Thinking Three-Dimensionally: A Self- and Externally-Controlled Approach to Screening for Drug-Drug-Drug Interactions Among High-Risk Populations.

The global rise in polypharmacy has increased both the necessity and complexity of drug-drug interaction (DDI) assessments, given the growing potential for interactions involving more than two drugs. Leveraging large-scale healthcare claims data, we piloted a semi-automated, high-throughput case-crossover-based approach for drug-drug-drug interaction (3DI) screening. Cases were direct-acting oral anticoagulant (DOAC) users with either a major bleeding event during ongoing dispensings for potentially interacting, enzyme-inhibiting antihypertensive drugs (AHDs) (Study 1), or a thromboembolic eve

Drug-drug-gene interactions as mediators of adverse drug reactions to diclofenac and statins: a case report and literature review.

Concomitant treatment with drugs that inhibit drug metabolising enzymes and/or transporters, such as commonly prescribed statins and nonsteroidal anti-inflammatory drugs (NSAIDs), has been associated with prolonged drug exposure and increased risk of adverse drug reactions (ADRs) due to drug-drug interactions. The risk is further increased in patients with chronic diseases/comorbidities who are more susceptible because of their genetic setup or external factors. In that light, we present a case of a 46-year-old woman who had been experiencing acute renal and hepatic injury and myalgia over two…

Drug-drug interactions between bedaquiline and the antiretrovirals lopinavir/ritonavir and nevirapine in HIV-infected patients with drug-resistant TB.

OBJECTIVES: Bedaquiline is a new anti-TB drug, which is metabolized by cytochrome P450 (CYP) 3A4. Concomitant ART is important for all HIV-infected patients treated for TB, but several antiretrovirals inhibit or induce CYP3A4. Single-dose drug-drug interaction studies found no significant interactions with nevirapine or lopinavir/ritonavir, but these findings could be misleading, especially because of bedaquiline's long terminal t1/2. We evaluated the effect of nevirapine and lopinavir/ritonavir on bedaquiline exposure. METHODS: We conducted a parallel-group pharmacokinetic study of three gro

Effect of midostaurin on the pharmacokinetics of P-gp, BCRP, and CYP2D6 substrates: assessing potential drug-drug interactions in healthy participants : Brief title: Drug-drug interaction of midostaurin.

PURPOSE: Midostaurin, approved for FLT3-mutated acute myeloid leukemia and advanced systemic mastocytosis, is mainly metabolized by cytochrome P450 (CYP) 3A4. Midostaurin exhibited potential inhibitory effects on P-glycoprotein (P-gp), breast cancer resistance protein (BCRP), organic anion-transporting polyprotein 1B1, and CYP2D6 in in vitro studies. This study investigated the pharmacokinetic (PK) effects of midostaurin on P-gp (digoxin), BCRP (rosuvastatin) and CYP2D6 (dextromethorphan) substrates in healthy adults. METHODS: This was an open-label, single-sequence, phase I clinical study ev

Traditional nonsteroidal anti-inflammatory drugs and postmenopausal hormone therapy: a drug-drug interaction?

BACKGROUND: Suppression of prostacyclin (PGI2) is implicated in the cardiovascular hazard from inhibitors of cyclooxygenase (COX)-2. Furthermore, estrogen confers atheroprotection via COX-2-dependent PGI2 in mice, raising the possibility that COX inhibitors may undermine the cardioprotection, suggested by observational studies, of endogenous or exogenous estrogens. METHODS AND FINDINGS: To identify an interaction between hormone therapy (HT) and COX inhibition, we measured a priori the association between concomitant nonsteroidal anti-inflammatory drugs (NSAIDs), excluding aspirin, in peri- a

Clinical Drug-Drug Pharmacokinetic Interaction Potential of Sucralfate with Other Drugs: Review and Perspectives.

Sucralfate, a complex of aluminium hydroxide with sulfated sucrose, forms a strong gastrointestinal tract (GIT) mucosal barrier with excellent anti-ulcer property. Because sucralfate does not undergo any significant oral absorption, sucralfate resides in the GIT for a considerable length of time. The unabsorbed sucralfate may alter the pharmacokinetics of the oral drugs by impeding its absorption and reducing the oral bioavailability. Because of the increased use of sucralfate, it was important to provide a reappraisal of the published clinical drug-drug interaction studies of sucralfate with

Decoding Nano-Bio Interactions: Gene Regulatory Networks in Advanced Drug Loading.

Conventional treatments often face challenges such as the limited ability to penetrate the blood-brain barrier (BBB). The Doxorubicin-loaded graphene oxide/magnetite (DOX/GO/Fe3O4) nanocomplex offers a promising platform due to GO's high surface area and pH-sensitive release, and Fe3O4's magnetic properties. This protocol describes the methodology for evaluating the cytotoxicity of free DOX versus the DOX/GO/Fe3O4 nanocomplex in the A-172 glioblastoma cell line, followed by advanced bioinformatics analysis to identify gene networks and indirect pathways that enhance the nanomaterial's biocompa

Drug Interactions in People on Cannabidiol: Is There Cause for Concern?

INTRODUCTION: Cannabidiol (CBD) exhibits multiple therapeutic properties, but its use in advanced cancer patients raises concerns about potential drug-drug interactions (DDIs) due to polypharmacy. This study aims to look for evidence of DDIs between concomitant medications and CBD oil in a randomized placebo-controlled trial of CBD oil for symptom control (MedCan-1 parent study). MATERIALS AND METHODS: Surrogate measures were used to identify possible drug interactions: (1) the maximum mL of oil self-selected by patients in CBD or placebo groups in relation to opioids, specific drug groups, o…

Molecular Mechanisms of Acute Drug Toxicity in Polypharmacy: Analgesic-Psychotropic Interactions.

Concurrent exposure to analgesic and psychotropic drugs is frequent in patients with pain, psychiatric comorbidity, frailty, or acute-care needs. The clinically important question is whether analgesics and psychotropic drugs act on the same metabolic, transporter, receptor, ion-channel, or cellular stress systems. This narrative mechanistic review discusses those points of contact. CYP-mediated inhibition or induction, phenoconversion, altered parent-to-metabolite ratios, and blood-brain barrier transporter effects can change both systemic and central exposure, particularly through CYP2D6, CYP

Revealing drug interactions: A computational study of non-P450 metabolizing enzymes.

Despite comprehensive studies on P450 enzymes, studies on non-P450 enzymes are rather limited and the mechanism by which the non-P450 enzymes participate in the drug metabolism is not fully understood. Thus, we aimed to analyze the interactions between ester-containing FDA-approved drugs and three non-P450 drug-metabolizing enzymes (Bche, Bphl, and Ces1), and to investigate the structural features of their active sites using computational methods. The interaction between 920 FDA approved drugs that contain a carbonyl group (ester), and three esterase enzymes, Bche, Bphl, and Ces1 were assessed

When Nature Meets Oncology: Unraveling Herb-Drug Interactions in Cancer Therapy.

Natural product (NP) use by patients alongside their conventional cancer therapies is ubiquitous. This common, yet often hidden, practice can potentially contribute to significant patient harm, given the narrow therapeutic window of most anticancer drugs. This review takes on this challenge directly, moving past theoretical concerns to summarize current clinical evidence on interactions between widely used NPs and modern cancer treatments, including chemotherapy, targeted therapy, and immunotherapy. We break down the key pharmacokinetic (PK) mechanisms, such as the disruption of cytochrome P45

Collaboration and Decision-Support Tools to Mitigate Nirmatrelvir/Ritonavir Drug Interactions.

PURPOSE/OBJECTIVES: Nirmatrelvir/ritonavir (Paxlovid) is used to treat mild to moderate COVID-19 for patients at high risk for disease progression. However, its use presents challenges due to drug-to-drug interactions that affect its effectiveness and the safety of coadministered medications. In response, a nurse-initiated protocol and interprofessional prescribing guidance were developed at a large nonprofit academic medical center to expedite proactive treatment for at-risk, ambulatory patients with COVID-19. The guidance was based on a standardized risk score. DESCRIPTION OF THE PROJECT: D…

Variational Autoencoders for Generative Drug-Gene Interactions in Periodontal Bone Resorption.

Introduction Periodontal bone resorption is a significant dental problem causing tooth loss and impaired oral function. It is influenced by factors such as bacterial plaque, genetic predisposition, smoking, systemic diseases, medications, hormonal changes, and poor oral hygiene. This condition disrupts bone remodeling, favoring resorptive processes. Variational autoencoders (VAEs) can learn the distribution of drug-gene interactions from existing data, identify potential drug targets, and predict therapeutic effects. This study investigates the generation of drug-gene interactions in periodont
