INTRODUCTION: Primary lymphomas of peripheral nerves are extremely rare, and only a few cases have been reported.

The objective of this article is to report upon a simple technique of bone marrow specimen collection of intramedullary bone tumors using a pediatric chest tube. This technique is described along with two case reports which include a 60-year-old female with metastatic breast cancer to the proximal femur, and a 63-year-old male with multiple myeloma of the proximal humerus. This study included 11 patients with lower limb tumors who had wide local resection and reconstruction by vascularized fibula osteoseptocutaneous flap and their surgery performed at least 24 months before the end of the study. The average age at operation was 14.4 years. All tumors, except one, were high-grade sarcomas (stage IIA, IIB, and Ewing's sarcoma). The tumor volume averaged 200.6 cm3 (range 41.5-400). The resulting defect after tumor resection averaged 14.8 cm (range 9-20). The fibula was inserted as a single strut in eight patients and as a double-barrel construct in three patients.

From September 1986 to December 1989, 144 patients with osteosarcoma of the extremities were treated with combined surgery and neoadjuvant chemotherapy. The disease-free survival was 79% for good responders (necrosis greater than 90%) and 72% for poor responders (necrosis less than 90%), and the local recurrence rate was low. A case report is presented of a patient with a known malignant pheochromocytoma, who was to undergo an orthopaedic procedure using methylmethacrylate bone cement. Preoperative preparation of the patient included the use of prazosin and metyrosine. Epidural blockade was chosen as the anaesthetic technique for intraoperative management. Surgical fixation with methylmethacrylate did not produce profound or prolonged hypotension despite sudden massive blood loss prior to its use. Epidural morphine was administered for postoperative pain control.

Sixty-two cases of pathologic fractures occurring in 53 patients treated at Scott and White Memorial Hospital from 1966 to 1976 are reviewed and analyzed with regard to type of neoplastic lesion, location, mode of therapy initiated, functional stability of therapy selected, and average length of survival after orthopedic procedure. Pertinent literature is reviewed, and our results are compared with those of other series.

A 95-year-old woman was admitted to hospital with a fractured neck of femur following a fall. There was no past history of note other than numerous episodes of "funny turns" preceding falls some of which resulted in fractures. During her admission she had a "funny turn". Immediate investigations revealed hypoglycaemia and subsequent biochemical abnormalities, most likely due to a benign endocrine tumour--an insulinoma.

We report a case of osteomyelitis of the proximal femur caused by Lancefield group G streptococcus in a 71-year-old otherwise healthy man. The organism has rarely been identified as the cause of osteomyelitis. The subacute nature of the symptoms and the radiological appearance of the femur in this patient mimic bone tumour. The patient was successfully treated with conservative methods, including a prolonged period of oral antibiotics. We stress the importance of histological and bacteriological evidence in avoiding misdiagnosing patients with equivocal clinical and radiological presentation.
