Subaortic stenosis can develop in patients with single ventricle and transposition of the great vessels. Its presence increases the risk of septation or Fontan repair, and reports of surgical management have indicated high mortality. From Jan 1989 to December 1991, 61 patients had modified Fontan procedures for a variety of lesions, including tricuspid atresia (18), mitral atresia (3), double inlet ventricle (14), isomeric hearts (4) and others (22). We present a case of a young boy who developed persistent tachycardia despite fluid resuscitation, antipyretics, and analgesia after a Fontan procedure. In the setting of a single ventricle, subaortic stenosis may be enhanced by pulmonary artery banding and may later contraindicate a Fontan operation. The Norwood operation may prove a preferable alternative in some infants as a preparatory procedure. We have successfully used this procedure as the initial operation to palliate a newborn with tricuspid atresia, transposition of the great arteries, coarctation, and severe arch hypoplasia secondary to a restrictive bulboventricular foramen.
