Use of the "chimney patch" technique for Norwood stage I procedures.

The Norwood stage I procedure is often used for the initial treatment of infants with hypoplastic left heart syndrome. This procedure creates a systemic arterial to pulmonary artery shunt to establish pulmonary blood flow. We describe a method to facilitate placement of this shunt by attaching a polytetrafluoroethylene shunt to a pulmonary artery homograft patch before performing the median sternotomy. This technique facilitates the performance of the proximal shunt anastomosis and expedites the procedure.

The impact of Arctic environments on human cerebral blood flow velocity.

BACKGROUND: The Arctic environment represents an extreme living condition that has significant impact on life. But research on alterations of human cerebral blood flow velocity (CBFV) in a high-latitude and low-temperature environment in the Arctic is still lacking. METHODS: Members of the 12th Chinese National Arctic Research Expedition team who took the icebreaker R/V Xuelong 2 to the Arctic were recruited. Transcranial colour doppler (TCD) examination was performed at the beginning of the voyage (lower latitude and higher temperature, LLHT) and during the Arctic scientific expedition perio

Endothelial-specific telomerase inactivation causes telomere-independent cell senescence and multi-organ dysfunction characteristic of aging.

It has remained unclear how aging of endothelial cells (EC) contributes to pathophysiology of individual organs. Cell senescence results in part from inactivation of telomerase (TERT). Here, we analyzed mice with Tert knockout specifically in EC. Tert loss in EC induced transcriptional changes indicative of senescence and tissue hypoxia in EC and in other cells. We demonstrate that EC-Tert-KO mice have leaky blood vessels. The blood-brain barrier of EC-Tert-KO mice is compromised, and their cognitive function is impaired. EC-Tert-KO mice display reduced muscle endurance and decreased expressio

Voluntary Increase of Minute Ventilation for Prevention of Acute Mountain Sickness.

This study evaluated the feasibility and efficacy of voluntary sustained hyperventilation during rapid ascent to high altitude for the prevention of acute mountain sickness (AMS). Study subjects (n=32) were volunteer participants in a 2-day expedition to Mount Leoneras (4954 m), starting at 2800m (base camp at 4120 m). Subjects were randomized to either: 1) an intervention group using the voluntary hyperventilation (VH) technique targeting an end-tidal CO2 (ETCO2)<20 mmHg; or 2) a group using acetazolamide (AZ). During the expedition, respiratory rate (28±20 vs. 18±5 breaths/min, mean±SD, P<0.

Can donor narratives yield insights? A natural language processing proof of concept to facilitate kidney allocation.

Although expedited placement could ameliorate stagnant kidney utilization, precisely identifying difficult-to-place organs is crucial to mitigate potential harms associated with this policy. Existing algorithms have only leveraged structured data from the Organ Procurement and Transplantation Network (OPTN); however, detailed, free text case information about a donor exists. No known research exists about the utility of these data. We developed a model to predict the probability of delay or discard for adult deceased kidney donors between 2010 and 2018, leveraging donor free text data. The res

A double-blind randomized controlled study to evaluate the efficacy of low-dose oral interferon-alpha in preventing hepatitis C relapse.

Low-dose oral interferon could exert immune-modulating effects in human. We conducted a clinical trial to investigate the efficacy of oral interferon-alpha in preventing hepatitis C relapse. Totally 169 genotype 1b chronic hepatitis C patients having achieved end-of-therapy virological clearance were randomized to receive interferon-alpha lozenge 500 IU/day (n=59), 1,500 IU/day (n=53), or placebo (n=57) for 24 weeks. Overall, no significant differences were found for the relapse rates in the 3 groups (P>0.05). However, in patients with fibroindex 1.4-1.7, relapse occurred in 1/12 (8.3%) 500 IU

Enhancement of cold-induced vasodilatation following acclimatization to altitude.

The present study evaluated the effect of high-altitude acclimatisation on the cold-induced vasodilatation (CIVD) response. A group of highly trained mountaineers (N = 9; Alpinists) were tested before and after a 3 week high-altitude Himalayan expedition (altitude ranging from 3,985 to 6,828 m). A control group (N = 7) with no mountaineering experience was tested at the same time points. During each test, subjects first immersed their hand to the styloid process in 37 degrees C water for 5 min and then in 10 degrees C water for 30 min. Upon completion of the hand immersion, the same procedure

Operative versus conservative management for 'fetal distress' in labour.

BACKGROUND: Suspected fetal distress usually results in expedited delivery of a baby (often operatively). The potential harm to a mother and baby from operative delivery may not always be justified especially when fetal distress may be misdiagnosed. Even with a correct diagnosis it is not clear whether an operative or conservative approach is better. OBJECTIVES: The objective of this review was to assess the effects of operative management for fetal distress on maternal and perinatal morbidity. SEARCH STRATEGY: The Cochrane Pregnancy and Childbirth Group trials register and the Cochrane Cont

Standardization of gastric aspirate technique improves yield in the diagnosis of tuberculosis in children.

BACKGROUND: The diagnosis of tuberculous disease in children remains a difficult one, based on epidemiologic investigation, Mantoux skin testing and suggestive radiologic findings. Because children with pulmonary tuberculosis are unable to produce sputum, gastric aspirates remain the procedure of choice for microbiologic confirmation of tuberculous disease; however, yield is frequently low. OBJECTIVES: To evaluate the effect of a standardized gastric aspirate collection protocol on diagnostic culture yield. METHODS: The gastric aspirate culture yield for Mycobacterium tuberculosis in 13 hist

Tolerance to severe hypoxia: lessons from Mt. Everest.

Human tolerance to chronic severe hypoxia has been elucidated by two recent high altitude studies: the 1981 American Medical Research Expedition to Everest, and Operation Everest II, a 40-day low pressure chamber-simulated ascent of Everest. The severe oxygen deprivation of extreme altitudes can only be tolerated because of an enormous increase in ventilation which defends the alveolar Po2 against the reduced inspired value. Nevertheless the arterial Po2 on the Everest summit is less than 30 mmHg. The hyperventilation results in a very low arterial Pco2 which causes severe respiratory alkalosi

Optimizing Exposure for Mitral Repair: Anterior Annulus-to-Leaflet Stitch Technique.

Exposure of anatomic landmarks is key in mitral valve repair. We report a technique to improve exposure of the mitral valve posterior leaflet during video-assisted procedures. It consists of a traction stitch placed on the leaflet free edges after triangular resection and through the anterior left atrial wall close to the mitral annulus. Gentle traction on this suture pulls the posterior leaflet on the plane of the mitral annulus to improve exposure and to expedite the reconstruction.

Point-of-Care Ultrasound to Diagnose Postpericardiotomy Syndrome in a Child.

We report a case of a patient presenting with abdominal pain after cardiac surgery who was noted on point-of-care ultrasound (POCUS) to have pericardial and pleural effusion, in addition to ascites. The most notable findings were pleural and pericardial effusions, which combined with symptomatology met criteria for postpericardiotomy syndrome. Point-of-care ultrasound expedited the diagnosis of a pericardial effusion with impending tamponade and transfer for pericardiocentesis and placement of pericardial drain.

Cardiac tamponade or normal respiratory variation? An illustrative case of septal ablation for obstructive hypertrophic cardiomyopathy.

Cardiac tamponade can be a lethal complication in the cardiac catheterization laboratory. Patients with obstructive hypertrophic cardiomyopathy are particularly vulnerable to the hemodynamic effects of tamponade because of their heightened sensitivity to ventricular load in the generation of forward stroke volume. Cardiac tamponade should be distinguished from the normal effects of respiration on dynamic outflow tract obstruction in these patients. This recognition not only ensures an accurate hemodynamic diagnosis, but it also will expedite timely therapy for patients in need.

Imaging of the elbow in the overhead throwing athlete.

Elbow injuries in athletes who perform overhead throwing motions often present diagnostic challenges because of the undue stresses and often chronic, repetitive patterns of injury. Accurate and efficient assessment of the injured elbow is essential to maximize functional recovery and expedite return to play. Radiographic evaluation should be tailored to the specific injury suspected and requires a thorough understanding of normal anatomic relationships as well as familiarity with common injuries affecting these athletes.

The hunter and the hunted: mpox in the Mkpot rainforest community of Nigeria.

We describe challenges and opportunities during the mpox virus animal reservoir surveillance expedition to the Mkpot community in Nigeria, a remote enclave in the heart of the Cross River rainforest that is replete with viral and ecological footprints. Anecdotal evidence of mpox in Mkpot and adjoining villages, and the absence of domestic animals where wild animal hunting and consumption is intense, suggests potential spillover of infections at the human-animal interface, requiring active surveillance and monitoring.

Acute and Emergent Spinal Injury Assessment and Treatment.

Sports participation is a leading cause of catastrophic cervical spine injury (CSI) in the United States. Appropriate prehospital care for athletes with suspected CSIs should be available at all levels of sport. Planning the process of transport for home venues before the start of the season and ensuring that a medical time out occurs at home and away games can reduce complications of transport decisions on the field of play and expedite transport of the spine-injured athlete.

Pediatric Tongue Lesions: An Often-Overlooked but Important Collection of Diagnoses.

OBJECTIVE. The purpose of this article is to discuss imaging techniques and provide a pictorial review of pediatric tongue lesions. CONCLUSION. Pediatric tongue lesions represent an often-overlooked collection of abnormalities with a variety of clinical and radiologic features. The location of these lesions places the patient at risk for airway compromise. Systematic imaging evaluation of pediatric tongue lesions can help the radiologist arrive at a correct and timely diagnosis, thus improving and expediting patient care.
