Fractures of C1 and C2 in an infant gazelle: a model of the human fracture pattern.

Spontaneously occurring fractures of the anterior and posterior synchondroses of C1 and the superior facet of C2 of an immature mammal were analyzed. The posterior C1 injury showed comminution, with involvement of both sides of the synchondrosis. Just below the anterior C1 fracture was an incomplete, comminuted fracture of the C2 superior facet. These fractures, especially the C1 injury, appear similar to the radiographic injury patterns in skeletally immature humans. Presumably the histologic failure patterns in the child would also be similar.

Treatment of long bone fractures in South American camelids: 5 cases.

An adult llama and four alpacas were referred with long bone fractures. The llama presented with a closed, comminuted fracture of the right metatarsal bones. Two of the alpacas presented with comminuted fractures of the proximal radius. One fracture was closed and one was open. One alpaca had a closed, comminuted fracture of the distal radius, and the final alpaca had a closed, oblique fracture of the metatarsus. A diagnosis was made in each animal on the basis of clinical examination and radiographs. All fractures were managed by open reduction and internal fixation using selective placement

A Case Report: Interposition Arthroplasty for Post-traumatic Elbow Arthritis.

INTRODUCTION: Humeral shaft fractures constitute 1-1.5% of all fractures, with most occurring in the middle and distal segments. While conservative treatment is common, surgical fixation is often preferred for displaced or comminuted fractures, particularly in young, active individuals. Minimally invasive plate osteosynthesis (MIPO) techniques have been developed to minimize soft-tissue damage and reduce the risks of complications such as non-union and radial nerve palsy. Although anterior approaches are well-studied, the posterior approach remains less explored. This case report highlights th

Cerclage Wire Fixation for Fracture-Dislocations of the Proximal Interphalangeal Joint.

Proximal interphalangeal (PIP) joint fracture dislocations are challenging injuries to treat. Multiple and varied treatments have been proposed. We present the use of cerclage wiring as a helpful technique in these challenging scenarios. The technique has the benefit of securing fracture fragments from the volar or dorsal base of middle phalanges or a comminuted fracture involving the entire articular surface. We report on the use of cerclage wires in eight patients (average: 43 years of age). Three patients had volar base fractures, three dorsal base fractures, and two impacted fractures invo

[External fixation at the wrist and hand].

Distraction radio-metacarpal external fixation is an excellent technique for the osteosynthesis of distal radius fractures, in particular of comminuted articular fractures. The alternative is the locked palmar plate, a more demanding technique. The published literature does not allow concluding if one method is better than the other. The other main possible indications of external fixation at the wrist are comminuted fractures of the base of the thumb metacarpal, distal radius osteotomies, and wrist arthrodeses. At the hand, external minifixation is an excellent technique of osteosynthesis. St

Closed treatment of subtrochanteric fractures of the femur in a modified cast-brace.

We studied a series of fifteen consecutive subtrochanteric fractures treated in a long quadrilateral cast-brace with a pelvic band. Patients with severely comminuted fractures in which stability cannot be obtained by internal fixation, as well as those with open fractures, are considered candidates for such treatment. Treatment with preliminary traction followed by a ambulatory cast-brace with a pelvic band resulted in a shorter period of treatment, an excellent range of motion of the hip and knee, and no non-unions in the fifteen comminuted or open fractures. Shortening, angulation, and rotat

A classification and grading system for Barton fractures.

PURPOSE: We described a morphological classification and grading system for volar Barton fractures. METHODS: We divided these fractures into four types: typical Barton, ulna Barton, radial Barton, comminuted Barton. Moreover, we graded the fractures into two degrees: simple split and split-depression. We retrospectively reviewed all wrist radiographs showing Barton fractures in our hospital between January 2013 and January 2015. We identified 100 cases whose records and radiographs were reviewed and included 36 men and 64 women with a mean age of 50 years (15-78). The morphological classifica

[Long-term results of surgical management of patellar fractures].

Between 1973 and 1989, 129 patients with 135 fractures of the patella were treated operatively. More than two-thirds of these patients were younger than 40 years at the time of the accident. The main cause of trauma was a traffic accident in 65.9%. Sixty patients had polytraumata. There were 74 comminuted, 28 transverse, 17 starlike, 10 longitudinal and 6 fractures with one large fragment. Eighty-two patients with 85 fractures were followed up after 8.4 years on average (1.2-16.3 years). The overall results were excellent or good in 64.7%, fair in 18.8% and poor in 16.5%. The best results were

Zone 2 sacral fractures managed with partially-threaded screws result in low risk of neurologic injury.

BACKGROUND: Zone 2 sacral fractures account for 34% of sacral fractures with reported neurological deficit in 21-28% of patients. The purpose of this study was to examine the risk factors for neurological injury in zone 2 sacral fractures. The authors hypothesized that partially thread iliosacral screws did not increase incidence of neurologic injury. METHODS: A retrospective review of consecutive patients admitted to a level 1 trauma center with zone 2 sacral fractures requiring surgery from September 2010 to September 2014 was performed. Patients were excluded if no neurologic exam was avai

Fractures of the nasolacrimal fossa and canal: a CT study of appearance, associated injuries, and significance in 25 patients.

The bony nasolacrimal fossa and canal, which protect the more distal excretory portion of the lacrimal apparatus, the nasolacrimal sac and duct, are contained within the medial portion of the orbit and lateral aspect of the nose, sites that are commonly injured in facial trauma. The CT scans of 25 patients who sustained fractures of the nasolacrimal fossa and/or canal as a result of motor vehicle accidents were reviewed to determine the appearance of the fractures and to determine types of facial fractures also present. The patients' clinical records were reviewed to determine the frequency of

[Fracture of the tibial head].

Fractures of the tibial head are severe injuries, characterized by enormous variety. Fractures can be classified into fractures of the tibial plateau, luxation fractures, and comminuted fractures. Due to the mechanism of injury luxation fractures are frequently associated with lesions of the menisci and intra- and extra-articular ligaments. Multiple factors can be etiologic for post-traumatic gonarthrosis: nonanatomic reduction of the joint surface, malalignment, and unaddressed associated injuries. Therefore in addition to diagnostic steps such as X-ray, CT scan, and MRI a sophisticated thera

Internal distraction plating of distal radius fractures.

Internal distraction plating can be used for the treatment of highly comminuted distal radius fractures especially in elderly patients. The technique involves the use of 3.5, 2.7, or 2.5 dynamic compression plates. The instrumentation is applied in distraction dorsally from the radial diaphysis, bypassing the comminuted segment, and fixed distally to the long metacarpal. The advantages of this technique are: a) it can be used as an alternative for managing difficult fractures in the elderly population; b) it is indicated in patients with osteoporotic bone; c) complications associated with exte

A Comprehensive Study of Internal Distraction Plating, an Alternative Method for Distal Radius Fractures.

INTRODUCTION: The management of highly comminuted distal radius fractures still remains a major treatment challenge. Articular comminution and compromised bone quality are the culprits. One novel approach is the technique of Internal Distraction Plating which involves "bridging" the fracture with the use of a standard 3.5mm plate applied dorsally in distraction from the radius, proximal to the fracture, to the long finger metacarpal distally, bypassing the comminuted segment. The plate is removed once fracture union has been achieved. AIM: The present study was conducted with the aim to evalu

Relationship between distal radius fracture severity and 25-hydroxyvitamin-D level among perimenopausal and postmenopausal women.

AIMS: Low-energy distal radius fractures (DRFs) are the most common upper arm fractures correlated with bone fragility. Vitamin D deficiency is an important risk factor associated with DRFs. However, the relationship between DRF severity and vitamin D deficiency is not elucidated. Therefore, this study aimed to identify the correlation between DRF severity and serum 25-hydroxyvitamin-D level, which is an indicator of vitamin D deficiency. METHODS: This multicentre retrospective observational study enrolled 122 female patients aged over 45 years with DRFs with extension deformity. DRF severity
