Intraoperative modular stem lengthening to treat periprosthetic femur fracture.

Fracture of the femur around a femoral prosthesis can be a difficult problem to manage. Treatment options consist of traction, allograft struts, or plating with cerclage fixation, and revision arthroplasty. Intramedullary stabilization is usually preferable to plating for treatment of diaphyseal femur fractures; however, for a fracture around a cementless stem, disruption of the biologically fixed proximal area of the implant and revision to a long-stem component are generally required to permit intramedullary fixation of the fracture. Many modular femoral stems are currently available. Some o

Injuries associated with femoral shaft fractures with special emphasis on occult injuries.

BACKGROUND: Fractures of the femoral shaft are mostly the result of high-energy accidents that also cause multiple trauma injuries, in particular ipsilateral knee and hip injuries. The purpose of this study was to investigate the incidence of injuries associated with femoral shaft fractures and how many of them were undetected. METHODS: We studied 148 patients (150 femoral shaft fractures) with an average age of 52 (range: 18-97). Femoral shaft fractures were treated with antegrade intramedullary nailing in 118 cases (78.7%), and with open reduction and internal fixation in 32 cases (21.3%).

Intramedullary nailing of pediatric femoral fractures.

This retrospective study reviews our results with intramedullary nail fixation of 37 fractures of the femur in 35 skeletally immature patients. Five of these fractures were open. Twenty-two patients (average age 12 + 9 years) were treated with reamed intramedullary nails. Fifteen patients (average age 9 + 6 years) were treated with nonreamed nails. All fractures united in 6-12 weeks. There were no infections, delayed or nonunions, nor were there any incidences of avascular necrosis. There were very few significant complications. One patient required excision of heterotopic bone to restore hip

Evaluation and treatment of metastases to the humerus.

The humerus is the second most common long bone site of metastatic disease, and the proximal third and diaphysis are frequent sites. Purely lytic disease and cortical bone destruction increase the risk of fracture more than 50%. For most patients, external beam irradiation is effective as a means of pain control and halting bone destruction. Fractures of the head or surgical neck can be treated with standard endoprostheses, whereas extensive proximal bone destruction is treated with custom proximal humeral replacements. Impending and complete diaphyseal fractures can be treated effectively wit

Titanium elastic nailing radius and ulna fractures in adolescents.

Over the last 40 years, anatomic reduction with plate stabilization has become the standard in adult patients with diaphyseal fractures of the radius and ulna. When operative fixation has been indicated in skeletally immature patients with these fractures, a variety of techniques have been reported, with intramedullary fixation becoming increasingly accepted. There is currently significant variability in the treatment of adolescents with forearm fractures. The purpose of this study was to investigate the clinical and radiographic outcomes in the adolescent population after intramedullary fixat

Atypical femoral fractures related to bisphosphonate treatment: issues and controversies related to their surgical management.

AIMS: Treatment guidelines for atypical femoral fractures associated with bisphosphonates have not been established. We conducted a systematic review of the treatment of atypical femoral fractures first, to evaluate the outcomes of surgical fixation of complete atypical fractures and secondly, to assess whether prophylactic surgery is necessary for incomplete atypical fractures. MATERIALS AND METHODS: Case reports and series were identified from the PubMed database and were included if they described the treatment of atypical femoral fractures. In total, 77 publications met our inclusion crit

Biomechanical comparison of an intramedullary nail combined with a reconstruction plate combination versus a single intramedullary nail in unstable intertrochanteric fractures with lateral femoral wall fracture: A finite element analysis.

This study aimed to compare the biomechanical performance of an intramedullary nail combined with a reconstruction plate and a single intramedullary nail in the treatment of unstable intertrochanteric femoral fractures with a fracture of the lateral femoral wall (LFW). A three-dimensional finite element (FE) femur model was established from computed tomography images of a healthy male volunteer. A major reverse obliquity fracture line, associated with a lesser trochanteric fragment defect and a free bone fragment of the LFW, was developed to create an AO/OTA type 31-A3.3 unstable intertrochant

Interlocking intramedullary nail stabilization of a femoral fracture in a dog with osteomyelitis.

Complications developed in a dog that underwent intramedullary pin and cerclage wire fixation of a comminuted femoral fracture. The fracture was unstable, the intramedullary pin protruded through the skin over the hip, and Staphylococcus sp was isolated from the fracture site. The loose pin was removed, and the fracture was restabilized with a 6.0-mm interlocking intramedullary nail and 3 interlocking screws. Cephalexin was given orally for 6 weeks. Eight weeks after the second surgery, the fracture was healed radiographically. Seventeen months later, the dog had a persistent mild lameness tha

CLINICAL EVALUATION OF THE TREATMENT OF CLAVICLE FRACTURES: INTRAMEDULLARY NAIL × PLATE.

OBJECTIVE: Studies confirm the benefit of surgical treatment for fixation of displaced midshaft clavicle fractures. Plate fixation and intramedullary nail are the two most used techniques. Our study seeks to compare these two surgical techniques. METHODS: This is a retrospective study, conducted by the evaluation of patients treated for displaced midshaft clavicle fracture with intramedullary nail, and plate and screws. Socioeconomic variables were collected, a visual pain scale questionnaire was applied, the shoulder function was measured using CONSTANT and UCLA scores, and radiography was p

Use of compression screw in a metacarpal head Fracture: The "lollipop" technique.

Only 4 % of metacarpal fractures affect the distal articular region. In the literature, some studies have ended up causing confusion by mixing up extraarticular neck fractures with subcapital fractures, a term that should only be used to define transverse articular fractures of the head that are distal to the origin of the collateral ligament. Due to the need for anatomical reduction, displaced metacarpal head fractures must be reduced and fixed, mostly by open surgery. Recently, the use of intramedullary fixation has become the method of choice in the treatment of transverse metacarpal diaphy

The crest-pillar-column method for percutaneous fixation of the posterior column using a curved intramedullary device: surgical technique.

Treatment of posterior column fractures can be treated with open reduction internal fixation via plate osteosynthesis or with screw fixation. Due to common fracture planes and fixation trajectories, traditional anterograde posterior column screw fixation requires an open approach to access the starting point on the inner table. Retrograde posterior column implantation requires a start point in areas that are difficult to drape and access with familiar patient positioning and confident sterility. The advent of curved intramedullary implants allows implantation in nonlinear osseous fixation path

Percutaneous reduction and flexible intramedullary nailing for monteggia fracture in a skeletally mature patient.

INTRODUCTION: Monteggia lesions are defined as a fracture of the proximal ulna with an associated radial head dislocation. Management of these injuries varies depending upon the patient population, ranging from non-operative treatment with closed reduction and immobilization to surgical fixation. Percutaneous techniques of radial head reduction are often reserved for skeletally immature patients. PRESENTATION OF CASE: In this case report, a 14-year-old female presented with left elbow pain three days after a fall. Radiographs and CT images from an outside hospital revealed a displaced left ra

Metacarpal Fractures: An Evidence-Based Review to Guide Treatment.

Metacarpal fractures are among the most common fractures of the hand and upper extremity and can lead to substantial disability. Clinical and radiographic features of these fractures are important in approaching their management. While most metacarpal fractures can be treated nonsurgically with reduction and/or immobilization, some fractures and patients can have improved patient-centered outcomes with surgical fixation. Outcomes of surgical fixation (eg, percutaneous Kirschner wire fixation, plate/screws, interfragmentary screws alone, and intramedullary fixation) and postoperative care are b

Osteolysis after rigid fixation. The possible role of periosteal neural mechanoreceptors in bone remodeling.

Rigid intramedullary fixation in the rat tibiofibular bone of a tibial fracture caused the development of nonunion in the associated fibular fracture, with marked osteolytic resorption of the fibular fragments. In contrast, with flexible fixation, the associated fibular fracture healed normally. The tibial fractures healed uneventfully in both cases. Histologic examinations showed that there exists a plexus of encapsulated nerve endings (Pacinian corpuscles) in the periosteum of the fibula, at the distal tibiofibular junction. The rat fibula forms a bony contact with the lower midshaft of the

The Impact of Fibular Fixation Method on Pilon Fracture Healing.

Background: Pilon fractures are associated with high-energy injuries, and there is presently much debate as to optimal fixation strategies and timing of intervention. There is little evidence comparing the type of fibular fixation during pilon fracture fixation. The purpose of this study was to compare fibular fixation methods in complex pilon injuries as it relates to pilon union rates and development of post-traumatic arthritis. Methods: This was a retrospective review from an urban Level 1 trauma center from January 2009 to May 2019, including patients age ≥ 18 who sustained a pilon fractur

Adjunctive IlluminOss Fixation for a Vancouver B1 Periprosthetic Femoral Fracture: A Case Report.

Open reduction and internal fixation with plates and intramedullary nails is often the standard approach for periprosthetic femoral shaft fractures. However, in patients with recent hip arthroplasty, these traditional methods may be invasive, difficult to position, or carry a higher risk of periprosthetic complications. The IlluminOss system (IlluminOss Medical, East Providence, RI, USA) offers a minimally invasive alternative that delivers a light-curable polymer through a balloon catheter that takes the shape of the intramedullary canal without the need for rigid instrumentation that may ind

Functional Outcome of Forearm Fractures Managed With Screw Nails vs. Dynamic Compression Plates: A Prospective Study.

Background Fractures of the forearm are very frequently encountered in day-to-day practice. These fractures have a bimodal age distribution. The forearm fractures are considered intra-articular and need absolute stability for adequate healing. The current treatment modalities include using intramedullary devices such as a square nail, locking intramedullary nail, or using a plate for fixation. In this study, we aim to determine the functional outcome of forearm fractures managed with a screw nail used as an intramedullary device as compared to a locking plate using the Grace-Eversmann criteria
