The abundance of a preselection of transcripts involved in inflammation, immunosenescence and stress response was compared between PBMC of healthy aged donors and aged patients in acute phase of heart failure and at recovery. This study identified 22 transcripts differentially abundant in acute phase of heart failure versus healthy aged subjects. Transcripts involved in inflammation and oxidative stress were more abundant. Those associated with T-cell functions were less abundant. The results were compared to two other major acute geriatric issues: infectious diseases and hip fracture. In acut

BACKGROUND: In time-to-event analyses of geriatric outcomes, competing events such as mortality are common but are often handled inappropriately, leading to biased risk estimates and potentially incorrect inferences. We present a case study illustrating the consequences of failing to account for competing events and review how such analyses are conducted in recent JAGS publications.
METHODS: For the case study, we analyzed data from the Precipitating Events Project to estimate the risk and rate of two geriatric outcomes-injurious falls and restricted activity-using methods that appropriately

A testament to longevity, old age is a time of great vulnerability. An ageing and ailing body often weakens the immune system and leaves a person susceptible to illness. Ageing therefore presents a challenge for the medical profession, which addresses it through a specialised branch of medicine: geriatrics. In this field of medicine for older people, which focuses on health and quality of life, the medical profession has both positive and negative legal obligations aimed at protecting dignity.

BACKGROUND: Multifocal atherosclerosis (MFA) is one of the common diseases in elderly patients. Currently, there is no single approach to treating patients with MFA. The active implementation of endovascular surgery has significantly expanded the treatment options for comorbid elderly patients with MFA.
OBJECTIVE: Of the study: to evaluate the effectiveness and safety of various methods of surgical revascularization in geriatric patients with multifocal atherosclerosis.
MATERIALS AND METHODS: This single-center observational cohort study was conducted in the Department of Cardiovascular Surg

Rapid population ageing is widening the gap between geriatric rehabilitation needs and constrained care capacity. Digital health offers new ways to deliver rehabilitation beyond traditional settings, but its role and limitations in geriatric practice remain unclear. To provide a narrative synthesis of how digital health can support geriatric rehabilitation, highlighting advances, challenges, and future directions. We conducted a narrative review of digital health approaches in geriatric rehabilitation, informed by the authors' clinical and technical expertise, synthesizing evidence on telemedi

BACKGROUND: Emergency departments (ED) across the country are seeing an increasingly larger portion of visits from patients aged 65 and older due to the overall aging of the United States population. The ED is recognized as an important institution for screening and addressing social risks. Therefore, understanding the social risks of the geriatric ED population is of crucial public health importance. METHODS: This was a subgroup analysis of a prospective cohort study conducted between July 2022 and April 2023 in an ED at a single academic urban quaternary medical center in Los Angeles, Califo

Objective The purpose of this investigation was to identify the mortality rate at three months and one year for patients 65 years old or older with a nonoperatively managed pelvic ring fracture. Materials and methods A retrospective comparative cohort study was performed at a single level 1 trauma tertiary referral center. From 2014 to 2019, we reviewed all patients older than 65 years who underwent nonoperative treatment for a pelvic ring fracture (Current Procedural Terminology codes 27193 or 27197). Demographics, including age, sex, body mass index (BMI), and Charlson Comorbidity Index (CCI

IMPORTANCE: Geriatric syndromes are common in hospitalized older adults and complicate acute care; however, their overall prevalence and cumulative burden remain poorly understood, especially in resource-limited settings.
OBJECTIVES: To measure the prevalence of geriatric syndromes upon hospital admission and examine the independent association between the number of geriatric syndromes and 90-day mortality.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from the Creating a Hospital Assessment Network in Geriatrics (CHANGE) study, a multicenter, prospective cohort of 43 hospit…

Frailty is increasingly recognized as a major determinant of perioperative risk in the geriatric population. It is characterized by reduced physiological reserve and impaired stress tolerance and arises from multisystem dysregulation involving chronic inflammation, neuroendocrine changes, sarcopenia, and organ dysfunction. Unlike chronological aging, frailty reflects biological vulnerability independent of comorbidity or disability and is strongly associated with postoperative complications, prolonged hospitalization, cognitive decline, and mortality. Therefore, early and systematic frailty as

Shapiro syndrome is a rare condition with fewer than 100 cases reported, initially characterized by the triad of hypothermia, hyperhidrosis, and agenesis of the corpus callosum. The diagnostic triad is no longer systematic, and the condition is now more broadly referred to as spontaneous periodic hypothermia. An 86-year-old patient presented with coma, hypothermia, and sweating. The differential diagnoses of hypothermia were ruled out and a diagnosis of a variant of Shapiro syndrome was made. Brain imaging revealed small corpus callosums. Treatment was symptomatic, with external warming and ma

Two mechanisms underlie the development of fecal incontinence (FI), which poses a significant autonomic burden among individuals with neurological disease: neurogenic FI, in which neural lesions innervate the bowel/sphincter, and functional FI, without such lesions but occurring secondarily to cognitive decline and/or immobility. Both neurogenic FI and functional FI hinder a person's toiletings. Neurogenic FI is further divided into two mechanisms: overflow FI due to neurogenic constipation, which is common in older individuals, and sphincter FI caused by neurogenic sphincter weakness due to a

BACKGROUND: Anemia is among the most prevalent hematological disorders in the elderly population and is still a significant public health concern across the world. In the older population, this disorder usually has a multi-factorial origin, related to nutritional deficiency, chronic inflammation, renal insufficiency, or occult blood loss. Even mild anemia in the elderly has been associated with reduced physical ability, cognitive deficit, increased rates of admission to hospital, and increased mortality. Even though it is clinically important, anemia in the elderly is frequently underdiagnosed

The aging population in the United States imposes clinical, financial, and ethical challenges for healthcare facilities. As cancer risk rises with age, an urgent need exists to create organized and comprehensive geriatric oncologic care. Barriers to equitable care include physiologic decline, cultural biases, communication challenges, social determinants, and financial toxicity. This paper examines these five domains, highlighting their clinical implications and the ways they restrict access to standard oncologic care. Defining an organizational framework that emphasizes cultural competence, c

BACKGROUND: Geriatric patients manifest pain and physical findings differently and are associated with higher mortality and complications. We hypothesized that physical exam (PE) is unreliable versus computed tomography (CT) for comprehensive injury identification in geriatric patients. Additionally, we quantified significant incidental radiologic findings.
STUDY DESIGN: Our institution adopted a policy of Pan Scan (PS) CT for trauma activations of patients ≥65 years. PS included CT of head/neck and chest/abdomen/pelvis. PE and imaging findings were extracted from physician reports.
RESULTS:

INTRODUCTION: Comprehensive geriatric assessment (CGA) prior to transcatheter aortic valve replacement implantation (TAVI) for severe aortic stenosis (AS) may improve the selection of patients targeted for this strategy. Despite the fact that CGA includes multiple scales, comparative data among them are limited. Our aim was to evaluate the discriminative capacity of different scales used in CGA on the decision of suitability for TAVI implantation and the influence on prognosis after TAVI implantation.
METHODS: Single-center prospective observational study including consecutive patients with s
