Predictive Value of Single-Fiber Electromyography for Ocular Myasthenia Gravis in a Low Prevalence Cohort.

PURPOSE: Single-fiber electromyography (SFEMG) is a sensitive test to diagnose ocular myasthenia gravis (OMG) when the clinical suspicion is high but the predictive value of the test is also dependent on the prevalence of the condition in the referral population. In a low prevalence population, false positive results can be as common as true positive results. Most of the patients referred for SFEMG at our center do not have symptoms or signs that are strongly suggestive for OMG. METHODS: A retrospective chart review was performed of all patients referred to our laboratory for possible OMG for…

The Anterior Belly of the Digastric Muscle as a Novel Target for Needle Electromyography in Amyotrophic Lateral Sclerosis.

INTRODUCTION/AIMS: Needle electromyography (EMG) in the bulbar region aids in the diagnosis of amyotrophic lateral sclerosis (ALS) but remains technically challenging. The genioglossus muscle (GM) is commonly examined; however, pain and poor relaxation may limit its evaluation. The trapezius is an alternative target but is innervated by the spinal accessory nerve. We investigated the utility of the anterior belly of the digastric muscle (ABDM), innervated by the trigeminal nerve, for bulbar EMG evaluation. METHODS: In this unblinded, prospective observational study, consecutive patients with …

Ultrasound Depiction of the Optimal Window for Needle Placement for Electromyography of the Short Head of the Biceps Femoris.

INTRODUCTION/AIMS: Accurate needle electromyography (EMG) of the short head of the biceps femoris (SHBF) is often important to localize nerve damage when proximal to the fibular head. Techniques have been proposed for accurate SHBF EMG; however, needle placement accuracy is low and with the risk of inadvertent needle placement into the long head (LHBF) potentially resulting in false localization and inappropriate operative management. This study aimed to define the optimal window for accurate SHBF needle EMG placement through ultrasound (US) investigation of the anatomical relationships of the…

Surface-electromyography characteristics of clonic seizures with no scalp-EEG correlate: A comparative analysis with tremors.

INTRODUCTION: Clonic seizures are characterized by twitching movements at a frequency of 0.2-5 Hz. The clonic "twitch" is produced by a brief synchronized contraction of agonist and antagonist muscles, followed by a synchronized silent period. In this study, we aimed to compare the surface-electromyography (sEMG) characteristics of scalp-EEG negative clonic seizures with those of nonepileptic movements like tremors that can resemble clonic seizures. METHODS: We retrospectively identified patients who were diagnosed with scalp-EEG negative clonic seizures or tremors. We only included patients …

Medium- and Long-Term Effectiveness of Custom Insoles for Cavus Foot: A Surface Electromyography Study.

Background: Cavus foot, defined by an increased medial longitudinal arch and often forefoot plantarflexion, alters biomechanics and increases peak plantar pressures, raising the risk of musculoskeletal disorders such as metatarsalgia, Achilles tendinopathy, and gait instability. Custom foot orthoses are the preferred conservative treatment, offering plantar support, pressure redistribution, and reduction in compensatory muscle activity. This study evaluated the medium- and long-term effectiveness of custom orthoses in 71 patients with cavus feet using surface electromyography (sEMG) and the sa…

Comparison of electromyography, sound, bioimpedance, and high-resolution manometry for differentiating swallowing and vocalization events.

OBJECTIVES: Non-invasive surface recording devices used for detecting swallowing events include electromyography (EMG), sound, and bioimpedance. However, to our knowledge there are no comparative studies in which these waveforms were recorded simultaneously. We assessed the accuracy and efficiency of high-resolution manometry (HRM) topography, EMG, sound, and bioimpedance waveforms, for identifying swallowing events. METHODS: Six participants randomly performed saliva swallow or vocalization of "ah" 62 times. Pharyngeal pressure data were obtained using an HRM catheter. EMG, sound, and bioimp…

Diagnostic limitation of laryngostroboscopy in comparison to laryngeal electromyography in synkinesis in unilateral vocal fold paralysis.

PURPOSE: In clinical practice, laryngo(strobo)scopy (LS) is still mainly used for diagnostics and management of unilateral vocal fold paralysis (UFVP), although only laryngeal electromyography (LEMG) can provide information on causes of vocal fold immobility, especially on possible synkinetic reinnervation after recurrent laryngeal nerve (RLN) injury. The goal of this retrospective study was the evaluation whether signs of synkinetic reinnervation in LS can be objectified in comparison to LEMG data. METHODS: Between 1/2015 and 2/2018, 50 patients with laryngostroboscopically suspected UVFP re…

Ultrasound, electromyography, and balloon guidance for injecting botulinum toxin for cricopharyngeal achalasia: A case report.

INTRODUCTION: Botulinum toxin (BTX) injection is a widely used treatment option for dysphagia associated with cricopharyngeal (CP) muscle achalasia, but uniform standards and protocols for administration techniques and injection sites are still lacking. This case study suggests that a unique administration technique involving a combination of ultrasound, electromyography, and balloon guidance for injecting the CP muscle can reduce inadvertent migration of BTX to non-injected tissues and increase the effectiveness and safety of BTX treatment. PATIENT CONCERNS: We describe the case of a 74-year…

Improved hand prostheses control for transradial amputees based on hybrid of voice recognition and electromyography.

The control of prostheses and their complexities is one of the greatest challenges limiting wide amputees' use of upper limb prostheses. The main challenges include the difficulty of extracting signals for controlling the prostheses, limited number of degrees of freedom (DoF), and cost-prohibitive for complex controlling systems. In this study, a real-time hybrid control system, based on electromyography (EMG) and voice commands (VC) is designed to render the prosthesis more dexterous with the ability to accomplish amputee's daily activities proficiently. The voice and EMG systems were combine…

Electromyography and muscle biopsy in paediatric neuromuscular disorders - Evaluation of current practice and literature review.

The conduct and interpretation of electromyography in children is considered difficult and therefore often avoided. We assessed the diagnostic accuracy of the paediatric electromyography protocol used in our tertiary reference centre and compared it to muscle biopsy results and clinical diagnosis. Electromyography was performed in unsedated children with suspected neuromuscular diseases between January 2010 and September 2017 and was analysed quantitatively. Muscle pathology was classified into seven groups based on existing histopathology reports. The clinical diagnosis, including myopathic, …

Electromyography and Mechanomyography Signals During Swallowing in Healthy Adults and Head and Neck Cancer Survivors.

Surface electromyography (sEMG) is used as an adjuvant to dysphagia therapy to demonstrate the activity of submental muscles during swallowing exercises. Mechanomyography (MMG) has been suggested as a potential superior alternative to sEMG; however, this advantage is not confirmed for signal acquired from submental muscles. This study compared the signal-to-noise ratio (SNR) obtained from sEMG and MMG sensors during swallowing tasks, in healthy participants and those with a history of head and neck cancer (HNC), a population with altered anatomy and a high incidence of dysphagia. Twenty-two he…

Determination of the normative values of the masseter muscle by single-fiber electromyography in myasthenia gravis patients.

The purpose of this study is to obtain normative values of the masseter muscle of myasthenia gravis (MG) patients and healthy volunteers by single-fiber electromyography (SFEMG). Stimulation of SFEMG in the masseter muscle was studied in 15 healthy volunteers (men 8, women 7; mean age 40.2, range 21-77) and 30 patients affected by MG (men 16, women 14; mean age 42.8, range 12-75). The mean consecutive difference (MCD) of the individual fiber and the mean MCD per study were determined in the normal group. We recommend the upper normal limit for the individual fibers of jitter and the mean MCD p…
