Pericranial flap fixation using titanium miniplates and screws.

Opening the frontal sinuses is a common occurrence during neurosurgical procedures involving the anterior cranial base. Obliteration of the exposed sinuses is usually accomplished packing them with various materials and/or covering them with an anteriorly-based pericranial flap. In the absence of more extensive cranial base defects, the flap only needs to be wrapped around the sinus and secured against its posterior wall. We describe the technique for and the results of using titanium miniplates and screws to secure the pericranial flap to the inner surface of the frontal bone. In 10 patients

Treatment of frontal sinus osteoma using a craniofacial approach.

Osteoma is one of most common benign tumors of the nose and paranasal sinuses, with the frontal sinus being its most frequent location. It may be locally destructive and aggressive with possible intracranial complications. Osteoma of the frontal sinus comprises 57% of all osteomas in the paranasal sinuses. In 1939, Childrey reviewed 3,510 consecutive sinus radiographs taken for any reason and found an incidence of 0.43% of paranasal sinus osteomas. There are many operative approaches for frontal sinus osteomas, such as external frontoethmoidal approach and osteoplastic frontal sinusectomy, bot

Frontal ostium neo-osteogenesis and restenosis after modified endoscopic Lothrop procedure in an animal model.

All patients who undergo a modified endoscopic Lothrop procedure have postoperative narrowing of the enlarged frontal ostium. The aim of this study is to evaluate neo-osteogenesis and restenosis of the frontal ostium and its effect on mucociliary clearance. Fourteen sheep underwent an endoscopic modified Lothrop procedure. Pre- and postoperative nuclear medicine gamma scintigraphy of the frontal sinuses was performed. The sizes of the frontal ostia were measured and biopsies taken from the bone of the frontal ostium. Histological evidence of new bone formation was found in 56% of biopsies. The

Frontal sinus outflow anatomy.

Most of the studies of frontal sinus anatomy were completed 50 to 70 years ago. The information they provide is not necessarily relevant or helpful to the modern rhinologic surgeon who approaches the frontal sinus transnasally and endoscopically. We performed anatomical dissections of the outflow tract of 82 frontal sinuses in 41 cadaver heads to illustrate the various drainage patterns from the frontal sinus to the nose and to correlate these drainage sites with the distance and angle from the pyriform aperture. We found that the frontal sinus drained anterior to the uncinate process in 24 sp

Statistical estimators of frontal sinus cross section ontogeny from very noisy data.

Cross-sectional areas of human frontal sinuses in the occipitofrontal projection are a good surrogate for frontal sinus volumes. This study looks at these areas in a dataset of some 200 children and 100 adults of both sexes. As measured by planimetry of roentgenograms, the areas are extremely variable ("noisy" in a statistical sense). In fact, they appear to be distributed log-normally with quite high variance. The mean of the distribution is evidently a function of age and the variances differ by sex. After logarithmic transformation, the data are adequately fitted by one sigmoid curve for ea

Aspergillosis of the frontal sinus.

Aspergillus is the most common fungal infection of the paranasal sinuses and usually appears as a chronic disease in an otherwise healthy person. Previously reported cases have involved the maxillary sinus alone in the majority of cases. To my knowledge, isolated frontal sinus aspergillosis has not been previously reported. A frontal osteoplastic flap approach was used to expose a fungus ball in the right frontal sinus of a healthy 69-year-old man. Histologic preparations and fungal cultures showed Aspergillus fumigatus. Follow-up substantiated prior experience that surgical removal of the fun

Sino-orbital aspergillosis in a dog.

Fungal rhinitis-sinusitis with orbital involvement was diagnosed in a dog with profuse unilateral ocular and bilateral nasal discharge, enophthalmos, and a corneal descemetocele. The descemetocele was treated with a conjunctival pedicle graft. Frontal sinusotomy was performed, and clotrimazole was infused through catheters placed in the frontal sinuses and nasal cavities to saturate the tissues for 1 hour. Successful resolution of orbital infection may have been aided by a fistula between the frontal sinus and orbit that allowed delivery of clotrimazole to the orbit. The dog retained sight in

Operative findings in the frontal recess at time of revision surgery.

OBJECTIVE: Endoscopic sinus surgery is the gold standard for the treatment of medically refractory chronic rhinosinusitis. There is, however, a population of patients for whom persistent disease is a problem. Of all the sinuses, the frontal sinus is the most likely to have recurrent obstruction. We evaluated the findings causing frontal recess obstruction at the time of revision surgery. STUDY DESIGN AND SETTING: A retrospective review was performed in a tertiary care academic otolaryngology department. RESULTS: Findings obstructing the frontal recess at the time of revision sinus surgery we

Mucoceles of the paranasal sinuses: uncommon location.

Mucoceles are the most common lesions causing expansion of the paranasal sinuses. The sinuses most commonly involved are, in decreasing order of frequency, frontal sinus, ethmoid sinuses, maxillary sinus and sphenoid sinus. We reviewed 46 cases of surgically proven mucoceles and the purpose of this study was to report five cases of mucoceles in an uncommon location.

Thermography for the Diagnosis of Acute Inflammation in the Paranasal Sinus.

Introduction  Although computed tomography scanning is the most common method for the diagnosis of sinusitis today, X-ray imaging is still used in outpatient clinics. Because X-ray imaging is beneficial for patients with severe sinusitis but not for those with mild sinusitis, an alternative method to visualize sinusitis without X-ray imaging is desirable. Objective  To study the possibility of using thermography to visualize sinusitis. Methods  In the present study, heat distribution on the faces of individuals with and without sinusitis was studied using thermography. Overall, 10 control subj

[Endoprosthesis of the frontonasal anastomosis after extranasal radical operations using biocompatible tubular polymeric materials].

The authors present pilot experience of extranasal radical operations on the frontal sinuses with the use of polymeric tube endoprostheses made of biocompatible polymeric materials EFOS 1, 2, 3 containing antimicrobial drugs (dioxidin, chinoxidin) and active substances promoting regeneration and epithelization (orotic acid derivatives). The above technique was employed in operations on frontal sinuses in 12 patients. Polymeric endoprostheses EFOS provide adequate lumen of the formed frontonasal anastomosis, reduce the occurrence of postoperative complications and recurrences as well as hospita

[Pedunculated osteoma of the frontal sinus].

Osteoma, the most frequent tumor of the paranasal sinuses (PNS), is often asymptomatic. Men are more commonly affected than women. A 52-year-old patient complained of increasing incidence of headaches following the resolution of frontal sinusitis. Computer tomography demonstrated a pedunculated osteoma of the frontal sinus. Histomorphologically, PNS osteomas usually show cortical bone differentiation with sparse medullary space and a lamellar configuration. At 6 months following surgical resection, the patient was still symptom-free.

[The frontal pneumosinus dilatans: surgical management by modelling osteoplasty].

UNLABELLED: Pneumosinus dilatans is a rare disease that most commonly occurs in young men. It is characterized by an hyperpneumatization of sinus cavities. Its etiopathogenis remains unclear. The bony and mucosal structures are normal and there is no sinus disorders associated with. Most of patients with pneumosinus dilatans present with a cosmetic complaint. CASE REPORT: We report a new surgical technique performed in a young male with bilateral frontal pneumosinus dilatans. A Bi-coronal incision makes it possible to largely expose the anterior wall of the frontal sinus. This wall is then we

Computed Tomography, Magnetic Resonance, Cross-Sectional Anatomy, and 3D Volume Rendering of the Nasal Cavity and Paranasal Sinuses of the Alpaca (Vicugna pacos).

Understanding the anatomy of the paranasal sinuses and nasal cavity in alpacas (Vicugna pacos) is limited by scarce published data. This prospective cadaveric study aimed to enhance the interpretation of clinical cross-sectional imaging and expand knowledge of normal anatomy using computed tomography (CT), magnetic resonance imaging (MRI), macroscopic cross-sections, and 3D reconstructions (Amira v5.3.3; Thermo Fisher Scientific, Waltham, MA, USA). Seven alpacas (median age 3.9 years; five males, two females) underwent CT and MRI. Frozen anatomical slices were obtained from three heads, and si

Allergic fungal sinusitis with massive intracranial extension presenting with tearing.

A 24-year-old male presented with tearing, and subsequent workup and imaging showed a mass with fluid involving the nasopharynx, the paranasal sinuses, and the posterior dehiscence of the left frontal sinus intracranially compressing the frontal lobe significantly. Microscopic examination confirmed the diagnosis of allergic fungal sinusitis. Endoscopic drainage and sinostomy was performed by the otolaryngology (ear-nose-throat) service. The patient was followed 9 months postoperatively and did well with resolution of the epiphora. Although epiphora alone is an unusual presentation of allergic
