Growth monitoring in children.

EDITORIAL NOTE: This review is superseded by the published Cochrane Review, Taylor 2023: https://www.cochranelibrary.com/cdsr/doi/10.1002/14651858.CD014785.pub2/full. BACKGROUND: Growth monitoring is widely accepted and strongly supported by health professionals, and is a standard component of community paediatric services throughout the world. We sought to evaluate research evidence of its impact. This requires definition, consideration of the setting, and discussion of the intended effects of this activity. In this review, we define growth monitoring as the regular recording of a child's we

Vitamin A deficiency and nocturnal growth hormone secretion in short children.

In the growth hormone (GH) neurosecretory dysfunction syndrome affecting slowly growing children with delayed bone age, low nocturnal GH secretion is accompanied by normal responses to pharmacological stimuli. We compared plasma vitamin A with physiological nocturnal and stimulated GH secretion in 68 short prepubertal children. Fasting plasma vitamin A correlated with nocturnal GH secretion but not with stimulated GH secretion. Total dietary vitamin A intake was significantly lower in short children with abnormal nocturnal GH secretion than in normal children and in endocrinologically-normal s

X-linked agammaglobulinemia, growth hormone deficiency and delay of growth and puberty.

Coinheritance of X-linked agammaglobulinemia and growth hormone deficiency (XLA/GHD) has been classified as an independent primary immune deficiency. We evaluated the pattern of growth and endocrine function in seven XLA subjects (ages 10.9-20.1 years); four belonged to two different XLA pedigrees and three represented sporadic XLA cases. Three had reached adulthood (final stature 176.0, 173.5 and 165.0 cm, respectively) and their retrospective growth showed delay in growth and puberty during adolescence. In the other four subjects, growth hormone production was measured by growth hormone phar

Role of ghrelin in promoting catch-up growth and maintaining metabolic homeostasis in small-for-gestational-age infants.

Small-for-gestational age (SGA) has been a great concern in the perinatal period as it leads to adverse perinatal outcomes and increased neonatal morbidity and mortality, has an impact on long-term health outcomes, and increases the risk of metabolic disorders, cardiovascular, and endocrine diseases in adulthood. As an endogenous ligand of the growth hormone secretagotor (GHS-R), ghrelin may play an important role in regulating growth and energy metabolic homeostasis from fetal to adult life. We reviewed the role of ghrelin in catch-up growth and energy metabolism of SGA in recent years. In ad

Weight-for-length trajectories in the first year of life in children of mothers with eating disorders in a large Norwegian Cohort.

OBJECTIVE: To describe weight-for-length (WFL) trajectories in the children (birth-12 months) of mothers with and without eating disorders. METHOD: This study is based on the Norwegian Mother and Child Cohort Study (MoBa) conducted by the Norwegian Institute of Public Health. We categorized women (N = 57,185) based on diagnosis prior to and during pregnancy: anorexia nervosa, bulimia nervosa, eating disorder not otherwise specified-purging subtype, binge eating disorder, or no eating disorder. The primary analysis included a shape invariant model fitted with nonlinear mixed effects to compare

Insights and Implications of Genome-Wide Association Studies of Height.

CONTEXT: In the last decade, genome-wide association studies (GWASs) have catalyzed our understanding of the genetics of height and have ide … ay. The genes implicated by GWASs are often the same genes that are the genetic causes of Mendelian growth disorders or skeletal dysplasias, and GWAS results can provide complementary information about these disorders. CONCLUSIONS: Here, we review the rationale behind GWASs and what we have learned from GWASs for height, including how it has enhanced our understa …

Relationship Between the Serum FGF21 Level and Growth in Children of Short Stature.

BACKGROUND: This study investigated the relationship between fibroblast growth factor 21 (FGF21) levels and growth in children with growth hormone deficiency (GHD) and idiopathic short stature (ISS), and the effects of the FGF21 level on response to growth hormone (GH) treatment. METHODS: We included 171 pre-pubertal children with a GHD (n = 54), ISS (n = 46), and normal height (n = 71). Fasting FGF21 levels were measured at baseline and every 6 months during GH treatment. Factors associated with growth velocity (GV) after GH therapy were investigated. RESULTS: The FGF21 level was higher in

[The diagnostic evaluation of tall stature in children].

In this case series, we describe four children and adolescents with tall stature or growth acceleration to illustrate the diagnostic evaluation of tall stature according to the new Paediatric Association of the Netherlands (NVK) Guideline on growth disorders. A 14-year-old girl with tall stature and a relatively late onset of puberty was diagnosed with idiopathic familial tall stature, and the patient decided not to opt for epiphysiodesis. A 14-year-old boy with prepubertal growth acceleration and a history of behavioural problems was diagnosed with Klinefelter syndrome. A 7-year-old boy with …

Cerebral dysfunction following infantile dietary chloride deficiency.

Twenty-two children who were chloride-depleted in infancy due to a chloride-deficient diet and who had resultant hypochloremic alkalosis were analyzed in regard to their signs and symptoms, metabolic studies, and growth parameters. Deceleration of weight, linear growth, and head growth occurred in most, and persistent growth failure occurred in some. The majority had cognitive deficits at follow-up. Comparison with growth parameters in a chronically malnourished group of children who had a variety of disorders revealed a similar degree of deceleration of weight (p = 0.50) and height (p = 0.70)

Healthy short stature.

Idiopathicshort stature (ISS) has been used for more than five decades to label children whose height is below -2 SDS without an identified underlying cause. Although originally conceived as a pragmatic diagnosis of exclusion, ISS has gradually been reified as a disease entity and is now embedded in clinical guidelines, regulatory frameworks and indications for recombinant human growth hormone therapy. In parallel, advances in genomic technologies have uncovered a growing spectrum of monogenic and chromosomal variants among children previously classified as ISS, and highlighted the continuous,

Amelogenesis imperfecta with growth hormone deficiency in a 12 year-old boy.

Amelogenesis imperfecta (AI) is a diverse group of hereditary disorders that are characterized by a defect in the formation of the tooth enamel and a high degree of clinical diversity. X-linked, autosomal dominant and recessive inheritance have been demonstrated. Growth hormone (GH) has an effect on bone and soft tissue development. Dental and facial abnormalities associated with pituitary dwarfism have been reported, but GH deficiency with AI is very rare. We describe a 12 year-old pre-pubertal boy who was referred to our hospital with teeth deformities and growth retardation. His teeth had b

Is there a place for urinary growth hormone measurement?

The evaluation of growth hormone (GH) secretion is an important problem in pediatric endocrine practice. The diagnosis of GH insufficiency is based on the finding of a "blunted" GH response to GH provocative tests or on the demonstration of a decreased endogenous secretion. From a practical point of view, these methods are uncomfortable, expensive and time consuming. Recently, very sensitive specific assays to measure human GH in urine have been developed. We present a discussion of available data on these tests in order to estimate their role in the evaluation of a short or slowly growing chi

[Growth hormone therapy in childhood. Certain indications].

The diagnosis of growth hormone deficiency is established on the basis of auxological and endocrinological studies. Without treatment, the final stature will be reduced; by way of treatment, administration of biosynthetic human growth hormone is required. Total and partial growth hormone deficiency can be distinguished, as also neurosecretory dysfunction of growth hormone secretion. Growth hormone can also be successfully administered to girls with the Ullrich-Turner syndrome, but the doses required are somewhat higher than in the case of growth hormone deficiency. Other forms of reduced statu

The social outcome of adults with constitutional growth delay.

It is well established that children with extreme short stature, secondary to GH deficiency (GHD) or achondroplasia, face a disability that may affect their adult psychosocial outcome, while contrasting data are reported for short normal children. In our hands, adults with GHD have shown an unsatisfactory psychosocial adjustment due to the low adult height and hypothetically to the persistent GHD. This study has been carried out to evaluate the influence of growth in height on the social outcome in 45 adults with constitutional growth delay (CGD). Interviews with subjects and parents were used

[Treatment with growth hormone: results and new perspectives].

The use of human growth hormone is being extensively reevaluated in children with hypopituitarism, as we enter a new phase with sufficient supply of recombinant hGH. Simultaneously much emphasis has been placed on the methods of measurement of GH secretion in order to define a more appropriate cutoff level defining partial GH deficiency. Among non conventional indications of hGH, Turner syndrome has turned out to be most important with very encouraging short term results. The extension of hGH therapy to other clinical groups, as non endocrine short stature is still under careful investigation

Growth hormone for the treatment of growth failure in children.

The etiology, diagnosis, and clinical features of growth failure in children are presented, with discussion of exogenous growth hormone (GH) replacement, its indications, efficacy, and adverse effects. Causes of growth delay include malnutrition, systemic illness, emotional deprivation, deficiency of endogenous growth hormone, thyroid hormone deficiency, and cortisol excess. Growth hormone (somatotropin) is secreted by the anterior pituitary gland in response to various stimuli, including exercise, hypoglycemia, and arginine. This hormone stimulates growth of skeletal muscle and connective tis

Reversible growth hormone deficiency and delayed puberty triggered by a stressful experience in a young adult.

Growth retardation and delayed puberty developed in a young man after emotional trauma at age 12 years, three months. He had an eating disorder, was profoundly malnourished for several months, and then resumed eating spontaneously. Deficient secretion of growth hormone was documented during multiple arginine-insulin tolerance tests, and gonadotropin levels were low. These hormonal deficits persisted for many years in spite of good nutrition and lack of abuse, a course inconsistent with psychosocial dwarfism or effects of malnutrition per se. Later the patient spontaneously achieved normal heig

Increasing growth with raised circulating somatomedin but normal immunoassayable growth hormone.

Two patients are described with elevation of circulating somatomedin A concentration but normal growth hormone levels. One,a young male, appeared clinically acromegalic; the other, a young female, was tall with kyphoscoliosis, and her appearance resembled Marfan's syndrome. In both patients plasma growth hormone concentration was suppressible by hyperglycaemia. It is suggested that their clinical syndromes resulted from excessive somatomedin activity.
