Anatomic Considerations for Epicardial and Endocardial Left Atrial Appendage Closure.

Left atrial appendage closure is an increasingly used means of achieving thromboprophylaxis in atrial fibrillation, particularly in patients with contraindications to anticoagulation. Left atrial appendage anatomy is highly variable, and preprocedural imaging is critical to choosing the correct device and approach for left atrial appendage closure. This article reviews the common endocardial and epicardial closure systems, including anatomic considerations, advantages and disadvantages, as well as complications to be avoided.

Human subpulmonary infundibulum has an endocardial network of specialized conducting cardiomyocytes.

BACKGROUND: The right ventricular outflow tract is the most common source of ventricular arrhythmias in nonstructural heart disease. Most of these arrhythmias are of endocardial origin, but their morphologic substrate is mostly unknown. OBJECTIVE: The purpose of this study was to identify potential morphologic substrates for such arrhythmias originating within the right ventricular outflow tract. METHODS: Three adult human hearts that had been fixed in 4% formaldehyde were examined. In 2 of the hearts, which were obtained subsequent to necropsies, the base of the anterior papillary muscle of

Contemporary and future trends in cardiac resynchronization therapy to enhance response.

The rationale for cardiac resynchronization therapy (CRT), expectations in terms of patient benefit, patient selection for CRT, selection of a CRT pacemaker (CRT-P) vs CRT plus implantable cardioverter-defibrillator (CRT-D) platform, and studies evaluating device programming to enhance benefit from CRT are reviewed. The notion of an "optimal" left ventricular (LV) pacing site, the rationale for identifying and avoiding LV pacing in regions of scar, the use of anatomic, hemodynamic, and electrical parameters to identify an optimal LV pacing site, and the potential utility of multisite LV pacing

Use of contrast enhancement for the assessment of left ventricular function.

Endocardial visualization is essential for accurate interpretation of regional wall thickening abnormalities, which constitute the diagnostic hallmark of coronary artery disease, and for reproducible assessment of left ventricular (LV) ejection fraction. Unfortunately, in many cardiac patients, poor image quality does not allow consistent visualization, let alone automated detection, of the endocardial boundary in all segments throughout the cardiac cycle. Continuing efforts directed towards improving endocardial visualization made by both echocardiographic imaging equipment manufacturers and

Alternative locations for internal defibrillator electrodes.

Successful defibrillation is described in two patients in whom the defibrillating electrode was positioned in the coronary sinus and right ventricular outflow tract as alternative sites. Internal cardiac defibrillation has been successful with single or multiple endocardial electrodes, epicardial patch electrodes, and subcutaneous surface electrodes (patch, array) in varying combinations and recently with an active can electrode. While the traditional location of the endocardial electrode has been the right ventricular apex, we describe two patients in whom defibrillation was successful in alt

Effects of endocardial endothelium in myocardial mechanics of hypertrophied myocardium of rats.

AIM: To investigate the effects of endocardial endothelium (EE) in myocardial mechanics of hypertrophied myocardium of rats. METHODS: Hypertrophied myocardium has been developed through long-term renovascular hypertension in rats. Selective stripping of EE was achieved chemically and the effects of EE on the activation and relaxation of hypertrophied myocardium were investigated and compared to those in the presence and absence of intact EE. RESULTS: Resting tension (RT) and peak developed tension (DT) of isometric contraction of the left ventricular papillary muscles of renovascular hyperte

Heterophil nature of EVI antibody in Trypanosoma cruzi infection.

Previous findings implying the autoreactive capability of circulating complement-fixing (EVI) antibodies in patients with Chagas' disease (American trypanosomiasis), under both in vitro and in vivo conditions have been irreproducible. The presence of the tissue antigens involved in the reactivity with EVI antibodies is exclusively confined to nonhuman substrates, suggesting the heterophil nature of these antibodies which, on the other hand, have also been described in human sera collected in areas free from Chagas' disease. This would preclude any direct pathogenic effect of EVI antibodies whe

Wall thickness changes considered as regional myocardial function in ischemic heart disease.

Dynamic changes in left ventricular wall thickness represent a function of the myocardium which can be described, in normal subjects or in disease, without reference to behavior of other parts of the wall. Abnormalities of amplitude, rate and timing of wall thickness change interact in influencing overall ventricular performance, so that even this local function cannot be expressed as a single "index". Infarcted and acutely ischemic myocardium display reduced amplitude of thickness change. The degree to which they also show delayed wall dynamics in man is uncertain. Cross-sectional echocardiog

Sudden death due to undiagnosed cardiac metastasis of malignant melanoma revealed by autopsy: a case report.

Cardiac metastases from malignant melanoma are rare and often remain undiagnosed until autopsy due to their nonspecific clinical presentatio … tic melanoma that included the lungs, liver, spleen, lymph nodes and notably the myocardium and the endocardium. Importantly, no cutaneous or mucosal primary site was identified, classifying this case as melanoma of unknown primary (MUP). The histological and immunohistochemical analysis confirmed the diagnosis. This case report summarises the range of diagno …

Injectable hydrogel electrodes as conduction highways to restore native pacing.

There is an urgent clinical need for a treatment regimen that addresses the underlying pathophysiology of ventricular arrhythmias, the leadi … ng data demonstrated that the activation wavefront from the hydrogel reaches the mid-myocardium and endocardium much earlier than current single-point pacing modalities. Such uniform activation of broad swaths of tissue enables an opportunity to minimize the delayed myocardial conduction of heterogeneous tissue that underpins re-entry. Collectively, these stu …

In-Depth Review of Loeffler Endocarditis: What Have We Learned?

Loeffler endocarditis, eosinophilic endocarditis or eosinophilic endomyocardial disease are conditions associated with hypereosinophilia and they affect the heart function. Loeffler endocarditis is a rare endomyocardial disorder thought to be caused by eosinophilic damage. The disorder is characterized by inflammatory infiltration, formation of thrombus within cardiovascular system, and ultimately fibrosis of the afflicted area. It can lead to multiple severe complications, including thromboembolic disease, thickening of fibrous tissue in the endocardium of ventricles, valve involvement, apica…
