A generalized cutaneous reaction induced by granulocyte colony-stimulating factor.

BACKGROUND: The increasing use of recombinant forms of granulocyte and granulocyte-macrophage colony-stimulating factors (GCSF and GMCSF) for neutropenic conditions has resulted in reports of a variety of cutaneous reactions. OBJECTIVE: We studied the clinical and histologic findings in three patients who underwent bone marrow transplantation and subsequently had a cutaneous eruption associated with the use of GCSF. METHODS: Biopsy specimens taken at the height of the eruption were studied histologically and immunohistochemically. RESULTS: The patients had indurated, well-demarcated, occasi

A prolonged use of granulocyte colony stimulating factor in Felty's syndrome.

Current treatment for Felty's syndrome, a triad of rheumatoid arthritis, splenomegaly and neutropenia, is often unsuccessful. Felty's syndrome may be related to decreased production of hematopoietic growth factors. We treated a patient with Felty's syndrome, profound neutropenia and a history of multiple complicated hospitalization for severe infections, with granulocyte colony stimulating factor (GCSF) for 18 months. After initiation of GCSF, the patient's neutrophil count has remained in the normal range for 18 months. After 2 easily treated infections at the start of therapy, she had only o

Granulocyte-colony stimulating factor therapy in drug-induced agranulocytosis.

Drug-induced agranulocytosis in the outpatient setting is a rare but potentially fatal adverse effect of many classes of medications. Five patients with this disorder presented to Yale-New Haven (Conn) Hospital during 1990 through 1992. Three patients treated with granulocyte-colony stimulating factor and two patients treated with routine care were studied for relevant clinical outcomes. Treatment with granulocyte-colony stimulating factor was associated with a shorter duration of neutropenia and a decreased length of hospital stay, consistent with recent case reports. Despite the high cost of

A recessive defect in lymphocyte or granulocyte function caused by an integrated transgene.

A line of transgenic mice has been identified with a recessive defect in lymphocyte or granulocyte function, presumably as a result of insertional mutagenesis by the integrated transgene. Transgenic mice homozygous for the transgene integrant showed nearly complete absence of lymphocytes in peripheral lymph nodes and Peyer's patches, a severely diminished thymus medulla, and a greatly enlarged spleen. These animals also developed a syndrome characterized by granulocyte and mononuclear infiltrates in numerous tissues, including skin, liver, and lung, and immunoglobulin deposits in kidney glomer

Case report: role of granulocyte colony stimulating factor in radiotherapy.

Two patients who received granulocyte colony stimulating factor (G-CSF) before or during radiotherapy are reviewed. Both patients developed granulocytopenia after multi-agent chemotherapy. One patient had breast cancer with multiple distant metastasis and received palliative radiotherapy to the spine and hip joint. The other patient had locally advanced anal carcinoma and received concurrent 5-fluorouracil, mitomycin, and radiation therapy. Both patients completed the planned course of radiotherapy without interruption, after administration of G-CSF.

Cytoenzymological-cytogenetical correlations concerning chronic granulocytic leukemia leucocytes.

By means of histochemical demonstration of dihydrofolate reductase activity and chromosomal analysis, peripheral white blood cells from patients with chronic granulocytic leukemia were studied in vitro cultures grown on normal leucocytes feeder-layers. After 14 days of incubation, two different types of colonies were found corresponding to the leukemic and normal leucocytic populations: (a) made up of Ph'-positive leucocytes which have a high dihydrofolate reductase activity; (b) made up of Ph'-negative leucocytes, slightly positive for dihydrofolate reductase activity.

A retrospective study of radiolabeled granulocyte kinetics in patients with systemic vasculitis.

Patients with systemic vasculitis, including Wegener's granulomatosis (WG) and microscopic polyarteritis (MP), may undergo white cell scanning for the investigation of infective complications and/or occult fever. In a retrospective study of 12 patients with systemic vasculitis (six each of WG and MP), all with renal disease, we observed increase diffuse lung radioactivity soon after the injection of 111In-labeled granulocytes or 99mTc-HMPAO-labeled leukocytes in all patients with WG and in three with MP. Lung activity was quantified by comparison with the liver or spleen. The lung:liver count …

Structural analysis of the receptors for granulocyte colony-stimulating factor on neutrophils.

We investigated granulocyte colony-stimulating factor (G-CSF) receptors on neutrophils from three patients with chronic myelogenous leukemia (CML) in the chronic phase, in comparison with four normal volunteers. Because we experienced some difficulties in radioiodinating intact recombinant human G-CSF, we developed a new derivative of human G-CSF termed YPY-G-CSF. It was easy to iodinate this protein using the lactoperoxidase method because of two additional tyrosine residues, and its radioactivity was higher than that previously reported. The biological activity of YPY-G-CSF as G-CSF was full

Granulocytic sarcoma of the prostate. Two cases causing urinary obstruction.

Two cases of patients with prostatic granulocytic sarcoma in whom urinary obstruction occurred are presented. The diagnosis was made by tissue examination with hematoxylin and eosin and specific esterase stains. One patient had a myelodysplastic syndrome and the other patient had acute myeloblastic leukemia. In both cases the diagnosis of prostatic granulocytic sarcoma was unexpected. Granulocytic sarcomas should be considered in the differential diagnosis of urinary obstruction in patients with myeloproliferative or myelodysplastic syndromes.

Granulocytic sarcoma (chloroma) of the oral cavity: a case with aleukemic presentation.

A case of granulocytic sarcoma, or chloroma, of the palatal mucosa, which developed 15 months before the onset of acute myelogenous leukemia (AML), is reported. The diagnosis was suspected on the basis of the light microscopic findings and confirmed by histochemical studies. Granulocytic sarcomas are rare, may be observed in a variety of body locations, and are considered specific lesions of AML or of the onset of blast crisis in chronic myelogenous leukemia. Primary granulocytic sarcomas of the oral cavity without systemic manifestations of AML are extremely rare. Clinical diagnosis of these

Bone marrow granulocyte reserve in chronic benign idiopathic neutropenia.

Fifteen patients with chronic benign idiopathic neutropenia (CBIN) with neutrophil counts less than 1.0 X 10(9)/1 have been studied. The mean age was 33 years (range 23-50) comprising 11 females and 4 males. Bone marrow cellularity was normal except in two patients who showed slight reduction and one who had a slight increase. Bone marrow differential counts were normal apart from a small increase in the percentage of promyelocytes and reduction in the myeloid/erythroid ratio in some patients. Peripheral blood counts showed no 'compensatory' monocytosis. Epinephrine stimulation tests showed no

Myelofibrosis in chronic granulocytic leukaemia: clinicopathologic correlations and prognostic significance.

We performed 221 marrow trephine biopsies in 139 patients with Ph1-positive (Ph1+) chronic granulocytic leukaemia (CGL) in order to assess the incidence, degree and prognostic significance of marrow fibrosis (MF) at various stages of the disease. We also attempted to elucidate the relationship between development of MF and the various clinical and haematological features of CGL. A significant correlation was found between the amount of fibrosis (graded from 0 to 3) and the stage of CGL, indicating that major fibrotic changes are associated with accelerated or blastic disease. Survival studies

Chronic granulocytic leukemia following radiation therapy for Hodgkin's disease.

A 33-year-old white man was treated with irradiation for Hodgkin's disease involving the mediastinum; four years later he developed typical Philadelphia chromosome-positive chronic granulocytic leukemia. The patient has been treated with chronic low-dose busulfan to maintain peripheral blood counts in the normal range and he continues to remain well 27 years after the initial diagnosis of Hodgkin's disease and 23 years following the onset of leukemia.

Early granulocyte transfusions in high risk febrile neutropenic patients.

A total of 39 febrile neutropenic patients at high risk of gram-negative rod bacteremia and poor prognosis relative to likely response to infection were randomized prospectively to receive granulocyte transfusion within 24 hours of beginning broad spectrum combination antibiotic therapy. Of 16 infected patients receiving granulocytes 11 (69%) improved and of 23 not receiving granulocytes 18 (78%) improved (NSD). Similar results were found for bacteremia (3 of 4 and 6 of 7 improved, respectively). These results do not support the routine early use of granulocyte transfusions in this defined gro…

Granulocyte transfusions and outcome of alloimmunized patients with gram-negative sepsis.

The effect of white cell alloimmunization on patient outcome during gram-negative sepsis treated with granulocyte transfusions was studied. Twenty-five episodes of sepsis were observed; 19 were associated with resolution of sepsis and six with continuing sepsis and death. Compatibility testing included the granulocyte indirect immunofluorescence test and the lymphocytotoxicity assay. The number of compatible and incompatible granulocyte transfusions determined by the indirect immunofluorescence test compared with patient outcome was significant (X2 = 44, p less than 0.001). The same comparison

Preleukemic granulocytic sarcoma of cervix and vagina: initial manifestation by cytology.

Granulocytic sarcoma is an unusual form of tumefaction caused by acute granulocytic leukemia. On rare occasions, the lesion precedes the leukemic phase and presents as a mass with a normal peripheral white cell count. This report describes the initial manifestation of granulocytic sarcoma by vaginal cytology in a 39-year-old female with Down's syndrome. Six days after admission, the patient died of acute peritonitis following spontaneous perforation of the bowel. Autopsy revealed involvement of cervix, vagina, bowel wall and one pelvic lymph node by granulocytic sarcoma. Bone marrow examinatio
