Early Versus Delayed Pancreatic Enzyme Replacement Therapy After Pancreatic Cancer Surgery: A Real-World Cohort Study.

OBJECTIVES: The optimal timing of pancreatic enzyme replacement therapy (PERT) after pancreatic surgery for pancreatic cancer remains uncertain. We compared outcomes after early versus delayed PERT initiation in a real-world cohort. METHODS: This retrospective cohort study used the TriNetX US Collaborative Network database. Adults with pancreatic cancer undergoing pancreatic surgery were categorized as early PERT if treatment was started within 1 month after surgery, and late PERT if started between 2 and 3 months after surgery. Outcomes were assessed from 90 to 730 days after the index event

Enzyme-Guided Assembly Enables Precision Delivery of a Singlet Oxygen Battery for Membrane Lipid Peroxidation Therapy.

Lipid peroxidation (LPO) of tumor cell membranes is a pivotal executor of regulated cell death and a potent trigger for antitumor immunity. However, most LPO inducing strategies either depend on external irradiation and adequate oxygen supply or require broad disruption of the glutathione (GSH)/glutathione peroxidase 4 (GPX4) antioxidant axis. As a result, their performance is often compromised by limited tissue penetration, heterogeneous tumor microenvironments, insufficient tumor selectivity with potential off target toxicity, and adaptive anti-ferroptotic responses that blunt sustained LPO

Autophagy-mediated amplification of ferroptosis via a curcumin-loaded metal-enzyme nanoplatform for synergistic tumor therapy.

Ferroptosis is an iron-mediated cell death process driven by lipid peroxidation, yet its antitumor potential is often counteracted by the limited endogenous H2O2 content, the strict catalytic conditions required for the Fenton reaction, and protective autophagy. Notably, sustained autophagy drives ferritin degradation and iron release, thereby amplifying ferroptotic signaling. To exploit this mechanism, we designed a CD44-targeted nanoplatform, Cur@MG@HA, using hyaluronic acid for tumor-specific delivery. The system co-delivers glucose oxidase (GOx) and curcumin (Cur) within a Fe-Cu MOF. Follo

Efficacy of pancreatic enzyme replacement therapy on exocrine pancreatic insufficiency: a systematic review and meta-analysis.

Exocrine pancreatic insufficiency (EPI) results in impaired digestion of fat, protein, and carbohydrates due to inadequate pancreatic enzyme secretion and is commonly managed with pancreatic enzyme replacement therapy (PERT). However, the extent to which PERT improves objective measures of nutrient absorption across different etiologies of EPI has not been fully clarified. A comprehensive search of PubMed/MEDLINE, Embase, Scopus, Web of Science, the Cochrane Library, and Google Scholar was performed from inception through December 2025. Randomized controlled trials (RCTs) comparing PERT with p

Assessment of bone health and bone mineral density in patients with mucopolysaccharidosis receiving enzyme replacement therapy.

BACKGROUND: Most patients with mucopolysaccharidosis (MPS) experience skeletal involvement due to impaired bone remodeling, glycosaminoglycan (GAG) accumulation, reduced mobility, and nutritional deficiencies. These factors may predispose to low bone mass and increased fracture risk. This study assessed bone mineral density (BMD) using height-adjusted dual-energy X-ray absorptiometry (DXA) in pediatric MPS patients receiving enzyme replacement therapy (ERT) and examined its associations with clinical, functional, and biochemical parameters. METHODS: This cross-sectional study included 29 chil

Targeting InhA for Tuberculosis Therapy: A Review of Recent Advances in Enzyme Inhibition and Drug Development.

Mycobacterium Tuberculosis (Mtb) is developing resistance to available antibiotics, posing new challenges and making first-line drugs (Isoniazid, ethambutol, pyrazinamide) ineffective. Second-line medications are also losing effectiveness due to their high toxicity and limited availability, necessitating more advanced treatment in modern drug discovery and development. Tiliacorinine, 2'-nortiliacorinine, and griselimycin, derived from natural sources, paved the way for the development of compounds and approved drugs, such as bedaquiline, delamanid, pyrifazimine, proteomandid, and telacebec, wh

Predicting non-compliance to pancreatic enzyme supplementation therapy in chronic pancreatitis: A machine learning-based approach.

OBJECTIVES: Chronic pancreatitis results in pancreatic exocrine insufficiency (PEI), which is treated with pancreatic enzyme replacement therapy (PERT). Despite the clinical benefits, non-compliance to PERT is a frequent problem. Our study aimed at (a) identifying predictors of non-compliance to PERT using machine learning (ML) algorithms and (b) analyzing patient-reported reasons for non-compliance to PERT. METHODS: A prospective observational study was conducted at a high-volume tertiary care center on two independent cohorts of chronic pancreatitis patients on PERT. In Cohort 1 (1057 patie

Successful pregnancy outcome in a woman with cholesteryl ester storage disease treated with enzyme replacement therapy.

Cholesteryl ester storage disease (CESD) is a rare autosomal recessive metabolic disorder resulting from a deficiency of lysosomal acid lipase (LAL). It is characterized by the accumulation of cholesterol esters in various tissues, leading to atherosclerotic diseases or severe hepatic dysfunction in younger individuals. Pregnancy has remained an essential challenge for women with CESD because of the poor prognosis. Enzyme replacement therapy (ERT) using sebelipase alfa, a recombinant form of LAL, is effective in improving lipid profiles and reversing liver dysfunction in patients with CESD. Th

Clinical outcomes of exclusive enzyme therapy (laronidase) in a cohort of patients with mucopolysaccharidosis type I.

BACKGROUND: Mucopolysaccharidosis type I (MPS I), is an autosomal recessive disorder caused by a deficiency in the enzyme α-L-iduronidase (IDUA), leading to the accumulation of glycosaminoglycans (GAGs) in tissues. Early diagnosis and treatment [i.e., bone marrow transplantation and/or enzyme replacement therapy (ERT) with laronidase] are essential to prevent irreversible damage. The long-term effectiveness of exclusive ERT has been primarily described in attenuated phenotypes, while only a few cases have been reported in severe phenotypes. METHODS: This study is a retrospective analysis summ

Immunophenotype associated with high sustained antibody titers against enzyme replacement therapy in infantile-onset Pompe disease.

INTRODUCTION: The efficacy of enzyme replacement therapy (ERT) with alglucosidase alfa for infantile-onset Pompe disease (IOPD) is limited in some patients due to the development of high and sustained antibody titers (HSAT; ≥12,800). METHODS: We carried out detailed immunophenotyping of IOPD patients (n=40), including analysis of circulating cell populations by flow cytometry and plasma cytokines by multiplex array, to determine whether patients with HSAT have unique immunological characteristics compared to those with low titers (LT; <12,800). RESULTS: Compared to patients with LT, patients

Twin-bioengine self-adaptive micro/nanorobots using enzyme actuation and macrophage relay for gastrointestinal inflammation therapy.

A wide array of biocompatible micro/nanorobots are designed for targeted drug delivery and precision therapy largely depending on their self-adaptive ability overcoming complex barriers in vivo. Here, we report a twin-bioengine yeast micro/nanorobot (TBY-robot) with self-propelling and self-adaptive capabilities that can autonomously navigate to inflamed sites for gastrointestinal inflammation therapy via enzyme-macrophage switching (EMS). Asymmetrical TBY-robots effectively penetrated the mucus barrier and notably enhanced their intestinal retention using a dual enzyme-driven engine toward en

Chitosan-based oral hydrogel formulations of β-galactosidase to improve enzyme supplementation therapy for lactose intolerance.

β-Galactosidase supplementation plays an important role in the life of people with lactose intolerance. However, these formulations are rendered ineffective by the low pH and pepsin in the stomach and pancreatic proteases in the intestine. Therefore, it is necessary to develop oral transport systems for carrying this enzyme in the active form up to the intestine, where the lactose digestion occurs. In this research, a new hydrogel was developed that could potentially be used for enzyme supplement therapy. In this regard, the chitosan-based β-Gal formulations described in the manuscript are an
