Fracture dislocation of the glenoid fossa with open physis: a case report.

BACKGROUND: Scapular fractures are uncommon injuries that account for 1% of all fractures. Ten percent of these lesions involve the glenoid fossa and intraarticular displacement is rare, not exceeding 10% of the cases. The classification of intraarticular glenoid fractures was described by Ideberg on the basis of his findings in 100 patients with this type of injury. His classification, however, does not include a dislocation of the glenoid fossa without displacement of the humeral head. METHODS: We report the case of a fracture dislocation of the glenoid fossa without displacement of the hum…

Dual mobility cup reduces dislocation rate after arthroplasty for femoral neck fracture.

BACKGROUND: Hip dislocation after arthroplasty for femoral neck fractures remains a serious complication. The aim of our study was to investigate the dislocation rate in acute femoral neck fracture patients operated with a posterior approach with cemented conventional or dual articulation acetabular components. METHODS: We compared the dislocation rate in 56 consecutive patients operated with conventional (single mobility) cemented acetabular components to that in 42 consecutive patients operated with dual articulation acetabular components. All the patients were operated via posterior approa

[Bilateral radiocarpal dislocation with fracture of the radial styloid. A case report].

INTRODUCTION: Radiocarpal dislocation is a rare injury and thus the number of cases described is limited. The course of the injury is favorable when the treatment provided is appropriate. CLINICAL CASE: A 21-year-old male patient who sustained radiocarpal dislocation and fracture of the radial styloid process in both wrists. He was treated with closed reduction and external fixation. The evaluation at 3 months showed favorable results. DISCUSSION: This case is one of the few similar cases reported because it is an uncommon pathology and it is even less frequent because the injury involves bo

Proximal tibiofibular dislocation associated with fracture of the tibia: a case report.

INTRODUCTION: We report a case of proximal tibiofibular dislocation associated with an ipsilateral tibial fracture, a rare association of injuries that can remain undiagnosed. CASE PRESENTATION: A white 23-year-old man experienced a road accident and was diagnosed with proximal tibiofibular dislocation associated with an open fracture of the tibia and injury to the external popliteus sciatic nerve. He was treated immediately with an intramedullary tibial nail and a cancellous screw at the level of the proximal tibiofibular articulation. CONCLUSION: In addition to this case and the surgical t

Traumatic fracture-dislocation of the hip following rugby tackle: a case report.

Posterior fracture-dislocation of hip is uncommonly encountered in rugby injuries. We report such a case in an adult while playing rugby. The treating orthopaedician can be caught unaware and injuries in such sports can be potentially misdiagnosed as hip sprains. Immediate reduction of the dislocation was performed in theatres. The fracture was fixed with two lag screws and a neutralization plate. This led to early rehabilitation and speedy recovery with return to sporting activities by 12 months.

Irreducible fracture dislocation of the ankle caused by tibialis posterior tendon interposition.

A patient with severe irreducible open fracture dislocation of the ankle was admitted to our emergency department. After wound irrigation and debridement, skeletal traction was applied to the calcaneus to minimize soft tissue injury and swelling. The patient was followed in traction for 1 week, after which reduction and fixation of the fibula was attempted but not achieved. We extended the incision distally, visualized the ankle, and located the tibialis posterior tendon between the distal tibia and fibula, thereby inhibiting the reduction. The tendon coursed into the tibiotalar joint anterior…

Ipsilateral anterior then irreducible posterior hip dislocation without fracture: a case report.

Anterior hip dislocations and posterior hip dislocations are injuries that commonly result from high-energy trauma. Different mechanisms of injury and forces are typically required for anterior and posterior hip dislocations. We present the case of a patient who sustained an injury that initially resulted in an anterior hip dislocation. After reduction and without experiencing further significant trauma, the patient dislocated posteriorly while being transferred from a stretcher to a table in radiology some 14 hours later. To our knowledge, there have been no such presentations in the literatu

Asymmetrical bilateral traumatic hip dislocation in an adult with bilateral acetabular fracture.

This case report aims at presenting a unique case of simultaneous bilateral traumatic dislocation with right side anterior and the left side posterior together with bilateral acetabular fracture. Under general anesthesia, closed reduction of both hips was carried out. The case presented represents an unusual, severe combination of injuries resulting from a high-speed motor-vehicle accident. Traumatic hip dislocation represents a true orthopedic emergency. Given the severity of the associated complications, every effort should be made to ensure prompt diagnosis and immediate therapy.

Intrathoracic humeral head fracture-dislocation: is removal of the humeral head necessary?

Intrathoracic fracture dislocation of the humeral head due to a blunt trauma is very rare. It may be accompanied by local and systemic injuries associated with high-energy trauma. Because a limited number of cases were reported, appropriate treatment modality remains unclear. A case of intrathoracic humeral head fracture-dislocation caused by a high-speed motor vehicle accident is presented herewith, along with the treatment methods used within the scope of the current literature.

Adequacy of pain relief in closed manipulative reduction of fracture and dislocation.

Adequate pain relief is a requisite for a successful closed manipulative reduction (CMR) of fractures and dislocations. This prospective study was undertaken to assess the mode and adequacy of pain relief given to patients undergoing such procedures at Seremban Hospital from the 1st April to the 31st May 2001. All patients with fractures and dislocations scheduled to undergo CMR were included in this study. The type of sedative agents and analgesia administered were recorded. Demographic data and the type of fracture or dislocation of the selected patients were documented. A visual analogue sc

Surgery for acute subaxial traumatic central cord syndrome without fracture or dislocation.

Twenty-two patients with subaxial acute traumatic central cord syndrome (CCS) without fracture or dislocation who underwent surgery between 1995 and 2002 were reviewed, retrospectively. There were 13 males and nine females ranging in age from 24 to 84 years (mean 61.2). Falls were the most common injury (68%), followed by motor vehicle accidents (32%). All patients had dynamic cervical lateral radiographs and magnetic resonance imaging (MRI). Cord compression was present in all cases and cervical instability in 11. Associated pathology included disc herniation in seven patients, cervical spond

CASE REPORTS: unusual lateral clavicle fracture dislocation: case reports and museum specimens.

We present two clinical cases and four museum specimens of a type of lateral clavicle fracture dislocation that has not been detailed in any of the commonly accepted classification systems. With this injury, the major clavicular element is displaced downward and backward into the supraspinatus outlet and may cause symptomatic impingement. A method for surgical correction is described.

The Essex-Lopresti fracture-dislocation factors in early management and salvage alternatives.

Treatment recommendations for the Essex-Lopresti lesion have not come very far in 50 years. Although there have been multiple biomechanical studies, the biomechanics of forearm loading and stability remain somewhat elusive. Clinical studies have yielded some insight, but predictable outcomes are exceptional. More studies are needed to further understand the biomechanics of the forearm and provide a basis for reconstruction of the IOL. Although current clinical studies regarding IOL reconstruction and radial head replacement seem promising, long-term results with substantial patient numbers are

Operative treatment of a transverse fracture-dislocation at the S1-S2 level.

The authors recently treated a multiply injured patient who sustained a rare fracture-dislocation at the S1-S2 level. The S1 vertebra was displaced forward into the pelvic cavity and was located just in front of the S2 vertebra. Because the patient also had extensive neurologic injury to the lumbar plexus and instability of the pelvic ring, operative treatment was deemed necessary. Surgery to stabilize the pelvis and decompress the lumbar plexus proved successful, and the patient experienced marked improvement in her postoperative neurologic function. Nonoperative treatment has traditionally b

Acute cervical cord injury without fracture or dislocation of the spinal column.

OBJECT: It is known that the spinal cord can sustain traumatic injury without associated injury of the spinal column in some conditions, such as a flexible spinal column or preexisting narrowed spinal canal. The purpose of this study was to characterize the clinical features and to understand the mechanisms in cases of acute cervical cord injury in which fracture or dislocation of the cervical spine has not occurred. METHODS: Eighty-nine patients who sustained an acute cervical cord injury were treated in our hospitals between 1990 and 1998. In 42 patients (47%) no bone injuries of the cervic

Late superior gluteal nerve palsy following posterior fracture-dislocation of the hip.

An unusual case of late superior gluteal nerve palsy complicating posterior fracture-dislocation of the hip is reported. Posterior fracture-dislocation of the hip was Grade V according to the classification of Thompson and Epstein, and Type IV according to the subclassification of Pipkin. The palsy resulted from traction by scar tissue formation. Excision of the scar tissue and decompression of the superior gluteal nerve led to complete recovery.
