Research Progress for Probiotics Regulating Intestinal Flora to Improve Functional Dyspepsia: A Review.

Functional dyspepsia (FD) is a common functional gastrointestinal disorder. The pathophysiology remains poorly understood; however, alterations in the small intestinal microbiome have been observed. Current treatments for FD with drugs are limited, and there are certain safety problems. A class of active probiotic bacteria can control gastrointestinal homeostasis, nutritional digestion and absorption, and the energy balance when taken in certain dosages. Probiotics play many roles in maintaining intestinal microecological balance, improving the intestinal barrier function, and regulating the i

The role of the gastrointestinal tract in calcium homeostasis and bone remodeling.

While skeletal biology was approached in a rather isolated fashion in the past, an increasing understanding of the interplay between extraskeletal organs and bone remodeling has been obtained in recent years. This review will discuss recent advances in the field that have shed light on how the gastrointestinal tract and bone relate to each other. In particular, the importance of the GI tract in maintaining calcium homeostasis and skeletal integrity will be reviewed as impaired gastric acid production represents a major public health problem with possible implications for sufficient calcium abs

Computer-assisted two-dimensional analysis of gastric mucosal hemoglobin distribution using electronic endoscopy.

An electronic endoscope was modified to assess the imaging of two-dimensional distribution of gastrointestinal mucosal hemoglobin with the aid of a computer and image analyzing system. The band-pass filters ranging from 530-470 nm (G'), and 770-830 nm (IR) replaced ordinary filters in a conventional electronic endoscope (Olympus V-10 or EVIS-200). As the hemoglobin is the most abundant biological pigment in the gastrointestinal mucosa absorbing the green light with little absorption in the infrared region, we used the algorithm of 32 [log2(Vir/Vg)] as a reflection of hemoglobin content in the

Beta-casomorphin (BCM) and human colonic lamina propria lymphocyte proliferation.

BCM is a milk-derived peptide with opiate-like properties which is absorbed through the gastrointestinal mucosa. It has been shown to affect gastrointestinal motility, absorption and secretion. Recently, modulation of the immune system by BCM was also reported. In this study we investigated the in vitro effect of BCM on the human mucosal immune response as represented by lamina propria lymphocyte (LPL) proliferation. Results show that BCM significantly inhibited concanavalin A (ConA) stimulated LPL DNA synthesis. BCM also inhibited ornithine decarboxylase activity (ODC) in ConA-stimulated LPL.

The role of activated charcoal and gastric emptying in gastrointestinal decontamination: a state-of-the-art review.

Gastrointestinal decontamination has been practiced for hundreds of years; however, only in the past few years have data emerged that demonstrate a clinical benefit in some patients. Because most potentially toxic ingestions involve agents that are not toxic in the quantity consumed, the exact circumstances in which decontamination is beneficial and which methods are most beneficial in those circumstances remain important topics of research. Maximum benefit from decontamination is expected in patients who present soon after the ingestion. Unfortunately, many overdose patients present at least

Ferric pyrophosphate in precision iron fortification: mechanistic insights into food-system suitability, bioavailability modulation, and evaluation strategies.

Iron is an essential trace nutrient, and iron deficiency remains a major global public health problem. Iron-fortified foods are important vehicles for preventive iron supplementation. Precision iron fortification requires matching iron sources to specific food matrices to balance product quality and fortification efficacy. Ferric pyrophosphate (FePP) shows good food-system stability and gastrointestinal tolerability, but low solubility and limited bioavailability restrict wider application. This review addresses FePP food-system suitability, bioavailability modulation, and evaluation strategie

Gastrointestinal Mucosal Cell Injury Caused by Sevelamer Crystals: Case Series and Literature Review.

In end-stage kidney disease (ESKD), hyperphosphatemia occurs secondary to decreased renal elimination with continued intestinal absorption of dietary phosphate. Even in chronic kidney disease, glomerular filtration rate lower than 30 mL/min markedly decreases the filtration of inorganic phosphate and increases its serum level. Sevelamer, a noncalcium phosphate binder, is commonly used to control hyperphosphatemia. Available in 2 forms, sevelamer hydrochloride and sevelamer carbonate, it absorbs phosphate in the gastrointestinal tract and is known to have minimal adverse effects. These are limi

Gut dysbiosis impairs intestinal renewal and lipid absorption in Scarb2 deficiency-associated neurodegeneration.

Scavenger receptor class B, member 2 (SCARB2) is linked to Gaucher disease and Parkinson's disease. Deficiency in the SCARB2 gene causes progressive myoclonus epilepsy (PME), a rare group of inherited neurodegenerative diseases characterized by myoclonus. We found that Scarb2 deficiency in mice leads to age-dependent dietary lipid malabsorption, accompanied with vitamin E deficiency. Our investigation revealed that Scarb2 deficiency is associated with gut dysbiosis and an altered bile acid pool, leading to hyperactivation of FXR in intestine. Hyperactivation of FXR impairs epithelium renewal a

Anaphylaxis to Polyethylene Glycol (Colyte®) in a Patient with Diverticulitis.

Polyethylene glycols (PEGs) are believed to be chemically inert agents, but larger PEG polymers could have immunogenicity. A 39-year-old man was referred to emergency room for loss of consciousness and dyspnea after taking of PEG-3350 (Colyte®). In laboratory findings, the initial serum tryptase level was increased to 91.9 mg/L (normal range: 0.00-11.40 mg/L) without any other laboratory abnormalities. The intradermal test with 10 mg/mL Colyte® showed a 5 × 5 mm wheal, but basophil activation and histamine releasability tests were negative. PEG-3350 is widely used as an osmotic laxative due to

Malignancy in adult celiac disease.

Prior studies have suggested that the incidence of some neoplastic disorders, particularly malignant lymphoma and small intestinal adenocarcinoma, are increased in celiac disease. Earlier studies from the United Kingdom have also suggested a link between celiac disease and esophageal carcinoma, although this has not been confirmed in North America. The risk of other gastrointestinal cancers seems to be limited. Gastric cancer does not appear to be detected more frequently, although direct endoscopic visualization of the upper gastrointestinal tract is now very common in patients with celiac di

Interaction of carbamazepine and chlorpromazine in rabbits.

The interaction of carbamazepine and chlorpromazine in rabbits has been studied. The drugs were administrated as single oral doses (200 mg of each drug). The sequence of administration of the drugs was varied. It has been established that by simultaneous administration these drugs decrease absorption of each other in plasma. This may be explained by competition of the drugs to transfer from the gastrointestinal tract into plasma, as well as by the formation of complexes, more or less stable and more or less bound to gastrointestinal tissues. Carbamazepine intensifies the biotransformation of c

Gastrointestinal loss of galanthamine hydrobromide in rats in-situ.

The disappearance kinetics of the acetylcholinesterase inhibitor galanthamine hydrobromide from the gastrointestinal tract of male Wistar rats, 200-250 g, in-situ has been examined. After 30 min the galanthamine loss was 16% in the stomach (pH 2), 54-85% in the duodenum and the successive small intestinal segments (pH 6.8), 43% in the colon and 76% in the rectum. Compared with the other segments, the disappearance rate was higher in the terminal ileum (0.38 x 10(-2) mg cm-1 min) and in the rectum (1.27 x 10(-2) mg cm-1 min). In the proximal jejunum, terminal ileum and rectum the disappearance

Dose-dependent absorption of chlorogenic acids in the small intestine assessed by coffee consumption in ileostomists.

SCOPE: Until now, the question of how the ingested doses of chlorogenic acids (CGA) from coffee influence their absorption and metabolism remains unresolved. To assess absorption in the small intestine, we performed a dose-response study with a randomized, double-blinded, crossover design with ileostomist subjects. METHODS AND RESULTS: After a polyphenol-free diet, the volunteers consumed, on three separate occasions, coffee with different total CGA contents (high 4525 μmol; medium 2219 μmol; low 1053 μmol). CGA concentrations in plasma, ileal effluent, and urine were subsequently determined

Application of In Vivo MRI Imaging to Track a Coated Capsule and Its Disintegration in the Gastrointestinal Tract in Human Volunteers.

Oral specially coated formulations have the potential to improve treatment outcomes of a range of diseases in distal intestinal tract whilst limiting systemic drug absorption and adverse effects. Their development is challenging, partly because of limited knowledge of the physiological and pathological distal gastrointestinal factors, including colonic chyme fluid distribution and motor function. Recently, non-invasive techniques such as magnetic resonance imaging (MRI) have started to provide novel important insights. In this feasibility study, we formulated a coated capsule consisting of a h

Site-specific delivery of anti-inflammatory drugs in the gastrointestinal tract: an in-vitro release model.

Mesalazine and budesonide are anti-inflammatory drugs that are used to induce and maintain remission of inflammatory bowel diseases (IBD), such as Crohn's disease and ulcerative colitis. Both drug substances are intended to act locally at the inflamed sites of the gastrointestinal tract. The therapeutic objective for per oral treatment with these drugs is to achieve a high concentration of the active drug at the sites of inflammation while minimizing systemic absorption. The aim of this study was to develop a test system able to reflect the changing environment that a dosage form incorporating

Drug interactions with alcohol.

Ethanol and drugs can affect each other's absorption, distribution, metabolism, and excretion. When ingested together, ethanol can increase drug absorption by enhancing the gastric solubility of drugs and by increasing gastrointestinal blood flow. However, high concentrations of ethanol induce gastric irritation causing a pyloric spasm which in turn may delay drug absorption and/or reduce bioavailability. The 'quality' of the alcoholic beverage, independent of its ethanol content, can contribute to altered absorption of a drug. Ethanol is not bound to plasma proteins extensively enough to modi

Avenanthramides as digestion-stage modulators: Mechanistic links across the gastrointestinal continuum.

Oat avenanthramides (AVNs) are phenolic alkaloids unique to oats and are widely recognised for their antioxidant and anti-inflammatory properties. However, human studies consistently report very low oral bioavailability (<2%), highlighting a disconnect between dietary intake and systemic exposure. This review presents a digestion-stage-resolved conceptual framework integrating current evidence on AVN fate during gastrointestinal digestion, including matrix entrapment, digestive release, limited absorption, microbial biotransformation, and interindividual metabotype variability. AVN bioaccessib

Magnesium: The recent research and developments.

Magnesium is the fourth most abundant mineral in the human body, which facilitates more than 300 enzymatic reactions. Magnesium is essential for nucleic material and protein synthesis, neuromuscular conduction, cardiac contractility, energy metabolism, and immune system function. Gastrointestinal system and kidneys closely regulate magnesium absorption and elimination to maintain adequate storage of magnesium. Magnesium deficiency has been linked to many diseases and poor health outcomes. Magnesium has also been proven to be an effective therapeutic agent in many diseases, such as bronchial as

Proton pump inhibitor-induced hypomagnesemia: A new challenge.

Proton pump inhibitors (PPIs) are commonly used in clinical practice for the prevention and treatment of peptic ulcer, gastritis, esophagitis and gastroesophageal reflux. Hypomagnesemia has recently been recognized as a side effect of PPIs. Low magnesium levels may cause symptoms from several systems, some of which being potentially serious, such as tetany, seizures and arrhythmias. It seems that PPIs affect the gastrointestinal absorption of magnesium. Clinicians should be vigilant in order to timely consider and prevent or reverse hypomagnesemia in patients who take PPIs, especially if they …
