INTRODUCTION: Lateral temporal lobe epilepsy (LTLE) is characterized by auditory auras and is often associated with genetic factors. Previous studies have identified various genes linked to LTLE, including LGI1. However, there remains a need to explore other genetic variants that contribute to the LTLE phenotype, particularly in the absence of LGI1 mutations.
METHODS: A cohort followed in our epilepsy center and diagnosed as LTLE with auditory aura was recruited to the study. We have performed whole exome sequencing data analysis of 19 patients using a two-step approach. In the first step, we

Objective. Epilepsy is characterized by dynamic disruptions in brain networks, yet the spatiotemporal organization of directional interactions underlying ictal transitions remains incompletely understood. This study investigates peri-ictal transition patterns in mesial temporal lobe epilepsy (MTLE) by characterizing directional connectivity dynamics from intracranial stereo-electroencephalography (SEEG) seizure recordings.Approach. We analyzed SEEG data from 10 patients with MTLE who subsequently achieved seizure freedom following surgical resection. We quantified directional connectivity usin

OBJECTIVE: To identify clinically meaningful patterns in ictal electroclinical features of focal epilepsy using a data-driven, unsupervised learning approach, and to assess whether such patterns can localize and lateralize the epileptogenic zone (EZ) more accurately than conventional electroclinical interpretation.
METHODS: Ictal scalp EEG and seizure semiology were systematically extracted using the SCORE system in 116 patients with drug-resistant focal epilepsy who subsequently underwent resective surgery. Unsupervised clustering was applied separately to EEG and semiology features using k-

Pharmacoresistance to anti-seizure medications (ASMs) remains a major unmet challenge in temporal lobe epilepsy (TLE), and occurs diversely, as classified to primary or acquired manner. The pathophysiological underpinnings of acquired pharmacoresistance remain elusive. Here, using a hippocampal kindling mouse model, we established that prolonged lamotrigine (LTG) treatment-either during or after kindling-induces broad-spectrum resistance to multiple ASMs, effectively recapitulating clinical patterns of acquired pharmacoresistance. Multimodal interrogation revealed hyperexcitability of subicula

INTRODUCTION: Approximately 20-30% of temporal lobe epilepsy (TLE) cases present with no visible abnormalities on conventional magnetic resonance imaging (MRI). When interictal electroencephalography (EEG) recordings are also persistently normal, the diagnosis and lateralization become major clinical challenges. We aimed to investigate whether automated hippocampal subfield volumetry could detect subtle structural alterations in these "electro-clinically silent" MRI-negative cases and to correlate these findings with seizure semiology.
METHODS: We analyzed 24 patients with clinically diagnose

Microvascular remodeling is implicated in the pathophysiology of drug-resistant temporal lobe epilepsy (TLE). However, characterizing these changes in adult human surgical tissue is methodologically limited by the accumulation of lipofuscin, an autofluorescent pigment that obscures microvascular structures and confounds standard automated quantification. To address this, we developed a dual-modality workflow optimized for archival human tissue obtained from drug-resistant TLE patients and one post-mortem control. This approach integrates hematoxylin and eosin (H&E) staining for precise anatomi

BACKGROUND AND OBJECTIVES: Temporal lobe epilepsy (TLE) co-occurs with interictal impairments in autonomic regulation and respiratory chemoreception, which have been related to higher risk of sudden unexpected death in epilepsy (SUDEP). Studies indicate that seizures lead to reorganization of central autonomic circuits extending beyond the temporal lobe. However, the relationship between brain network disruptions and interictal cardiorespiratory dysfunction remains largely unknown. The goal of this work was to test whether central autonomic network alterations underlie interictal cardiorespira

OBJECTIVE: In temporal lobe epilepsy (TLE), patients often present with neurobehavioral comorbidities encompassing affective and cognitive difficulties. Whereas the latter have been related to atypical connectivity of mesiotemporal and frontotemporal circuits, the brain basis of affective symptoms remains incompletely understood. Here, we assessed functional network substrates of affective symptoms and examined their relationship to structural magnetic resonance imaging (MRI), cognition, and clinical parameters.
METHODS: We studied 42 drug-resistant patients who underwent multimodal 3-T MRI a

Background/Objectives: Children with single-sided deafness (SSD) have normal hearing in one ear and are deaf in the other. Navigating complex auditory environments with SSD may cause reallocation of cognitive resources necessary for executive functioning (EF), adding potential cognitive burden to listening, though this is not well understood. To characterize EF in children with SSD, we compared their test performance and everyday functioning on performance-based and caregiver-rated EF measures to normative values and to a group of children with temporal lobe epilepsy (TLE). Methods: A retrospe

INTRODUCTION: Temporal lobe epilepsy is the most common form of focal epilepsy, often associated with cognitive impairments, particularly in memory functions, and depression. Sex and APOE ε4 genotype play a crucial role in modulating cognitive outcomes and depression in various neurological conditions like Alzheimer's disease. However, the combined effects of APOE genotype and sex on cognitive performance and depression in temporal lobe epilepsy have not been previously investigated.
OBJECTIVE: This study aims to (i) identify impaired cognitive performance and clinically relevant depression;

OBJECTIVE: Temporal lobe epilepsy (TLE) has been recognized as a network disorder with widespread gray matter atrophy. However, the role of connectome architecture in shaping morphological alterations and identifying atrophy epicenters remains unclear. Furthermore, individualized modeling of atrophy epicenters and their potential clinical applications have not been well established. This study aims to explore how gray matter atrophy correlates with normal connectome architecture, identify potential atrophy epicenters, and employ individualized modeling approach to evaluate the impact of differ

OBJECTIVE: To investigate the reliability of very high-frequency oscillations (VHFOs, 500-2000 Hz) as biomarkers in identifying the epileptogenic zone (EZ) in mesial temporal lobe epilepsy (MTLE) patients.
METHODS: We reviewed 19 MTLE patients who underwent stereo-electroencephalography-guided radiofrequency thermocoagulation. Interictal and ictal VHFOs were captured in four regions of interest (ROIs): amygdalohippocampal complex (AHC), parahippocampal gyrus (PG), white matter, and lateral temporal lobe. We analyzed the occurrence of VHFOs per minute, per contact: (a) in each ROI; (b) during

BACKGROUND: Ferroptosis is a form of regulated cell death that damages neurons in the central nervous system. In this study, we aimed to construct ferroptosis-related gene signatures in temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) and explore their diagnostic role in TLE-HS.
METHODS: The GSE205661 dataset was acquired for training purposes, while the GSE71058 was obtained to serve as the validation dataset. Subsequently, ferroptosis-related differentially expressed genes (FR-DEGs) in TLE-HS were further analyzed. We used weighed gene co-expression network analysis (WGCNA) algori

Mesial temporal lobe epilepsy (MTLE) is a common drug-resistant epilepsy, often requiring surgery to remove the epileptogenic zone (EZ). The mean efficiency of the surgery is 50-70 %, so the accurate spatial localization of the EZ remains a challenge. In a previous study from Hamelin et al. (2021), gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter, was shown to be highly increased in the EZ of a MTLE mouse model, with a concomitant decrease of Glx (Glutamate + glutamine). The authors proposed the GABA/Glx ratio as a potential specific biomarker of the EZ. As it is the only w

OBJECTIVE: The thalamus participates in seizure propagation and is a target for neuromodulation in epilepsy. Although thalamic connectivity changes have been reported in adults with temporal lobe epilepsy (TLE), pediatric TLE is distinct, characterized by greater etiological diversity and impact on brain maturation. This study applied a novel gradient mapping technique to investigate whole-brain thalamic connectivity patterns in children with TLE.
METHODS: Sixty-two children with TLE (ages 5-18) and 61 controls (ages 6-20) underwent a covert verb generation task functional magnetic resonance

BACKGROUND: Hippocampal sclerosis (HS) is the most frequent neuropathological finding in mesial temporal lobe epilepsy (MTLE-HS), leading to hippocampal atrophy due to neuronal loss and gliosis. Anxiety and depression are highly prevalent in drug-resistant MTLE-HS patients, but their neuroanatomical correlates remain unclear. This study investigates the independent association between affective symptoms and hippocampal subfield volumes in unilateral MTLE-HS.
METHODS: We analyzed 46 consecutive patients with unilateral drug-resistant MTLE-HS using MRI-based volumetry of hippocampal subfields o

OBJECTIVE: Temporal Lobe Epilepsy (TLE) often leads to cognitive decline. The Basal Forebrain Cholinergic System (BFCS), essential for memory processes, may play a critical role. This study investigates BFCS alterations, associated white matter tracts, and cognitive correlations in TLE patients.
METHODS: We analyzed 100 unilateral TLE patients and 25 healthy controls. Neuropsychological assessments included the Wechsler Memory Scale-Revised and the Rey Complex Figure Test. Regions of interest (ROIs), such as the hippocampus, amygdala, and dorsolateral prefrontal cortex, were identified using

OBJECTIVE: Epilepsy patients face significantly elevated cardiovascular risks, with cardiac arrhythmias occurring 2-3 times more frequently than in the general population. Current knowledge of brain-heart functional coupling abnormalities in epilepsy, particularly during interictal periods, remains limited. We investigated brain-heart interplay characteristics in temporal lobe epilepsy through synchronized electroencephalographic-electrocardiographic analysis using a synthetic data generation model and microstate analysis.
METHODS: We enrolled 52 patients with temporal lobe epilepsy (mean age

Considering the growing age of the world population, the incidence of epilepsy in older adults is expected to increase significantly. It has been suggested that late-onset temporal lobe epilepsy (LO-TLE) may be neurodegenerative in origin and overlap with Alzheimer's disease (AD). Herein, we aimed to characterize the pattern of cortical atrophy and CSF biomarkers of AD (total and phosphorylated tau and amyloid-β) in a selected population of LO-TLE of unknown origin. We prospectively enrolled individuals with temporal lobe epilepsy onset after the age of 50 and no cognitive impairment. They und

AIMS: To examine the association between postoperative lesions in distinct ROIs of the brain and the impact that their ablation would have on the structural and functional brain connectivity relative to outcomes.
METHODS: We retrospectively reviewed 21 patients with refractory unilateral MTLE. The percentage of each ablated gray matter region of interest (ROIs) was calculated, using a voxel-by-voxel comparison. The percentage of the affected fibers was calculated by assessing the neuronal change reflected by a decrease in anisotropy in the repeat scans (i.e., pre and postoperative). Graph the
