Effect of quantitative partial valgus stress during baseball pitching on ball velocity and subjective pitch-effort.

BACKGROUND: Excessive elbow valgus stress can often cause pitching elbow injuries, and rehabilitation is usually required before an athlete can resume playing. However, there is a lack of information on the partial load rehabilitation of pitching elbow injuries caused by valgus extension overload based on elbow valgus stress. The purpose of this study was to clarify how quantitative partial elbow valgus stress while pitching affects ball velocity and subjective pitch-effort. METHODS: Forty-six male baseball pitchers participated in this study. Each player wore a wearable device on the elbow t…

Pitch Types and Their Influence on Elbow Varus Torque and Spin Rate in Professional Baseball Pitchers.

BACKGROUND: Elbow injuries are prevalent among professional baseball pitchers as nearly 25% undergo ulnar collateral ligament reconstruction. Pitch type, ball velocity, and spin rate have been previously hypothesized to influence elbow varus torque and subsequent risk of injury, but existing research is inconclusive. PURPOSE: To examine elbow varus torque, cumulative torque, and loading rate within professional pitchers throwing fastball, curveball, change-up, and slider pitches, as well as to identify potential influences of ball spin on the elbow. STUDY DESIGN: Descriptive laboratory study…

Imaging of the pediatric athlete: use and overuse.

This article highlights the unique patterns of sports-related injury of the upper extremity that radiologists are likely to encounter in children and adolescents. The injuries are classified as acute "use" injuries or chronic "overuse" injuries, and reviewed separately for the shoulder, elbow, and wrist. Recommendations for imaging strategies are provided and characteristic imaging findings are discussed and illustrated.

The assessment and treatment of nerve dysfunction after trauma around the elbow.

Nerve dysfunction after trauma around the elbow can lead to significant long-term pain and functional deficit. Fortunately, most of these injuries are neurapraxias that will recover spontaneously after conservative treatment. The necessity and time frame for surgical intervention for specific patterns of nerve dysfunction remains controversial. Often surgical exploration exacerbates rather than alleviates the presenting nerve problem. Distal humeral shaft fractures, elbow dislocations, Monteggia fracture-dislocations, supracondylar fractures in children, and proximal forearm trauma all have be

Use Of POCUS for the Paediatric Patient with an Undifferentiated Upper Limb Injury.

A 2-year-old girl presented to the Paediatric Emergency Department following an unwitnessed injury to her left arm while playing at nursery limiting further examination. On assessment she was reluctant to use her left arm and further examination was difficult. In cases of unwitnessed and undifferentiated elbow injuries point of care ultrasound (POCUS) can be used to evaluate for elbow joint effusion, fracture, or radial head subluxation, also known as nursemaid's elbow. Pulled elbow is a commonly encountered paediatric injury but based on the history and examination findings it may not always …

Arthroplasty with a metal radial head for unreconstructible fractures of the radial head.

BACKGROUND: Treatment of unreconstructible comminuted fractures of the radial head remains controversial. There is limited information on the outcome of management of these injuries with arthroplasty with a metal radial head implant. METHODS: The functional outcomes of arthroplasties with a metal radial head implant for the treatment of twenty-five displaced, unreconstructible fractures of the radial head in twenty-four consecutive patients (mean age, fifty-four years) were evaluated at a mean of thirty-nine months (minimum, two years). There were ten Mason type-III and fifteen Mason-Johnston

Arthroscopy of the elbow.

For arthroscopy of the elbow to be safe and effective, the arthroscopist must develop a systematic and reproducible arthroscopic examination technique. We believe that the technique and the identification of anatomic structures described in this article provide a safe and reliable means of performing this procedure. Attention to detail, especially when establishing the portals, is essential in performing a safe examination and in avoiding damage to the neurovascular structures. We prefer the 4-mm, 30-degree-angled arthroscope. It provides the wide field of view necessary for both diagnostic an

Anteromedial coronoid facet fractures.

The anteromedial coronoid facet and the medial lip of trochlea represent one of the most important stabilizing columns of the elbow to prevent posterior dislocation of the elbow. But on average, 58 % of the anteromedial facet extends from the proximal ulnar without sufficient support by the proximal ulnar metaphysic. Some important soft tissue structures insert on the coronoid process. The fracture of anteromedial coronoid facet was recognized recently in clinic as a distinct type of the coronoid fracture. The special injury mechanism is varus posteromedial rotational injury force. This mechan

Injury patterns and technique-specific determinants in competitive arm wrestling: implications for performance-oriented rehabilitation.

BACKGROUND: Arm wrestling is characterized by high torsional and compressive forces acting on the upper limb, predisposing athletes to both acute traumatic and overuse injuries. Despite increasing recognition of injury patterns, quantitative evaluation of technique-specific determinants and their relationship to injury localization remains limited. The aim of the study is to investigate the patterns and mechanisms of upper limb injuries in competitive arm wrestling and to identify technique- and experience-related determinants associated with injury occurrence. METHODS: A cross-sectional obse

Diagnostic Dilemma: Testing to Differentiate UCL Sprains From Flexor-Pronator Elbow Strains in the Overhead Athlete.

Elbow joint injuries continue to increase in frequency in baseball at all levels. It is the most commonly injured joint in professional baseball and approximately 25% of all Major League Pitchers have had a ulnar collateral ligament (UCL) surgery and approximately 30% of Minor League pitchers. Injuries to the elbow joint are increasing in scholastic and youth baseball players too. Often the differential diagnosis between UCL injuries and flexor/pronator tendinitis is difficult for the clinician. The purpose of this commentary is to describe the clinical thought process used to assist in making

Epidemiologic Trends in Common Injury Mechanisms and Associated Consumer Products in Pediatric Radial Head Subluxation Cases.

BACKGROUND: Nursemaid's elbow injuries occur in young children, with axial traction on the arm as a commonly-described mechanism. Prior demographic studies have noted a significant increase in this injury over the last three decades. We sought to examine incidence of nursemaid's elbow over time and assess the impact of COVID-19 on injury frequency. Additionally, certain household products and female gender have been frequently seen in cases presenting to emergency departments. The present study aims to further investigate injury scenarios to better understand the difference in injury rates and…
