Therapeutic brain stimulation for epilepsy.

DBS has been a possible therapy for epilepsy for more than 30 years, and now it is moving to the point of clinical utility. Animal models have shown efficacy of DBS at several brain regions, although not all animal studies have shown efficacy. Clinically, an array of sites have been explored, including the cerebellum, anterior nucleus of the thalamus, CM nucleus, hippocampus, subthalamic nucleus, brainstem, and corpus callosum; direct stimulation of the cortex has also been explored. Interest in evaluating these sites for treatment of epilepsy has been enhanced by the success of vagus nerve st…

Interview accuracy in partial epilepsy.

The statistical concept of accuracy has never been applied to verify the history data collected on seizure disorders by open format interview. We compared patients'/witnesses' descriptions of epileptic seizures with videotaped seizure characteristics and analyzed the accuracy (ACC), sensitivity (SN), specificity (SP), false-positive rate (FPR), and false-negative rate (FNR) of various components of the semiology in patients with partial epilepsy. Language disturbances, complex automatisms, and autonomic signs have high ACC and intermediate FNRs. This means that these manifestations are most ob…

Recent patents on epilepsy genetics.

The field of epilepsy genetics is contentious, particularly when it concerns the common epilepsies. More than a dozen loci have been suggested, the result of either linkage analysis and/or association analysis, but few of these findings have been replicated, let alone proven, and those that have are mostly for rare forms of epilepsy. Molecular genetics has revolutionised the understanding of epilepsy genetics and is beginning to have a significant clinical impact. Technological advances have resulted in new high-throughput approaches that promise to further our understanding of the molecular g…

Epilepsy in inherited metabolic disorders.

INTRODUCTION: The study of neurometabolic diseases is still in a prolonged preliminary stage. The catalogue of these diseases continues to grow; some known clinical syndromes have been subdivided into a number of variants once the genes that cause them have been identified, and at the same time new metabolic disorders have been discovered that aggravate or contribute to forms of epilepsy not previously classified as cerebral metabolic disorders. RESULTS: This review presents the basic principles underlying the recognition and treatment of epilepsy caused by neurometabolic diseases. These diso…

Oligoastrocytoma presenting with intractable epilepsy.

OBJECTIVE: Oligoastrocytomas (OA) are mixed gliomas with distinct oligodendroglial and astrocytic neoplastic components. Very little about OA has been reported in the intractable epilepsy population. METHODS: We undertook a retrospective review of 923 patients who underwent resective surgery for intractable epilepsy between 1996 and 2004. RESULTS: 6/923 (0.7%) patients were diagnosed with OA. Five patients were female, one was male. Median age at diagnosis was 25 years (range 19-44 years). Tumors arose from the left side in all patients and from the temporal lobe in five patients. Three pati…

Cerebral cavernous malformations and epilepsy.

Seizures and epilepsy are frequent clinical manifestations of cerebral cavernous malformations (CCMs) and represent the most common symptomatic presentation of supratentorial lesions. Clinicians often diagnose CCMs in patients after a first seizure, or in some cases after obtaining neuroimaging studies in patients suffering from chronic epilepsy previously thought to be idiopathic. In some cases, the lesion is clinically significant solely because of its epileptogenicity, but in others there may be concern about potential hemorrhage or focal neurological deficits from a similar lesion. The aut…

Hypersalivation in temporal lobe epilepsy.

PURPOSE: We sought to determine whether hypersalivation helps lateralize seizure onset during complex partial seizures of temporal lobe origin. Several clinical signs, which help lateralize seizure onset, have been reported in temporal lobe epilepsy (TLE). Increased salivation only occasionally has been reported as a manifestation of partial epilepsy. METHODS: Of 590 consecutive patients admitted for video-EEG monitoring, either as a part of a presurgical evaluation of medically intractable epilepsy or for diagnosis and clarification of their paroxysmal symptoms, we identified 10 patients wit…

Occipital epilepsy: lateral versus mesial.

This study compares ictal semiology, neurological examination and scalp EEG between lateral and mesial occipital epilepsy to assess the contribution non-invasive data make in determining the epileptogenic region within an occipital lobe. We assessed seizure origin in 41 occipital patients as lateral (11 patients), mesial (20) and both surfaces (10) as indicated by subdurally recorded seizures (nine), a lesion whose removal reduced seizure quantity by > or =90% (six), or who met both criteria (26). No aspect of semiology distinguished lateral from mesially originating occipital seizures. A pre-…

Posterior resection for childhood epilepsy.

We reviewed our experience with posterior resection for intractable childhood epilepsy. Fifteen patients, seven males and eight females, were studied. Age of surgery was 18 months to 16 years. Nine patients had a parietal resection and six patients had an occipital resection. Surgical outcome was variable. Nine patients had an excellent outcome (Engel Class I or II), two patients had a poor outcome (Engel Class III), and four patients had a very poor outcome (Engel Class IV). Pathology at surgery included focal cortical dysplasias (4), brain tumors (4), tubers of tuberous sclerosis (2), cerebr…

[Type 1 neurofibromatosis and epilepsy].

Epilepsy has been rarely reported with type 1 neurofibromatosis (Reckling Hausen disease). It may occur in 3 to 6% of cases. We report in this study three cases of patients with type 1 neurofibromatosis associated with epilepsy. The patients were respectively 23, 30 and 35 years old. Epilepsy was focal and complex in one patient, generalized in one case and simply focal in another one. Cerebral MRI showed sphenoidal dysplasia and temporal lobe ectopy compressing orbital structures in one patient and normal in the other cases. We discuss trough these three cases the relationship between Recklin…

Pediatric epilepsy - an Indian perspective.

Prevalence studies from India suggest that epilepsy prevalence is similar to developed nations. Neurocysticercosis (NCC) predominates as an etiology. A large treatment gap is still a public health problem. Benign epilepsies and West syndrome appear to be underrepresented in studies on classification of seizures/syndromes. Febrile seizures prevalence in India is similar to other countries and appear to be as benign. Risk factors of intractable epilepsy (IE) in Indian studies include early age of onset, neurodevelopmental abnormalities and certain seizure types. Perinatal injuries underlie many …

Diagnosing and predicting refractory epilepsy.

Over 30% of people with epilepsy will never achieve remission with antiepileptic drug (AED) therapy. These individuals are often severely disabled by their condition, have an unsatisfactory quality of life, and are at increased risk of sudden unexpected death. Early identification of refractory epilepsy would allow prompt referral to specialist services, where the diagnosis can be confirmed, seizures and syndromes classified, AED therapy optimized, and suitability for surgery assessed. Recent studies suggest that patients with symptomatic or cryptogenic epilepsy, those who experience multiple …

Excessive masturbation after epilepsy surgery.

Sexual behavior changes as well as depression, anxiety, and organic mood/personality disorders have been reported in temporal lobe epilepsy (TLE) patients before and after epilepsy surgery. The authors describe a 14-year-old girl with symptoms of excessive masturbation in inappropriate places, social withdrawal, irritability, aggressive behavior, and crying spells after selective amygdalohippocampectomy for medically intractable TLE with hippocampal sclerosis. Since the family members felt extremely embarrassed, they were upset and angry with the patient which, in turn, increased her depressiv…

Surgical Treatment of Refractory Epilepsy.

The extension of cortical resection to treat children with intractable epilepsy is one of the most exciting advances in pediatric neurology in the past two decades. Many children with epilepsy who previously had little hope have been given a new chance at life. Many patients who were not considered for surgery are now recognized to be excellent surgical candidates. Most notably, children with generalized seizures such as infantile spasms or gelastic seizures caused by hypothalamic hamartomas now have the opportunity to benefit from surgery. In adults, there is one goal for epilepsy surgery: fr…

Sleep, Sleep Apnea, and Epilepsy.

Sleep disorders occur commonly in patients with epilepsy, and can be responsible for symptoms of daytime somnolence and also can contribute to the intractability of epilepsy. The most important aspect of treating sleep disorders, especially sleep apnea, is the recognition of the problem. In a busy clinical practice, symptoms of sleep disorders are frequently overlooked or mistaken. Whenever sleep disruption or excessive daytime somnolence is potentially problematic, the patient should be referred to a sleep specialist and, if indicated, diagnostic testing performed (usually polysomnography wit…

Bone health in pediatric epilepsy.

Childhood and adolescence are critical periods of skeletal mineralization. Peak bone mineral density achieved by the end of adolescence determines the risk for later pathological fractures and osteoporosis. Chronic disease and medication often adversely affect bone health. Epilepsy is one of the most common neurological conditions occurring in persons under the age of 21. Epilepsy may affect bone in a number of ways. Restrictions of physical activity imposed by seizures; limitations on physical activity resulting from cerebral palsy, frequently present in patients with symptomatic epilepsy; an…

[Villous atrophy and idiopathic epilepsy].

Reports of an association between epilepsy and coeliac disease (CD) are not new. Chapmann reported a prevalence of epilepsy in CD of 5.5% and accepted a figure of 0.5% for the community. Prevalence of coeliac disease among epileptic patients is not well established. The aim of this study is to determine the prevalence of CD in epileptic patients for this 49 epileptic patients (31 male and 28 female) were selected between January 96 et June 99 to underwent fibroscopy with intestinal biopsy. Anti body to gluten were measured in most patients. The patients with villous atrophy underwent cranial c…
