Family quality of life and associated factors among people with leprosy or podoconiosis/lymphatic filariasis with visible disabilities and their family members in Ethiopia: a baseline study for a randomized controlled trial.

Leprosy, podoconiosis and lymphatic filariasis (LF) cause impairment and disability affecting family quality of life (FQoL). The purpose of this study was to assess and compare FQoL and associated factors in people in Ethiopia with disabilities caused by leprosy, FL or podoconiosis and their family members. A cross-sectional study was conducted in the East and West Gojjam Zones of Ethiopia. Study participants with visible disabilities were selected by simple random sampling; however, family members were selected purposively. Data were collected using the FQoL, SARI, PHQ-9 and Participation Sca

Development of an integrated, holistic care package for people with lymphoedema for use at the level of the Primary Health Care Unit in Ethiopia.

BACKGROUND: Neglected Tropical Diseases (NTDs) are a group of several communicable and non-communicable diseases prevalent in tropical and subtropical areas. The co-endemicity of these diseases, the similarity of their clinical signs, and the need to maximize limited financial and human resources suggest the importance of adoptingan integratedapproach to their prevention and treatment. AIMS: This study describes the development of a comprehensive package of physical, mental health and psychosocial care for people with lower-limb lymphoedema caused bypodoconiosis, lymphatic filariasis (LF)or l

Quantifying the socio-economic impact of leg lymphoedema on patient caregivers in a lymphatic filariasis and podoconiosis co-endemic district of Ethiopia.

BACKGROUND: Lymphoedema caused by lymphatic filariasis (LF) or podoconiosis can result in physical disability and social exclusion, which is exacerbated by painful acute dermatolymphangioadenitis (ADLA) episodes. These conditions have a significant impact on patients, however, little is known about the indirect effects on their caregivers. This study, therefore, aimed to determine the impact on caregivers for patients with leg lymphoedema in a co-endemic district of Ethiopia. METHODOLOGY/PRINCIPAL FINDINGS: A cross-sectional survey of lymphoedema patients and their caregivers was conducted us

Patrick Manson: A Physician Pioneer in Parasitology Research.

Patrick Manson, a physician-scientist who worked in China between 1866 and 1889, made significant discoveries about the transmission of tropical infectious diseases. He identified that many such diseases require a vector, unique to warmer climates, to facilitate human-to-human transmission. Manson demonstrated that microfilariae have nocturnal periodicity in the blood of patients with elephantiasis. He showed that these microfilariae undergo transformation on ingestion by mosquitoes, which act as vectors to complete their life cycle. By observing operculated eggs in the sputum samples, Manson …

Unravelling the Biological Activities of the Byttneria pilosa Leaves Using Experimental and Computational Approaches.

Byttneria pilosa is locally known as Harijora, and used by the native hill-tract people of Bangladesh for the treatment of rheumatalgia, snake bite, syphilis, fractured bones, elephantiasis and an antidote for poisoning. The present study was carried out to determine the possible anti-inflammatory, analgesic, neuropharmacological and anti-diarrhoeal activity of the methanol extract of B. pilosa leaves (MEBPL) through in vitro, in vivo and in silico approaches. In the anti-inflammatory study, evaluated by membrane stabilizing and protein denaturation methods, MEBPL showed a significant and dose…

Functional genomics in Brugia malayi reveal diverse muscle nAChRs and differences between cholinergic anthelmintics.

Many techniques for studying functional genomics of important target sites of anthelmintics have been restricted to Caenorhabditis elegans because they have failed when applied to animal parasites. To overcome these limitations, we have focused our research on the human nematode parasite Brugia malayi, which causes elephantiasis. Here, we combine single-cell PCR, whole muscle cell patch clamp, motility phenotyping (Worminator), and dsRNA for RNAi for functional genomic studies that have revealed, in vivo, four different muscle nAChRs (M-, L-, P-, and N-). The cholinergic anthelmintics had diff…

Filarial parasites develop faster and reproduce earlier in response to host immune effectors that determine filarial life expectancy.

Humans and other mammals mount vigorous immune assaults against helminth parasites, yet there are intriguing reports that the immune respons … l nematodes, the parasites responsible for debilitating diseases in humans like river blindness and elephantiasis, accelerate their development in response to the IL-5 driven eosinophilia they encounter when infecting a host. Consequently they produce microfilariae, their transmission stages, earlier and in greater numbers. Eosinophilia is a primary host determ …

Lymphatic filariasis in a rural community in Nigeria: a challenge ahead.

Lymphatic filariasis (LF) represents a major public health problem in tropical and subtropical regions of the world. Following the admission … d the mean MF count in males (22.8mf/ML) was higher than females (21.7mf/ML). The prevalent rate of elephantiasis and hydrecoele observed in the community was 2.2% respectively. The result of this study confirms and observed a high prevalence rate of LF in this community. This finding establishes the need for Ministry of Health (MOH) and Global Alliance to Elim …

Localized eosinophil degranulation mediates disease in tropical pulmonary eosinophilia.

To explore the mechanisms underlying the eosinophil-mediated inflammation of tropical pulmonary eosinophilia (TPE), bronchoalveolar lavage (BAL) fluid, serum, and supernatants from pulmonary and blood leukocytes (WBC) from patients with acute TPE (n = 6) were compared with those obtained from healthy uninfected individuals (n = 4) and from patients with asthma (n = 4) or elephantiasis (n = 5). Although there were no significant differences in the levels of interleukin-4 (IL-4), IL-5, IL-13, eotaxin, granulocyte-macrophage colony-stimulating factor, RANTES, or eosinophil cationic protein, there…

The abundant larval transcript-1 and -2 genes of Brugia malayi encode stage-specific candidate vaccine antigens for filariasis.

Lymphatic filariasis is a major tropical disease caused by the mosquito-borne nematodes Brugia and Wuchereria. About 120 million people are infected and at risk of lymphatic pathology such as acute lymphangitis and elephantiasis. Vaccines against filariasis must generate immunity to the infective mosquito-derived third-stage larva (L3) without accentuating immunopathogenic responses to lymphatic-dwelling adult parasites. We have identified two highly expressed genes, designated abundant larval transcript-1 and -2 (alt-1 and alt-2), from each of which mRNAs account for >1% of L3 cDNAs. ALT-1 an…

Gender-specific gene expression in Brugia malayi.

Brugia malayi is a mosquito-borne filarial nematode that causes lymphatic filariasis and elephantiasis in humans. The purpose of this study was to identify and characterize genes that are expressed differentially in male and female B. malayi in hopes of gaining new insight into the reproductive biology of the parasite. Two approaches were used. A 5' differential display PCR (splice leader differential display PCR, SL DD-PCR) was performed by PCR with splice leader and random primers on cDNA templates, and electronic subtraction was performed on expressed sequence tag (EST) cluster databases de…
