An infant with acrodermatitis continua of Hallopeau: successful treatment with thalidomide and UVB therapy.

Acrodermatitis Continua of Hallopeau is a rare, chronic, recurrent disorder classified as a form of pustular psoriasis, and most cases affect one or two digits. It tends to be resistant to both topical and systemic treatments for psoriasis. We present an infant with Acrodermatitis Continua of Hallopeau affecting nineteen nails, with an excellent response to the combination of thalidomide and ultraviolet B phototherapy.

Nondermatophytic filamentous fungi infection in South America--reality or misdiagnosis?

For the last 10 years, a steady increment in the diagnosis of nondermatophyte filamentous fungal infections has been observed. This trend also applies to the valuation of the medical mycology. It can be attributed, in part, to the increased concern of the population with the aesthetic aspect, the easiest access to health information, and the increased demand for specialized jobs. Moreover, increase in the diagnosis of new emerging fungi is also caused by the improvement of the diagnostic techniques available as well as the qualification and constant update of the professionals in medical mycol

[595 nm pulse dye laser therapy for viral warts: a single-blind randomized comparative study versus placebo].

INTRODUCTION: Pulse dye laser (PDL) appears an attractive method to treat warts. However, data concerning the efficacy of this approach are based chiefly upon open clinical studies or case reports and results vary widely from one study to another. The purpose of this study was to compare the efficacy and safety of PDL with a placebo in the treatment of a homogeneous group of patients presenting palmoplantar warts. METHODS: This was a randomized, prospective, placebo-controlled, single-blind study conducted between December 2004 and July 2005 in patients consulting for palmoplantar warts at th

Photodynamic therapy with 20% aminolevulinic acid for the treatment of recalcitrant viral warts in an Asian population.

BACKGROUND: Topical photodynamic therapy (PDT) is based on the principle of targeted tissue destruction using selective photosensitization via a topical porphyrin precursor, followed by light exposure. It is well established for the treatment of actinic keratoses and superficial nonmelanoma skin cancers. Some studies have reported good efficacy when using PDT to treat viral warts in the Western population. METHODS: We carried out a prospective, single-arm, phase II study of 5-aminolevulinic acid (5-ALA)-PDT in the treatment of recalcitrant viral warts in an Asian population. Recalcitrant vira

Idiopathic palmoplantar eccrine hidradenitis in children.

Idiopathic palmoplantar eccrine hidradenitis (IPPH) is a recently described disorder characterized by painful erythematous plantar nodules and in three cases, showed a typical neutrophilic infiltrate around and within the eccrine sweat apparatus. Five cases of IPPH on the soles of the feet in healthy children are reported. The disorder presented after intense physical activity in four cases. The course was benign and self-limiting. Complete bed rest for several days without any medical therapy led to alleviation of the pain and disappearance of all the lesions. Conclusion. Idiopathic palmoplan

Costello syndrome with decreased glucose tolerance.

We report a case of Costello syndrome, which is an uncommon multisystemic condition with cutaneous manifestations on the palms and soles. In the literature there are 29 cases described, all the studies are published in the genetic literature with a few exceptions. We add a further case associated with impaired glucose tolerance. The diagnostic clinical signs are impressive, and highly characteristic. Cutaneous manifestations are: loose skin of the hands and feet "washer woman's hand", hyperkeratosis palmoplantaris, curly or sparse hair, acanthosis nigricans, papillomata nasi. Coarse, progeroid

Management of onychomycosis in children.

Onychomycosis is less common in children than in adults. When onychomycosis is suspected, appropriate mycologic tests should be carried out for confirmation. Tinea pedis may be associated with onychomycosis. Family members also may have onychomycosis or tinea pedis. Oral antifungal therapy is required, particularly when the extent of onychomycosis is moderate or severe. The longest clinical experience with antifungal therapy is with griseofulvin, a drug that has to be administered for several months. The newer oral antifungal agents for the treatment of onychomycosis in children include itraco

Recalcitrant viral warts: results of treatment with the KTP laser.

We report the use of a potassium-titanyl-phosphate (KTP) continuous wave laser in an open study to treat 25 patients with multiple nongenital warts that had failed to respond to conventional therapies. All patients were treated monthly using the 532 nm KTP continuous wave laser and robotic scanner. Twenty patients (80%) responded to treatment with warts: in 12 patients there was complete clearing. Moderate discomfort was reported during the procedure but subsequent morbidity was slight with no evidence of scarring. Follow-up by postal questionnaire revealed that warts recurred in those patient

Generalized cutaneous leishmaniasis in acquired immunodeficiency syndrome.

BACKGROUND: Leishmaniasis associated with HIV/AIDS has been reported in the last decade. CASE REPORT: A case of generalized cutaneous leishmaniasis in a patient with AIDS is described. The case is paradoxical in that the cutaneous lesions resemble anergic leishmaniasis caused by Leishmania (L.) amazonensis and Jorge Lobo's disease, but the positive Montenegro test and the clinical improvement with antimonial are more commonly observed in leishmaniasis caused by L.(V.) braziliensis. COMMENT: The immunosuppression due to HIV/AIDS seen in this patient may explain the dissemination of the lesion

Ulcerative and oral lichen planus associated with sicca syndrome and primary biliary cirrhosis.

The following case report supports the hypothesis that the coexistence of lichen planus, primary biliary cirrhosis, and sicca syndrome is not co-incidental. The fact that all three diseases are manifest in the same patient substantiates a theory of pathogenesis based on a cell-mediated immune response analogous to that in allogeneic bone marrow transplant recipients with chronic graft-versus-host disease.

Successful 2-week treatment with terbinafine (Lamisil) for moccasin tinea pedis and tinea manuum.

A new orally active antifungal agent, terbinafine, was used in the treatment of tinea pedis ('dry type' or moccasin type) and tinea manuum. Fifty-three adults over the age of 16 years with fungal infections of the feet and/or hands were treated with either oral terbinafine, 250 mg, or placebo, once daily for 2 weeks. The diagnosis of fungal infection was confirmed by examination of skin scrapings by microscopy and culture. Of these, 28 patients were evaluable for efficacy. At 8 weeks, 12 out of 14 (86%) patients who received terbinafine were mycologically negative (microscopy and culture) comp

[How do I treat dyshidrosiform eruptions?].

Dysfunction of sweat glands and acrosyringia are not involved in the pathogenesis of dyshidrosis. Clinically, dyshidrotic dermatitis (atopic, toxic, contact allergic, hematogen-allergic) and dyshidrotic tinea should be differentiated. Therapeutic possibilities comprise symptomatic topical and systemic treatment, whereas causal treatment is only possible in cases of tinea.

[Vertebral and thoracic osteoarticular manifestations in palmoplantar pustulosis].

Two cases of inflammatory joint disease during attacks of aseptic palmo-plantar pustulosis are reported. Both patients had vertebral involvement: disco-vertebral erosions at several levels in one case and spondylitis and intersomatic ossification in the other. One patient also had sacroiliitis and involvement of the anterior thoracic skeleton (costoclavicular joints, costal cartilages ans xiphoid) confirmed by bone scintigraphy. The relation between this cutaneous-articular syndrome and other inflammatory joint diseases, especially ankylosing spondylitis and psoriatic arthropathy are discussed

A questionnaire study of road pavers' and roofers' work-related skin symptoms and bitumen exposure.

BACKGROUND: Skin contact with the components in asphalt and bitumen can lead to irritant and allergic contact dermatitis, but few data are available in the dermatologic literature on the skin symptoms caused by work with bitumen. In addition, working methods have changed markedly during recent decades. METHODS: A questionnaire was delivered to 50 roofers and 101 road pavers. The questionnaire dealt with skin symptoms, symptoms caused by solvent products, the use of personal protection, smoking, eating habits, work conditions, changing and cleaning of overalls, etc. RESULTS: Forty-eight per c

Onychomycosis.

Onychomycosis is a fungal infection of the nail, and the most common nail infection worldwide, causing discoloration and thickening of the nail plate. It is predominantly caused by dermatophytes. Clinical presentation is polymorphous. Diagnosis must be confirmed by mycological examination before initiating any therapy. Management is an ongoing challenge, often requiring several months' treatment, with a high risk of recurrence. Treatment must be adapted to clinical presentation and severity and to the patient's history and wishes. Debridement of all infected keratin is the first step, reducing

Acitretin: Could it be a new therapeutic player in the field of onychomycosis?

BACKGROUND: Onychomycosis represents a therapeutic challenge. The complete cure rate with itraconazole pulse therapy remains unsatisfactory implying the need for an effective therapeutic regimen. Given the successful treatment of recurrent dermatophytosis with isotretinoin and itraconazole, we investigated the therapeutic use of acitretin in onychomycosis. AIM: To evaluate and compare the efficacy of combined itraconazole and acitretin versus monotherapy with each in onychomycosis. PATIENTS AND METHODS: The study included 135 adult patients with finger- and/or toenail onychomycosis. They wer

Monotherapy for toenail onychomycosis: a systematic review and network meta-analysis.

BACKGROUND: Onychomycosis is a fungal infection of the nail caused by dermatophytes, yeasts and nondermatophyte moulds that accounts for approximately 50% of all nail-related disease. OBJECTIVES: This study aims to assess the effectiveness and safety of monotherapy and combination treatments for toenail onychomycosis using a network meta-analysis (NMA). METHODS: Quality of evidence was assessed using Cochrane-compliant rules and the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. Efficacy and safety outcomes were compared using a random-effects NMA to es
