Lyme disease is a vector-borne illness that can affect numerous organ systems during the early disseminated phase, including the heart. The clinical course of Lyme carditis is usually benign with most patients recovering completely. In rare instances, death from Lyme carditis has been reported. The cardinal manifestation of Lyme carditis is conduction system disease, which generally is self-limited. Heart block occurs usually at the level of the atrioventricular node but often is unresponsive to atropine sulfate. Temporary pacing may be necessary in more than 30% of patients, but permanent hea

The ability of antiarrhythmic drugs to aggravate and even cause arrhythmias is now well-established. The incidence of such effects for a wide range of agents is of the order of 3-15%. The two major mechanisms are facilitation of reentry circuits and the production of triggered activity due to early after depolarisations. This latter effect probably underlies the drug-induced long QT syndromes and associated arrhythmias including 'torsade de pointes' and is more likely to occur in association with bradycardia, hypokalaemia, or concomitant digitalis therapy. While some or all of these mechanisms

A case of a baby with supraventricular tachycardia who died is reported. Histological examination of the cardiac conduction system revealed a hypoplastic sinus node and an unusual connexion between the branching bundle and the atrioventricular node. There was no electrocardiographic evidence to suggest that the anomalous pathway functioned; the unusual configuration was probably within the normal range of morphological variations.

A dog weighing 15 kg and anesthetized with intravenous sodium thiopental was subjected to transcatheter ablation of the AV junction with high-frequency currents, in order to induce a partial alteration in AV conduction (first degree AV block). Two conventional bipolar electrode-catheters were introduced through the right femoral vein and used one for atrial pacing and the other for His bundle recording and current delivering. The catheter used for ablation was situated in the AV junction where the distal monopolar recording of the His bundle electrogram showed an A/V ratio close to one in the

Many antiarrhythmic drugs are available for treatment of supraventricular tachycardia. Selection of the appropriate drug is helped by the identification of the site of origin, mechanism and pathway of the arrhythmia. For most types of supraventricular tachycardia greater than 1 antiarrhythmic drug is available. Comparative studies between the effect of different antiarrhythmic drugs on the same arrhythmia are few, preventing scientifically proved advice on "the best" drug for a given arrhythmia. Familiarity with a limited number of antiarrhythmic drugs will increase therapeutic efficacy and re

Two adolescents with no obvious cardiac were investigated for paroxysmal tachycardia at a rate of about 200/mn with wide QRS complexes showing left bundle branch block, reduced by vagal manoeuvres. Atrial pacing showed shortening of the HV interval associated with widening of the QRS complex to appearances identical to those of the tachycardia, suggesting right ventricular preexcitation masked in sinus rhythm. The progressive prolongation of the atrio-ventricular conduction time with increasing prematurity of the extrastimulus and atrioventricular block after intravenous injection of ATP were

The use of a digital display automatic chronometer during epicardial mapping is reported. The epicardial potential recorded by a tripolar electrode is transformed electronically by amplification, filtering, rectifying negative deflections and then summation. The interval between the reference signal (electrical impulse or other cardiac potential) is calculated automatically and displayed at an adjustable rate. The presence of a delay increases the range of application of the apparatus. Epicardial mapping is thus simplified and shortened.

Congenitally corrected transposition of the great arteries is a rare heart defect that can be associated with systemic ventricular dysfunction and conduction disturbances. The use of cardiac resynchronization therapy in patients with congenital heart disease is not fully established, and achievement of successful pregnancies after implantation of transvenous, biventricular system has never been described, and which resulted in a significant clinical improvement. We describe a 33-year-old female with congenitally corrected transposition of the great arteries, who achieved six pregnancies and su

Specific tachycardia characteristics, such as the zones of initiation, transition, and termination, may yield important insights into the underlying mechanism of the tachycardia even before diagnostic maneuvers are performed.

Bradycardia due to cardiac conduction abnormalities is prevalent in patients with dementia with Lewy bodies possibly due to autonomic dysfunction and neurodegeneration. Cholinesterase inhibitors remain the most effective treatment for cognitive and neuropsychiatric symptoms in patients with dementia with Lewy bodies, but are often withheld due to concerns of potentially worsening cardiac risk. Randomised clinical trials and meta-analysis evidence support a favourable cardiac safety profile for cholinesterase inhibitors in dementia with Lewy bodies. Emerging evidence challenges the conventional

The introduction of His bundle electrography by Benjamin Scherlag (New York) in 1969, together with programmed stimulation of the heart by Philip Coumel (Paris) in 1967, and Hein Wellens (Amsterdam) in 1972, were decisive turning points on the way to invasive electrophysiology and the development of an independent, now distinctly interventional subspecialty of cardiology. The main topic of the 1970s was bradycardic arrhythmias, promoted by pacemaker therapy, which had been introduced just over 10 years earlier. The recording of the potentials of the bundle of His and other recording locations

The histopathological features of the atrioventricular (AV) conduction system of nine cats with high-grade atrioventricular block (H-AVB) were compared with those of 22 cases of third-degree atrioventricular block (3-AVB). All nine H-AVB cats had experienced syncopal attacks of variable severity and had been diagnosed electrocardiographically. The AVB, which was permanent in all cases, had been observed for 15-1,981 days (average 663 days) before death. Histological examination of the AV conduction system revealed moderate (25-50%) or severe (>50%) loss of conduction fibres associated with fib
