Association between the toe angle and bony factors in the transverse plane for osteoarthritic knees compared with healthy knees.

BACKGROUND: The association between the toe angle and bony rotational factors is critical to explain issues related to the onset and progression of knee osteoarthritis (OA). OBJECTIVE: The study aimed to clarify the association between the toe angle and each of the femoral and tibial transvers direction relative to gait direction, rotational knee alignment, and bony torsional deformity for the subjects with knee OA. METHODS: This study evaluated 58 knees in 24 healthy elderly (72 ± 5 years) and 34 varus knee OA (72 ± 6 years). A three-dimensional (3D) assessment system was used on 3D models

Intertrochanteric Osteotomies in Children and Adolescents: Principles of Proximal Femoral Deformity Correction.

Intertrochanteric osteotomies are used to correct a variety of congenital and acquired deformities of the proximal femur in children and adolescents. Varus, valgus, flexion, extension, or rotational osteotomies at the intertrochanteric level can be used to restore normal alignment of the proximal femur, increase congruency and stability of the hip joint, facilitate healing or remodeling of the femoral head and neck, redirect the articular surface, and improve functional limb position in the case of soft-tissue contractures. The steps necessary to assess proximal femoral deformity and plan the

Is It Time to Phase Out the Austin Moore Hemiarthroplasty? A Propensity Score Matched Case Control Comparison versus Cemented Hemiarthroplasty.

We compared the Austin Moore hemiarthroplasty versus cemented hemiarthroplasties using a propensity score matched cased control study. For a consecutive cohort of 450 patients with displaced intracapsular neck of femur fractures, 128 matched cases in each group were selected based on age, gender, walking status, nursing home residency, delays in surgery, ASA score, and the Charlson comorbidity score. At a mean follow-up of 16.3 months, we evaluated their outcomes. Significantly more patients with AMA experienced thigh pain (RR = 3.5, 95% CI: 1.67-7.33, p = 0.000), overall complications (RR = 4

Changes of the bone mineral density in proximal femur following total hip resurfacing arthroplasty in osteonecrosis of femoral head.

Total hip resurfacing arthroplasty (THRA) is being performed with increasing frequency for osteonecrosis of femoral head (ONFH). To evaluate femoral bone remodeling in ONFH after THRA and determine the impact of stem-neck angle (SNA) of inserted femoral component on bone remodeling, we monitored the changes in BMD in proximal femur in 23 patients with ONFH after surgery. Patients were divided into group A (SNA >or= 5 degrees ) and group B (SNA < 5 degrees ). The BMD was measured in seven Gruen zones and two neck zones using dual-energy X-ray absorptiometry preoperatively, then at 3, 6, 12, and

Decreased femoral fracture load after cephalomedullary nail removal : a biomechanical ex vivo study.

AIMS: Spontaneous neck fractures are feared complications of cephalomedullary nail removal after successful healing of per- and subtrochanteric fractures. To date, the initial postoperative stability as well as the correct weightbearing regimen remain unclear. The aim of this biomechanical ex vivo study was to evaluate the initial postoperative failure load after hardware removal of specimens, which received cephalomedullary nails during their lifetime. METHODS: A total of 20 specimens of voluntary body donors were included in this study. Group 1 (n = 10) consisted of specimens that received

Ipsilateral femoral neck and shaft fractures: case series from a single Level-I trauma centre and review of literature.

INTRODUCTION: Uncommon but increasingly diagnosed, ipsilateral femoral neck and shaft fracture represents a complex injury pattern with challenging management. The conundrum involves optimal diagnostic modality, timing of surgery, sequence of fixation, and the choice of implant. METHODOLOGY: A retrospective review was conducted at a Level-I trauma centre to assess the outcome of concomitant femoral neck and shaft fractures managed with various implants and attempt to provide solutions to the aforementioned queries. The time between injury and definitive surgery, choice of implant, sequence of

Skeletal-extraskeletal angiomatosis in a female Boxer puppy.

A 3-month-old female Boxer puppy presented for bilateral inguinal lymph node enlargement, vulvar swelling and hip pain. Pelvic radiographs revealed moth-eaten to permeative lytic lesions affecting the right hemipelvis and right femoral head and neck. An incomplete pathological fracture of the cranial proximal right femur, an incomplete right femoral neck fracture and a suspected folding fracture of the right ischium were also noted. The dog was euthanized and submitted for necropsy. The fractures were confirmed at necropsy and were surrounded by extensive dark red areas in the skeletal muscle

Understanding the variation of volumetric bone mineral density in the femur and tibia in a paediatric population.

Childhood and adolescence are crucial time for bone mineral accumulation with 25 % of bone mineral density (BMD) being laid during puberty. BMD development during growth was found to be correlated with the development of osteoporosis later in life. Mapping the variation of BMD in a paediatric population is important to understand how BMD change with age and sex. Therefore, the aim of this study was to evaluate the variation of BMD in the long bones for a paediatric population. CT-scans of 333 children and adolescents aged from 4 to 18 years were used to reconstruct 657 femora and 652 tibiae. V

Massive bone destruction in a human proximal femur from ancient Nubia.

Massive bone destruction in a human right proximal femur is described and differentially discussed in this case study. The individual was an older adult female buried at the ancient Nubian site of Tombos (modern-day Sudan) dating to the early Napatan/Third Intermediate Period (c. 1069-750 BCE). The right femur displayed a pathologic fracture with extensive lytic destruction and resorption of the entire femoral neck, most of the femoral head, and trochanters. Macroscopic and radiographic analyses revealed cortical thinning of the proximal diaphysis with new bone formation enclosing the medullar

Low-level chronic exposure to cadmium enhances the risk of long bone fractures: a study on a female rat model of human lifetime exposure.

In the present paper, the hypothesis that low chronic exposure to cadmium (Cd) enhances the risk of long bone fractures was investigated in a female rat model simulating human lifetime exposure in non-Cd-polluted areas. For this purpose, the femur and both tibias of control female rats and those exposed to Cd (1 mg Cd I(-1) in drinking water for 24 months since weaning) were assigned to geometric, densitometric (bone mineral content, BMC, and density, BMD), radiographic and biomechanical studies as well as assessing their chemical composition. The exposure to Cd disturbed mineralization (decre

Radiological evidence of femoroacetabular impingement in mild slipped capital femoral epiphysis: a mean follow-up of 14.4 years after pinning in situ.

Conventional treatment of mild slipped capital femoral epiphysis consists of fixation in situ with wires or screws. Recent contributions to the literature suggest that even a mild slip may lead to early damage of the acetabular labrum and adjacent cartilage by abutment of a prominent femoral metaphysis. It has been suggested that the appropriate treatment in mild slipped capital femoral epiphysis should not only prevent further slipping of the epiphysis, but also address potential femoroacetabular impingement by restoring the anatomy of the proximal femur. Between October 1984 and December 199

[Measurement of femoral bone density using dual photon absorptiometry. Values in osteoporosis].

X-ray absorptiometry allows measuring the density of bone in the upper end of the femur in 3 mains areas: the neck, Ward's triangle and the greater trochanter. Failin prospective data, it is not possible at present to know whether one of these areas has better performances than the others for the assessment of fracture risks. The interpretation of the measurement is based on the idea that bone demineralization is the main risk factor of osteoporotic fracture. It is carried out on the basis of reference normal values, which must be determined in the French population. More sophisticated predict

Intertrochanteric hip stress fracture in a male ultramarathon runner.

Stress fractures are injuries frequently seen in high-performance athletes, especially runners. In the femur, the most commonly affected locations are the femoral neck, condylar area and proximal shaft. Intertrochanteric fractures are much more common in the elderly population, especially among those with osteoporosis, but they can also be a result of high-energy repetitive mechanisms. We present a case of an intertrochanteric stress fracture in a young male runner. The diagnosis was suspected after persistent pain following his first marathon, and it was confirmed with an MRI. Operative fixat

A finite element analysis study based on valgus impacted femoral neck fracture under diverse stances.

The aim of the study was to determine the biomechanical environment of patients who suffer from valgus impacted femoral neck fracture. With the help of computational modeling, both of finite element hip fracture and normal three-dimensional model were reconstructed from a patient with hip fracture. The predicted stress distribution was compared between before and after fracture. After the fracture, during standing and the gait, the fracture site has a greater change in stress distribution due to the shortening of the femoral neck. The largest stress occurs at the middle and lower end of the fe

Minimally invasive medial hip approach for femoral head fixation: How much can we see and what is the anatomical at risk? - A cadaveric injection study.

INTRODUCTION: Most Pipkin I and II femoral head fractures are treated with either an anterior or a posterior approach. A medial hip approach is commonly used in children, and some surgeons have suggested it for femoral head fixation. The objectives of this study were to identify the structures at risk with the medial hip approach and to demonstrate the areas of the femoral head exposed using this approach. METHODS: The first part of this study involved vascular injection conducted in four fresh human cadavers using the medial hip approach. The surgical technique was described and the structur
