Pilon tibiale fractures with significant joint involvement (AO-classification B2/3 and C2/3) are considered one of the most unfavourable injuries of the lower extremity. It was possible for follow-up examinations to be performed on 50 patients with this injury pattern from the years 1984 to 1988. Primary plate osteosynthesis is primary blamed for causing a high infection rate. Remaining functional losses, uneven joints, defective positionings and early arthrosis are not avoidable with either a one step or a two step approach. The infection rate could be significantly lowered through a two step

External skeletal fixation has been proved to be an effective method in treating complex femoral shaft fracture in adult patients with multiple trauma, extensive soft tissue injury or vascular injury which need reconstructive procedures. This method has the advantages of less extensive surgical exposure, easy wound access, and early ambulation. Twenty-two cases of femoral shaft fracture, between the ages of 10 and 15, were treated with the Hoffmann external skeletal fixator in Chang Gung Memorial Hospital from 1981 to 1985. The average time for union and removal of fixator was 72 +/- 8 days (f

Grade III open tibial fractures are known for frequent complications and poor clinical results, yet published series are few and cite conflicting results. To address this dilemma, the authors report a prospective study of 202 consecutive Grade III tibial fractures. All injuries were treated under protocol at the authors' university with primary external fixation and serial debridement. Equinus deformity was prevented with a new tibiometatarsal frame extension. Severe injuries crossing the ankle or knee were temporarily stabilized with external fixation across these joints. Staged reconstructio

The authors describe their operative approach to metatarsus primus adductus deformity when present in conjunction with a congenitally short first metatarsal. Hallux abducto valgus correction in this clinical setting has traditionally advocated use of a crescentic or opening wedge osteotomy in order to prevent further shortening of the first metatarsal. Fixation of opening base wedge osteotomies has been fraught with problems in the past, with possibilities for displacement or eventual loss of initial correction. The authors have found the mini-Hoffmann external fixation device useful for this

INTRODUCTION: Role 2 (R2) U.S. military treatment facilities provide lifesaving far forward damage control resuscitation and surgery. Given the austere conditions at R2s, infection risk is a major concern. We aimed to evaluate the infection rate after external fixation (EF) in military casualties based on where in the evacuation pathway the EF was performed, hypothesizing that lower-echelon EF would be associated with increased infections.
MATERIALS AND METHODS: The Department of Defense Trauma Registry (DoDTR) was retrospectively reviewed from 2003 to 2024. Non-U.S. military patients, deaths

BACKGROUND: The temporary use of external fixators is an established method for stabilising fractures, particularly for injuries that do not initially allow definitive treatment. Despite its frequent use, it remains unclear whether the interval between fixator application and subsequent definitive fracture treatment affects microbial colonisation of the fixation screws prior to internal fixation.
METHODS: This prospective, single-centre study included patients aged 18 years or older between April 2023 and April 2024 who received a temporary fixator during initial fracture management. During d…

BACKGROUND: Lower limb defects with single-vessel supply and with monophasic flow are difficult to reconstruct due to paucity of coverage options. The cross-leg flap remains a reliable salvage option in these settings. But maintaining the position for three weeks is cumbersome for both the patient and the flap. Any movement of the lower limb generates a shearing force at the flap inset, predisposing to partial flap necrosis and dehiscence. An external fixator helps both the stable fixation of the lower limbs and the prevention of stretch in the flap. We aim to describe a modification in the co

INTRODUCTION AND IMPORTANCE: Brachymetatarsia is a rare deformity characterized by abnormal shortening of one or more metatarsals due to premature physeal closure. If conservative treatment fails, surgical options such as one-stage lengthening or progressive distraction with an external fixator are the usual alternatives. This report presents the case of a patient with type 1 Rubinstein-Taybi syndrome and first-metatarsal brachymetatarsia, who was treated by progressive distraction with a hexapod external fixator. This is the first such case reported in the literature.

OBJECTIVE: The objective of this study was to evaluate and compare the efficacy of multiple disinfectant agents commonly used to remove bacteria from external fixators infected with Staphylococcus aureus.
METHODS: A sterile external fixator was colonized with Staphylococcus aureus for 48 h to allow biofilm formation on the device. Following incubation, the device was treated with one of the following: Isopropyl Alcohol (IPA), Chlorhexidine Gluconate Scrub (CHG) + IPA, Betadine (PI) + IPA, or ChloraPrep (2% CHG and 70% IPA). Swabs were taken from locations on each device and plated on Tryptic …

BACKGROUND: Subtalar dislocations (simultaneous dislocations of the talocalcaneal and talonavicular joints) without concomitant fracture are rare and account for 1-2% of all dislocations. The treatment has been described in several case reports and consists primarily of closed reduction with the patient under anesthesia followed by immobilization.
CASE: A 30-year-old male patient sustained a multidimensional nonfractured dislocation of the ankle joint with a predominantly subtalar component (luxatio subtalolateralis) while climbing. As it turned out later, a closed reduction was not possible

BACKGROUND: Open fractures with bone and soft tissue loss pose significant challenges in orthopedic surgery. External fixator-assisted techniques, such as acute angulation and shortening, have been proposed to facilitate wound closure. However, the vascular implications of these techniques remain unclear. This cadaveric study aimed to evaluate the effect of acute angulation on arterial narrowing at the proximal tibia and distal femur, identifying angulation thresholds and comparing the vascular tolerance between these anatomical sites.
METHODS: Eight lower extremities from four fresh-frozen c

BACKGROUND: We aimed to evaluate if there would be loss of some correction achieved (caused by remodeling) after correcting tibia vara deformity with hexapod circular fixators and if that loss of correction would influence clinical outcomes.
HYPOTHESIS: The hypothesis of the study was that remodeling at the osteotomy site would lead to loss of some correction over time.
PATIENTS AND METHODS: This retrospective study included 18 patients with tibial varus deformity as a sequela of adolescent tibia vara who underwent proximal tibial deformity correction using the Hexapod External Fixator betwe

BACKGROUND: Multiple displaced rib fractures often result in a poor prognosis. Open reduction and internal fixation has been shown to provide benefits for patients with displaced rib fractures and flail chest. Nevertheless, for patients who are unwilling or unsuitable for surgery, the therapeutic options are limited. We developed a novel plastic vacuum device for rib fractures external stabilization. This study aims to compare the therapeutic efficacy of this device against a traditional chest strap in polytrauma patients with multiple rib fractures.
METHODS: A retrospective investigation was

BACKGROUND: Ankle fusion is considered a treatment of choice for end-stage ankle arthritis when a total ankle replacement procedure is not indicated. However, the potential risk of secondary arthritis in the adjacent joint after ankle fusion raises arguments on whether preserving the adjacent joint during an isolated tibiotalar (TT) fusion brings about any future benefits with regard to pain and gait discomfort. In this study, we intended to present midterm results following TT or tibiotalocalcaneal (TTC) fusion using an Ilizarov external fixator and to investigate whether spontaneous fusion o

Congenital hallux varus is an unusual condition in which the big toe is medially angulated at the metatarsophalangeal joint. Different surgical methods have been described to correct it; however, there is always a risk of recurrence. The multiplanar nature of the deformity and the disrupted anatomy from a previous surgery make recurrent cases difficult to treat. A 16-year-old adolescent boy presented with a recurrent hallux varus deformity after a previous surgical correction. This case report describes its successful treatment using a hexapod circular external fixator as a salvage procedure.

Aim - In order to fulfill the purpose of biomechanical substantiation for extrafocal pelvic osteosynthesis in osteoporosis we studied the stress-strain state (SSS) of LEG under conditions of external fixation by apparatuses with cylindrical and conical rods. Studies of the stress-strain state of the lower extremity belt by the finite element method were carried out under conditions of external fixation by devices with cylindrical and conical rods. In the finite element method, the approximate solution is constructed as a superposition of approximating functions. At the first stage of solving t
