Hinged external fixator and open surgery for severe elbow stiffness with distal humeral nonunion.

Joint stiffness is a common complication of elbow trauma. Treating elbow stiffness is challenging, especially in patients with severe elbow stiffness with distal humeral nonunion. To improve treatment outcomes, the authors applied a hinged external fixator after performing open reduction and internal fixation and evaluated the clinical outcome. Between 2005 and 2011, eleven patients with elbow stiffness and distal humeral nonunion underwent open arthrolysis, surgical reduction, internal fixation, hinged external fixation, and selective bone grafting. The ulnar nerve was anteriorly transposed i

Ipsilateral total shoulder and elbow arthroplasties in patients who have rheumatoid arthritis.

BACKGROUND: The data on seventeen patients with rheumatoid arthritis who had been managed with ipsilateral total shoulder and elbow arthroplasties were analyzed to determine whether the operative technique, the presence of total shoulder and total elbow prostheses in the same upper extremity, or complications of the arthroplasties affected the result in each joint or the overall functional outcome of the upper extremity. METHODS: Seventeen patients with rheumatoid arthritis who were managed with a total of eighteen ipsilateral total shoulder and elbow arthroplasties were evaluated. The most r

Squatting Throwing Is Not Associated With Increased Medial Elbow Torque in Youth Baseball Catchers.

PURPOSE: To compare medial elbow stress during squatting throwing, standing throwing, and maximum effort throwing and to analyze its relationships with throwing variables and the hip joint internal rotation (IR) range of motion (ROM) in youth baseball catchers. METHODS: Forty-five youth baseball catchers were recruited. Exclusion criteria included pain during throwing, history of surgery, or upper-extremity injuries within 12 months. Catchers performed 5 throws for each of the following throwing conditions: squatting throwing, standing throwing, and maximum effort throwing. Catchers were outf

[Elbow dislocation in childhood. Long-term observational study].

Traumatic dislocation of the elbow is rare in children with an incidence of 3-6% of all elbow injuries. In the literature the outcome after elbow dislocation in childhood is rarely discussed. In the present study 33 children treated in our clinic from 2001 to 2008 with an acute traumatic dislocation of the elbow were retrospectively included. All events were unilateral whereby 1 child (3%) showed a recurrence of elbow dislocation after 9 weeks, 30% had a pure dislocation, 70% had a concomitant fracture, 55% showed a fracture of the medial epicondyle, 6% a fracture of the lateral epicondyle and

Open reduction for late-presenting posterior dislocation of the elbow.

PURPOSE: To evaluate results of open reduction for late-presenting (more than 3 weeks) posterior dislocation of the elbow in 10 patients. METHOD: Elbow stiffness was the main indication for surgery. The mean age of the patients was 34 (range, 13-65) years; the mean time since injury was 4 (range, 2-6) months. All patients had non-functional elbow movement for any activity of daily living. Three patients had associated fractures around the elbow joint. RESULTS: At a mean follow-up of 19 (range, 11-28) months, 8 patients regained a functional range of movement for activities of daily living an…

Using the American alligator and a repeated-measures design to place constraints on in vivo shoulder joint range of motion in dinosaurs and other fossil archosaurs.

Using the extant phylogenetic bracket of dinosaurs (crocodylians and birds), recent work has reported that elbow joint range of motion (ROM) studies of fossil dinosaur forearms may be providing conservative underestimates of fully fleshed in vivo ROM. As humeral ROM occupies a more central role in forelimb movements, the placement of quantitative constraints on shoulder joint ROM could improve fossil reconstructions. Here, we investigated whether soft tissues affect the more mobile shoulder joint in the same manner in which they affect elbow joint ROM in an extant archosaur. This test involved…

Minimally invasive treatment of divergent elbow dislocation in a dog.

The aim of this case report was to present the outcomes of minimally invasive surgical treatment of a rare and atypical form of an elbow joint luxation, known as 'divergent dislocation of the elbow' in a dog. Initially, a closed reduction of the dislocated elbow joint was performed, then surgical treatment was decided. A small incision was made on the caudal side of the ulna, then a 2 mm Kirschner wire was inserted from the caudal aspect of the proximal ulna to the radial neck. Following that, a 20 mm, 2.4 mm cortical screw was placed, securing the luxated radius to the ulna. All the procedure…

Evidence for strength imbalances as a significant contributor to abnormal synergies in hemiparetic subjects.

Abnormal synergies in the paretic shoulder and elbow of hemiparetic subjects were quantified during maximal voluntary contractions (MVCs) in 27 subjects with a history of stroke and 8 age-matched control subjects. A six-axis load cell allowed simultaneous measurement of the primary torque the subject was attempting to maximize and the secondary torques at other joint actions. For example, during MVC of shoulder flexion, shoulder flexion is the primary torque and the secondary torques are internal/external rotation, abduction/adduction, and elbow flexion/extension. In general, the stroke subjec

Outcome of Long Standing Ulno-Humeral Dislocation Managed by Open Reduction and Stabilization with V-Y Plasty; Our Experience.

OBJECTIVE: To assess the outcome of long standing ulno-humeral dislocation managed by open reduction and stabilization with V-Y plasty. METHODS: This cross sectional study included 14 patients with elbow dislocation that was more than 3 months without joint ankylosis, myositis ossificans or nerve injury. The surgical management not only included open reduction of the elbow joint, but also elongation of the triceps aponeurosis, collateral ligament repair along with the stabilization of the elbow joint. The results were analyzed using the Mayo Elbow score and the range of motion at 3 months and…

Traumatic elbow instability.

Trauma can render the elbow unstable via a combination of bone and ligament injuries. Some of these injuries feature subluxation rather than dislocation of the elbow. Effective treatment centers on restoring enough of the bony and ligamentous structures to keep the elbow in joint so that recovery can proceed as for a simple elbow dislocation. Recognition of distinct patterns of injury can help determine the structures injured and the best methods for repairing them.

Fracture-dislocation of the elbow: diagnosis, treatment, and prognosis.

There are two main goals for the surgeon treating injuries around the elbow joint: the maintenance of a stable, concentric reduction of the joint and the initiation of early motion to help prevent stiffness and maximize functional out-come. The presence of a fracture decreases joint stability and increases the risk for early subluxation or dislocation with motion. Operative repair thus is indicated for most of these injuries to restore sufficient osseoligamentous support to allow safe, early motion and provide a stable functional elbow in the long term.

Arthroscopic fenestration of the olecranon fossa in the treatment of osteoarthritis of the elbow.

A new arthroscopic approach to the treatment of elbow osteoarthritis that permits debridement of the elbow joint together with the removal of intraarticular loose bodies is described. Twelve patients (11 male, one female) with an age range of 29-64 (mean 47.5 years) underwent the procedure. Post-surgery all patients had relief of elbow-locking and experienced a reduction in elbow pain (follow-up 3-30 months, mean 16 months). The technique is offered as a treatment option for patients with elbow osteoarthritis.

Changes in movement variability and task performance during a fatiguing repetitive pointing task.

Changes in neuromuscular strategies employed with fatigue during multi-joint movements are still poorly understood. Studies have shown that motor variability of individual joints increases when performing upper limb tasks to fatigue, while movement parameters related to the task goal remain constant. However, how the inter-limb coordination and its variability change during specific movement phases with fatigue is still unclear. The aim of this study was to assess the effects of neck-shoulder fatigue on shoulder and elbow kinematic variabilities, shoulder-elbow coordination and its variability

A robot-aided visuomotor wrist training induces motor and proprioceptive learning that transfers to the untrained ipsilateral elbow.

BACKGROUND: Learning of a visuomotor task not only leads to changes in motor performance but also improves proprioceptive function of the trained joint/limb system. Such sensorimotor learning may show intra-joint transfer that is observable at a previously untrained degrees of freedom of the trained joint. OBJECTIVE: Here, we examined if and to what extent such learning transfers to neighboring joints of the same limb and whether such transfer is observable in the motor as well as in the proprioceptive domain. Documenting such intra-limb transfer of sensorimotor learning holds promise for the

Relationship between shoulder rotation deficit and elbow instability in healthy baseball players.

BACKGROUND: Elbow valgus instability is a risk factor for elbow medial collateral ligament injury. This study aimed to investigate the relationship between shoulder range of motion and elbow valgus instability using an ultrasound imaging system. A questionnaire and ultrasound examination were used for the study. METHODS: Thirty-seven high school baseball players (15-18 years) with no history of shoulder or elbow disorders were included. Shoulder range of motion was measured at 90° of adduction, 10° of horizontal adduction, and 90° of elbow flexion in the supine resting position. To evaluate e

Posterior dislocation of the elbow as an unusual presentation after a total hip replacement: a case report.

INTRODUCTION: Posterior dislocation of the elbow is usually associated with trauma to the joint with a reported incidence of 3%to 6%. Chronic instability is usually symptomatic following the initial injury. CASE PRESENTATION: We report a case of posterior dislocation of the elbow occurring in a patient while using her arm to lift herself using a monkey pole on the second day following a total hip replacement. The dislocation was reduced under sedation in the ward. There were no signs or symptoms suggesting any joint hypermobility syndrome in the patient. Follow up 4 months following the injur

Clinical effectiveness and safety of intraarticular administration of a 117m Tin radiocolloid (Synovetin OATM ) for treatment of early and intermediate grade osteoarthritis of the elbow in a dose finding study conducted in 44 dogs.

Osteoarthritis of the elbow joint secondary to elbow dysplasia is common in dogs. Intraarticular radionuclide injection is thought to suppress both synovitis and inflammatory pain mediators in the joint which are not directly addressed by current treatments. This dose-finding investigation was a longitudinal, prospective, experimental parallel group, post-test study with repeated measures. Forty-four dogs, with low to intermediate-grade osteoarthritis, received a single injection into their most clinically affected elbow joint and were randomized into three treatment cohorts; 37 MBq, 64.75 MBq…
