and drowning injuries. The regional variation was strongest for drowning and showed a different profile from the overall pattern for bicyclists, passengers and falling injuries. The incidence of fatal injuries in Norway is comparable to those of Denmark, Finland …

Childhood sporting deaths.

Exercise-induced collapse and sudden death are unusual in childhood. For this reason, a study was undertaken of a series of 12 cases of sudden death in childhood occurring during physical exertion associated with sporting activities. The age range was 7 to 16 years (mean 12.3 years, M:F ratio 5:1). Deaths resulted from trauma associated with the sporting activity, from an idiosyncratic response to exertion, or from exacerbation of a known underlying disease. Trauma was directly fatal (n = 4: vascular trauma in 1, head injury in 2, drowning in 1), exacerbated an underlying medical condition (n …

Deaths in Bathtubs in Japan: Forensic and Clinical Implications.

In Japan, deaths in bathtubs or bathtub deaths are frequently investigated as unnatural deaths. About 19,000 bathtub deaths occur annually in Japan. This pattern of death has become a social issue in forensic pathology and emergency medicine and public health. It is assumed that the death of an adult by drowning in a bathtub cannot be avoided due to disturbance of consciousness. The PubMed database was used for literature search using the retrieval words, "bathing "OR" bathtub "AND" submersion "OR" drowning "OR" death "OR" cardiopulmonary arrest". From the epidemiological characteristics and p…

The motion of floating and submerged objects in the Chattahoochee River, Atlanta, GA.

Two mannikins, one designed to float and one designed to sink, were used to examine the ways in which human bodies move in a river. The floating mannikin was used to examine the movement of a body floating downstream on the surface and to determined the flow patterns of surface currents through bends in the Chattahoochee River in Atlanta, GA. The submerged mannikin was constructed to have a specific gravity of a deceased person (over a range of specific gravities) and was used to examine the motion of a body immediately upon entering the water. The submerged mannikin also was used to examine m

[Morphological features of non-traumatic intramuscular hemorrhage].

The formation of non-traumatic intramuscular hemorrhages does not depend on the cause of death. They usually occur in the muscles attached t … to increased respiratory activity and seizures. The motor activity of the upper extremities during drowning, prolonged static muscle tension during hypothermia may matter. A microscopic picture usually indicates the formation of hemorrhages shortly before death, and occasionally signs of anxiety are noted.

Development of a novel universal primer set with high sensitivity and specificity for detection of diatom.

The diatom test has long been used as an ancillary examination in the diagnosis of drowning. The conventional diatom test requires the handling of strong acids and depends on the investigator's skill and expertise. Several issues are also associated with the diatom test, such as sample contamination and diatom loss during testing. We have developed a novel primer set for detecting diatoms using real-time PCR. The primers were designed based on the rbcL gene as a marker, and the amplicon length was 68 bp. A high detection sensitivity (0.1 pg DNA) was achieved for all diatom species tested, and …
