Poor outcome of sphincter repair: an evacuation problem?

INTRODUCTION: Colorectal transport in idiopathic fecal incontinence has scarcely been studied, and it remains to be investigated in patients with fecal incontinence and anal sphincter lesion. The aim of the present study was to compare colorectal transport during defecation in patients with idiopathic fecal incontinence and patients with fecal incontinence due to anal sphincter lesions with transport in healthy volunteers. METHOD: Five women with idiopathic fecal incontinence (median age 72 years, range: 58-78 years) and five women with an obstetric sphincter lesion (median age 42 years, rang…

Early functional results of biofeedback and its impact on quality of life of patients with anal incontinence.

CONTEXT: Biofeedback has been used successfully in the treatment of fecal incontinence, working mainly on rehabilitation of the sphincter muscle. However, there are few studies presenting objective results of biofeedback, in terms of functional results and those related to the quality of life. OBJECTIVES: The aims of this study was to evaluate the immediate results of biofeedback in the treatment of fecal incontinence and its impact on the quality of life of patients by using validated questionnaires, correlating the results with those related to functional parameters of quality of life and c…

Defecatory symptoms during and after the first pregnancy: prevalences and associated factors.

A prospective cohort study was undertaken to evaluate the impact of pregnancy and the first delivery on the defecatory symptoms and to identify associated factors. Included were 487 nulliparous pregnant women who completed four questionnaires. Flatus and fecal incontinence, constipation, and painful defecation are already present in early pregnancy and are significantly predictive for reporting symptoms after delivery, except for fecal incontinence. A third or fourth degree sphincter tear was significantly associated with fecal incontinence 12 months postpartum and with de novo fecal incontine…

Biofeedback Therapy for Incontinence in a Post-Duhamel Hirschsprung Patient: A Case Report.

Fecal incontinence is a recognized but underreported postoperative complication in patients with Hirschsprung disease (HD) Retentive fecal incontinence may result from stool retention in the Duhamel pouch, altered rectal sensation, or dyssynergic defecation. We report the use of biofeedback therapy in a 17-year-old male with a history of HD and total colonic aganglionosis (TCA), who underwent a Duhamel pull-through in infancy and later developed recurrent fecal soiling and constipation. Clinical examination demonstrated stool retention with preserved perianal sensation and sphincter tone, cons…

Anorectal manometry.

Anorectal manometry is used in the treatment of complex anorectal disorders, such as fecal incontinence and intractable constipation. In a 3-year period at one institution 308 anorectal manometries were performed. A total of 168 procedures were performed for complaints of fecal incontinence, 77 for constipation, and the remainder for a variety of anorectal disorders. Anorectal manometry in those under 20 years of age was performed most often to differentiate Hirschsprung disease from functional constipation, as well as to provide a differential diagnosis related to congenital anorectal anomali…

Percutaneous tibial nerve stimulation (pTNS): success rate and the role of rectal capacity.

PURPOSE: Percutaneous tibial nerve stimulation (pTNS) was originally developed to treat urinary incontinence. Recently, some case series have also documented its success in the treatment of fecal incontinence. Nevertheless, the mechanism underlying this effect remains unknown but may be related to changes in rectal capacity. The aim of this study was to investigate the success of pTNS for the treatment of fecal urge incontinence and assess the influence of rectal capacity on treatment efficacy. METHODS: All patients undergoing pTNS for fecal incontinence between July 2009 and March 2014 were …
