Twenty-two new 1-[2-oxo-2-(4-phenylpiperazin-1-yl)ethyl]pyrrolidine-2,5-diones were synthesized and tested for anticonvulsant activity. Initial anticonvulsant screening was performed using standard maximal electroshock (MES) and subcutaneous pentylenetetrazole (scPTZ) screens in mice. Lorazepam reduced conflict behavior in rats and monkeys, inhibited pentylenetetrazol- and electroshock-induced convulsions, suppressed footshock-induced fighting behavior, and prevented morphine-induced stimulation in mice at lower doses than other benzodiazepines tested. This behavioral profile suggests that lorazepam will be an active anti-anxiety agent in man at low doses.

Three 16-point shapes were shown to 15 patients before undergoing either unilateral or bilateral electroconvulsive therapy. They were then tested for recognition following therapy. The same subjects were similarly tested for recognition memory with no intervening shock. Analysis of variance indicated no significant differential effects of the two methods of electroshock application.

Premedication with PCPA antagonized in rats the anticonvulsant activity of Primidone and of other drugs against seizures evoked by electroshock. Only in the case of Primidone, however, the anticonvulsant activity could not be re-established by increasing the dosage. Our investigations have shown that PCPA caused a strong inhibition of the conversion of Primidone to phenobarbital, both in vivo and in vitro.

Twelve new N-aminosuccinimides were synthesized by the condensation of hydrazines with succinic anhydrides in glacial acetic acid. The compounds were evaluated in the maximal electroshock seizure and subcutaneous pentylenetetrazol seizure threshold tests for anticonvulsant activity and in the rotorod test for neurotoxicity in mice. The lowest dose at which several of the compounds exhibited anticonvulsant activity was 300 mg/kg.

Nicotine administered acutely at subconvulsive dose of 4 mg/kg, significantly decreased the protective activity of valproate, carbamazepine, diphenylhydantoin, phenobarbital, topiramate and lamotrigine against maximal electroshock-induced tonic convulsions in mice. The obtained data may suggest that interaction between nicotine and antiepileptic drugs should be carefully considered as a cause of the therapeutic failure in epileptic patients.

In reaction of alpha-phenyl, alpha-p-chlorophenyl and alpha-m-chlorophenylsuccinic acid with various aminopyridines, N-pyridyl-substituted succinimides (compounds 1-14) were obtained. These compounds were investigated for their CNS activity. Compounds 1, 2, 5, 6 and 7 displayed anticonvulsant properties in the maximum electroshock test. A variety of acute supraventricular tachycardias may be encountered. In many instances the therapies for these rhythm disturbances overlap, but a rational approach to individual disturbances should be based on an understanding of the anatomy and physiology involved in the individual dysrhythmia. Numerous investigative approaches are underway at present, especially with regard to interruption of arrhythmia pathways by electroshock therapy or surgical therapy.
