[Inserts and shoes for foot deformities].

Prevention of habitual or shoe-induced foot and toe deformities in children and treatment of congenital or developing malformations require a change of view towards functional aspects and the use of modern technical possibilities based on our traditional knowledge.Statics, functional anatomy, biomechanics, neuromotor and psychomotor activity, and developmental physiology have to be considered as well as the broad variety of physiological interindividual differences. Modern materials and fabrication techniques make it easier to produce anatomically and functionally correct and appealing shoes, …

Foot management in the foal and weanling.

Foot management and hoof care of foals, weanlings, and yearlings tends to be considered a specialist job. Many farriers and veterinary surgeons have little exposure to young and growing stock. Nevertheless, a variety of angular and flexural limb deformities can benefit from simple podiatry techniques, which can be performed provided that a few basic rules are followed. The combined skills of the farrier and veterinary surgeon offer many young horses with angular or flexural limb deformities a chance of a future athletic career.

The causes and management of foot ulceration.

Managing foot ulceration is a challenge for all concerned. This article argues that the key to successful management of ulceration lies with the accurate identification of the underlying cause of the ulcer and consideration of the additional factors that are likely to impair healing. The common causes of foot ulceration are discussed, the means of evaluation and assessment are outlined and appropriate management strategies are identified. As with all care pathways, patient concordance is essential in reaching successful clinical outcomes and foot ulceration is no exception.

[Treatment of macrodactyly and foot gigantism].

Between 1996 and 2000 the authors followed-up 6 children with congenital hypertrophy of 7 feet. Surgical treatment was performed in 4 children (4 feet). The average age at the time of surgery was 9 years, and the average follow-up period was 3 years. 2 cases of macrodactyly of the I and II rays of the foot and 1 case of macrodactyly of the II ray of the foot were observed. Amputations of phalanges or toes in all 3 cases were performed with amputations of distal parts of adjacenet metatarsals in 2 cases. Soft tissue debulking was routinely done. Operative treatment of 1 case of the foot giganti…

Trauma of the pediatric ankle and foot.

This article presents a brief overview of the injuries to the ankle and foot encountered in children and adolescents. Trauma to the ankle or foot may result from acute, chronic, or repetitive forces. The role of the different imaging modalities in the assessment of ankle and foot trauma in the growing patient is discussed. Plain radiographs remain the mainstay in the diagnosis of most acute traumas, whereas CT may be helpful to unravel the complex anatomy of certain fractures like the triplane or juvenile Tillaux fracture. In the evaluation of chronic injuries, including osteochondrosis dissec…

Dynamic pressures on the diabetic foot.

A retrospective study was conducted in order to investigate the relation between increased plantar pressure and ulcers in the diabetic foot and contribute more information on this subject. Maximal plantar pressure (MPP) is reported on various areas of the plantar aspects of the feet in 328 diabetic patients and 75 healthy subjects. An increase in MPP under all plantar areas except for the heels was found to be related with increasing severity of the diabetes symptoms. There was also a significant decrease in the application of high pressures to the heels, the metatarsal heads II-IV and the toe…

Care of the pediatric foot in myelodysplasia.

Foot deformity is present in almost all patients paralyzed by myelomeningocele. This article outlines the pertinent pathoanatomy resulting in differing foot deformities and their effects on normal gait. Treatment of these deformities is discussed, and the most common deformities present for the different levels of paralysis are outlined. Emphasis is placed on surgical and orthotic treatments, which result in functional improvements for the pediatric patient with spina bifida.

Foot injuries and arthroscopy in sport.

Arthroscopy of the foot and ankle is a valuable tool for treating athletic foot injuries. The ability to evaluate and treat injuries without an extensive open approach is very important when dealing with the athletic population, allowing a much quicker rehabilitation and return to sport. The diagnostic and therapeutic indications for foot and ankle arthroscopy have increased significantly over recent years. Techniques and instrumentation have become more advanced and more readily available to the orthopaedist. This article defines the conditions in which arthroscopy is appropriate and presents…

Surgical treatment of the cleft foot.

Between 1970 and 1997, we treated a total of 32 cleft feet in 21 patients (11 male and 10 female). We classified cases of cleft foot on the basis of the number of central ray deficiencies. Fourteen patients with 22 cleft feet were followed up for more than 1 year postoperatively (9 feet had no or one central ray deficiency, and 13 feet had two or three central ray deficiencies). The mean follow-up period was 8.8 years. The objective of this study was to evaluate the results of operative treatment of cleft foot. We evaluated the results of three methods: simple closure of the cleft, application…

Foot and ankle injuries in dance.

Acute traumatic injuries are common in ballet dancers. A careful history, thorough examination, and appropriate imaging should allow for the diagnosis of most problems. The clinician must have a high index of suspicion for occult bony injuries, especially if the patient fails to recover as expected. Aggressive treatment of the sprained ankle is essential to maintain foot and ankle mobility and prevent prolonged disability and subsequent overuse injuries. Kinetic chain dysfunctions are common in ballet dancers with overuse injuries and commonly follow ankle sprains. They may represent a seconda…

Management of the diabetic Charcot foot.

The diabetic Charcot foot is a major limb-threatening complication of long-term diabetes mellitus and neuropathy. Although first described over 100 years ago, we are still lacking definitive studies regarding its prevalence in this population, precise etiology, or most effective treatments. Trauma in the presence of peripheral sensory neuropathy and abundant arterial perfusion seem to be the primary causal factors leading to this severe foot deformity. Misdiagnosis or delayed diagnosis of osteoarthropathy allows the destructive phase of this disorder to continue with resultant further destruct…

Foot care: focus on the elderly.

Problems in the foot develop as a result of the aging process or systemic disease. Common causes of pain and disability in the elderly are nail and skin problems, predominantly corns and calluses, along with circulatory and structural problems. Because patients with orthopaedic conditions may have preexisting foot problems, it is important for nurses to distinguish between minor foot problems that can easily be treated and more serious conditions that require referral to a specialist. This article discusses pathophysiology of the aging foot, a comprehensive foot assessment, common foot problem…

Free flap reconstruction of the foot.

Free flap reconstruction of the foot has been widely performed in the last 20 years, but choice of a free transferred substitute for the soft tissue of the particular defect remains controversial. The authors present a series of 77 free flaps to the foot performed in 68 patients during October 1976 and September 1997. Long-term follow-up ranged from 12 months to 18 years (median, 44.4 months). Seventy-three flaps were transferred successfully (95%). The indications for a specific flap depended on the localization and extension of the foot defect. In weight-bearing areas the authors favored the…

Tibiotalar arthrodesis in the rheumatoid foot.

The results of 14 open ankle arthrodeses in 13 patients with rheumatoid arthritis are reviewed. Ten arthrodeses were treated with Charnley compression clamps (six of these were combined with a fibular strut graft). Three had cannulated screws and one had a fibular strut graft and a short leg walking cast. The mean followup time was 41.2 months. There were five nonunions and one delayed wound healing. The results of the patients treated with fibular strut grafts are far better than the results in patients treated with open ankle arthrodesis without grafting procedure. This seems a promising tec

Clinical implications of diabetes on the foot.

OBJECTIVE: Athletic trainers must understand the clinical implications of diabetes on the athletic foot in order to promote proper foot care and footwear and to adapt protocols for treatment and exercise of the affected athlete. DATA SOURCES: The MEDLINE and CINAHL databases were searched for the years 1984 to 1996 with the terms "diabetes and foot," "neuropathy," and "Charcot joint." DATA SYNTHESIS: As more athletes with diabetes participate in sports, athletic trainers must develop the skills and knowledge necessary to manage this metabolic illness. Although the need to keep blood glucose …
