Exercise training in patients with heart disease: review of beneficial effects and clinical recommendations.

Over the last decades exercise training has evolved into an established evidence-based therapeutic strategy with prognostic benefits in many cardiovascular diseases (CVDs): In stable coronary artery disease (CAD) exercise training attenuates disease progression by beneficially influencing CVD risk factors (i.e., hyperlipidemia, hypertension) and coronary endothelial function. In heart failure (HF) with reduced ejection fraction (HFrEF) training prevents the progressive loss of exercise capacity by antagonizing peripheral skeletal muscle wasting and by promoting left ventricular reverse remodel

Exercise and pregnancy.

Exercise is an important component of a healthy lifestyle and, as such, is recommended during pregnancy. However, the response to exercise of both the expectant mother and fetus varies depending on the fitness level of the woman. The response to exercise is also affected by the known musculoskeletal and physiologic changes associated with pregnancy, such as increased ligament laxity, weight gain, change in the center of gravity, and carpal tunnel syndrome. Although the physiologic responses of the pregnant woman and fetus have been well studied, the literature contains comparatively few studie

Effects of exercise on chromium levels. Is supplementation required?

It is estimated that most individuals are not ingesting sufficient amounts of chromium in their diets. Although there is little information on chromium intake in athletes, many athletes ingest more calories than do non-athletes so their chromium intake should be adequate. However, athletes who restrict calories to maintain low bodyweights could compromise their chromium status. Some evidence also shows that exercise may increase chromium loss into the urine. At present, it is not known whether this loss necessitates additional chromium in the diet or whether the body will increase retention in

Exercise testing in patients with valvular diseases.

Sixty-eight patients with valvular diseases (VD) were subjected to maximal, symptoms limited, exercise testing (ET) using a cycloergometer. The exercise was limited in 82% of the valvular patients by dyspnea attributed to pulmonary capillary pressure rising, even if in the 20 patients with mitral stenosis the relation between the effort intensity and mitral valve area (MVA) (echo) was absent (r = 0.26). Myocardial aerobic impairment (MAI) was absent in 17%, mild in 41%, moderate in 39% and severe in 3% of the patients with VD. It was considered overestimated, the effort being stopped, in 2/3 o

Reversibility of the analgesic effect of meptazinol in volunteers.

A double-blind randomized placebo-controlled study was carried out in 8 healthy volunteers to determine whether the analgesic and other actions of meptazinol could be reversed by naloxone or hyoscine. Heart rate, blood pressure, respiratory rate, pupil diameter, forearm ischaemic exercise tolerance and dental pain threshold were measured before and up to 45 minutes after intramuscular meptazinol 100 mg or placebo, when 'reversal' was attempted with intravenous naloxone 2 mg, hyoscine 1.6 mg or placebo. Meptazinol caused mild pupillary constriction which is unlikely to be clinically significant…

Heart rate response to graded exercise correlates with aerobic and ventilatory capacity in patients with heart failure.

BACKGROUND: Autonomic dysfunction and reduced exercise tolerance are typical features of patients with congestive heart failure (CHF). Baro-chemoreflex balance and organ response may have a common role in conditioning exercise tolerance, ventilation, and chronotropic competence in patients with CHF. HYPOTHESIS: We tested the hypothesis that there is a relationship between functional capacity and chronotropic competence to exercise in CHF. METHODS: In all, 48 stable outpatients with CHF (age 65 +/- 10 years, 41 men, NYHA class 2.1 +/- 0, ejection fraction 31 +/- 7%, peak VO2 16 +/- 4 ml/kg/mi…

A double blind, placebo controlled, crossover trial of D-ribose in McArdle's disease.

To determine whether seven days oral D-ribose would improve exercise tolerance in a group of 5 patients with McArdle's disease, we performed a double blind placebo controlled crossover trial. Subjects performed weekly treadmill exercise tests with expired gas analysis until their times were reproducible. They then received 60 g D-ribose daily or placebo for seven days. Exercise testing was repeated on completion of this period. A seven day washout period then followed. Subjects then performed a new baseline exercise test prior to starting the other solution. Again after seven days the exercise…

Combined exercise hinders the progression of pulmonary and right heart harmful remodeling in monocrotaline-induced pulmonary arterial hypertension.

The aim of this study was to test whether combined physical exercise training of moderate intensity executed during the development of monocrotaline (MCT)-induced pulmonary arterial hypertension (PAH) hinders the progression of pulmonary and right heart harmful functional and structural remodeling in rats. Wistar rats were injected with MCT (60 mg/kg) and after 24 h were exposed to a combined exercise training program: aerobic exercise (treadmill running-60 min/day; 60% of maximum running speed); and resistance exercise (vertical ladder climbing-15 climbs; 60% of maximum carrying load), on alt

A single administration of glucocorticoid increases exercise capacity in men with stable chronic obstructive pulmonary disease.

INTRODUCTION: Oral use of glucocorticoids is a well-described therapy in the treatment of acute exacerbations of chronic obstructive pulmonary disease (COPD). Patients with COPD occasionally report subjective benefits of glucocorticoids in terms of fewer symptoms and better exercise performance, even when they do not experience exacerbation. This study investigated whether a single high therapeutic dose of oral glucocorticoid improves short-term exercise tolerance in patients with moderate-to-very severe COPD. METHODS: In a double-blinded crossover design, patients were randomised to either 1…
