Twenty-one patients with germ cell tumors (17 germinomas and 4 teratomas) involving the hypothalamic-neurohypophysial (HN) region were reviewed retrospectively. Eleven patients were males and 10 females, and their ages ranged from 7 to 45 years (average 18.5 years). Diabetes insipidus was the initial and the most prominent symptom in most germinomas; in teratomas the most prominent symptom was visual disturbance. Eighty patients with stage IV testicular germ cell tumours with lung metastases were treated with PVB chemotherapy and subsequent surgery in cases of residual disease. Out of 80 patients 28 (35%) achieved complete response following chemotherapy alone. Thirty-six patients (45%) with partial response underwent surgery: 17 had lymphadenectomy because of residual mass in the retroperitoneum, 15 had pulmonary surgery alone and 4 had both operations. Forty-nine patients with assumed extragonadal germ-cell tumors (retroperitoneum: 39, mediastinum: 8, CNS: 2) were included in the present study. The patients were treated with 'high' (40 mg cisplatin/m2 and 200 mg etoposide/m2 daily x 5) or 'conventional' (20 mg cisplatin/m2 and 100 mg etoposide/m2 daily x 5) doses of cisplatin and etoposide together with bleomycin, depending on the presence or absence of poor prognosis factors. During 1-20 days of pregnancy rats were given 20% alcohol orally in a dose 0.4 mg/kg bw. This led to a significant increase in the nuclear volume of germ cells in male 19-day embryos. A karyological analysis of 2-month offspring germ cell generations at stage VII of seminiferous epithelium cycle revealed a reliable decrease in the number of spermatids on step 7 of development after introduction of antenatal alcohol. The increased proportion of spermatocytes with meiotic chromosomes pathological behaviour due to alcohol was determined by the proposed quantitative analysis of meiotic divisions.

This report presents 3 cases with gonadoblastoma mixed with other germ cell tumours in intersex patients, all with a 46,XY karyotype. One 11-year-old patient was a true hermaphrodite, the others, aged 13 and 18, respectively, had both gonadal dysgenesis. Different clinical courses have been noted. Two patients had gonadoblastoma combined with dysgerminoma; in one of them a teratoma was also found. In the third case gonadoblastoma was transformed into teratocarcinoma. The malignant degeneration was more serious in the 2 pubertal patients. Thus, we recommend early and prophylactic gonadectomy.

Fifteen patients with biopsy-proven mediastinal germ cell tumors treated with platinum-based chemotherapy were reviewed. They had a period of 4 to 6 weeks between the onset of symptoms and diagnosis. Four patients had sarcomatous elements in their tumor in association with common germ cell histologies. A 17 year old male with mediastinal yolk sac tumor is herein reported. His chief complaint was anterior chest pain. Chest X-ray and chest CT indicated a 8 cm diameter mass in the anterior mediastinum. Laboratory analysis revealed 2,578 ng/ml for AFP and 54% for LDH I. HCG and CEA values were normal. We present here a rare autopsy case of malignant germ cell tumor with leukemia-like infiltration of the anterior mediastinum in a 35-year-old male. Chest X-rays revealed an abnormal mediastinum, which was diagnosed as thymoma. During the course of treatment, huge abnormal cells 40-50 mu in size were found in the peripheral blood smear and disseminated bone metastasis of the malignant thymoma was suspected. A 46 XY woman with a dysgerminoma and gonadoblastoma is described. Both dysgerminoma-gonadoblastoma and mixed germ cell-sex cord stromal tissue was present upon microscopic examination. This case exemplifies the transition that can exist between these two distinctly separate tumors.

Radioimmunodetection with an 131I anti-alpha-fetoprotein (alpha FP) has been performed in 5 patients with germ cell tumors.
