It is possible, in our midst, to obtain a survival rate of 68% and a marked improvement in the functional capacity of patients with advanced cardiac insufficiency due to systolic myocardial dysfunction through the combined use of ACE inhibitors, diuretics and digitalis as the main therapeutic agents. Patients with cardiac failure due to atherosclerotic heart disease have a slight better prognosis than those whose failure is secondary to dilated idiopathic congestive cardiomyopathy. The incidence of arrhythmias, sudden death and peripheral embolism suggest that the use of Amiodarone as antiarrh

The pathophysiology and the treatment of heart failure in patients with chronic cor pulmonale is described. The patients with chronic cor pulmonale were divided into two categories in terms of the cause of the disease, that is, due to chronic respiratory failure and due to chronic pulmonary vascular obstruction. The treatment for the patients in the first category is, mainly to control respiration and to continue chronic oxygen therapy, and in the second category is, to utilize vasodilator and anticoagulant therapy. The results of these treatments are rather poor, though in terms of improvemen…

We prospectively studied 69 consecutive patients hospitalized with a primary diagnosis of acute left ventricular failure so as to assess the impact of vasodilators on incidence and morbidity of acute symptomatic left ventricular failure. The determinants of duration of hospitalization, in-hospital mortality and symptomatic status 2 months after discharge were examined. There were 9 in-hospital deaths (13%), and survival at 60 days was 77%. Median duration of hospitalization was 9 days, and 33% of the surviving patients remained in New York Heart Association functional class III-IV 60 days subs

Diastolic heart failure is characterized by increased resistance to diastolic filling of one or both cardiac ventricles. Although some degree of diastolic failure exists in most patients presenting clinically with heart failure, a substantial subset of patients have relatively pure diastolic heart failure with normal systolic function. Diastolic heart failure can be due to structural abnormalities that increase resistance to ventricular inflow, and these structural abnormalities can be extramyocardial (e.g., constrictive pericarditis and mitral stenosis) or intramyocardial (e.g., fibrosis and …

Congestive heart failure is still one of the most common causes of death in our society. Treatment should be approached systematically with a set of well-defined objectives, which include rest, a low-sodium diet, inotropic agents, diuretics, and peripheral vasodilators. Patients receiving treatment for congestive heart failure should be examined daily for symptomatic improvement, cardiac signs, and accurate recording of total fluid intake and output. Serum electrolyte levels and chest X-ray films should also be checked intermittently. When using powerful diuretics or vasodilators, the physicia…

Acute heart failure involves various pathophysiological mechanisms among which primary reduction of myocardial contractility due to acute myocardial infarction, cardiomyopathy, and after open heart surgery are the most common. Therapy should be as causally related as possible. In patients with mechanical defects such as rupture of the interventricular reptum or acute mitral regurgitation due to papillary muscle rupture, surgical correction is mandatory. Systemic hemodynamics can often be temporarily stabilized by mechanical circulatory assist devices until spontaneous recovery has occurred or …

Heart transplantation (HT) remains the reference therapy for selected eligible patients with advanced heart failure (HF), recent developments in drug and device therapies notwithstanding. The majority of heart transplants are performed in Europe and the USA. However, the rate of heart transplants varies greatly among European countries, ranging from <1 to >10 per million population per annum. Due to donor organ shortage, durable mechanical circulatory support (MCS) became a practical solution for many advanced HF patients, either as a bridge to transplant or as destination therapy. The aim of …

INTRODUCTION AND OBJECTIVES: To evaluate the use of health care resources and their cost according to the effects of kidney failure in heart failure patients during 2-year follow-up in a population setting.
METHODS: Observational retrospective study based on a review of medical records. The study included patients ≥ 45 years treated for heart failure from 2008 to 2010. The patients were divided into 2 groups according to the presence/absence of KF. Main outcome variables were comorbidity, clinical status (functional class, etiology), metabolic syndrome, costs, and new cases of cardiovascular …

Organ injury and impairment are commonly observed in patients with acute heart failure (AHF), and congestion is an essential pathophysiological mechanism of impaired organ function. Congestion is the predominant clinical profile in most patients with AHF; a smaller proportion presents with peripheral hypoperfusion or cardiogenic shock. Hypoperfusion further deteriorates organ function. The injury and dysfunction of target organs (i.e. heart, lungs, kidneys, liver, intestine, brain) in the setting of AHF are associated with increased risk for mortality. Improvement in organ function after decon…

Background/Objectives: This is the first prospective, same-day, multi-modality comparison of lung ultrasound (LUS) and chest radiography (CXR) for detecting acute heart failure (AHF) in non-critical patients with dyspnoea, examining the impact of chronic heart failure and pleural effusion, using low-dose chest CT (LDCT) as an objective comparator, and cardiologists-adjudicated AHF as reference standard. Methods: An observational study of 240 consecutive non-critical patients ≥50 years admitted with dyspnoea was conducted. Unstable AHF cases were deemed ineligible. Each modality was evaluated a…

Several urinary markers reflect disease severity and have the potential to support the management of heart failure (HF). Collecting urine samples is easy and inexpensive, and urine sample composition can be altered not only by underlying kidney impairments (i.e. filtration barrier damage and tubular injury) but also via neurohormonal and inflammatory activation, ageing, comorbidities, other medical conditions and pharmacological interventions. For instance, urinary sodium may help to predict the response to loop diuretic therapy in acute HF, while albuminuria is used as a risk marker and thera…

Secondary mitral regurgitation (SMR) is highly prevalent in patients with heart failure (HF), associated with poor prognosis, and its treatment is typically preceded by implementing pharmacotherapy for HF as well as cardiac resynchronization therapy. Given the increase in experience and important technological iterations, transcatheter mitral valve repair has witnessed increasing safety and efficacy. Recently, two randomized controlled trials extended the level of evidence for this intervention in patients with SMR: RESHAPE-HF2 randomized patients with less severe SMR to either medical therapy…

BACKGROUND: Although the clinical features of chronic Chagas' cardiomyopathy (CCC) have been well established, clinical data about the patients are scarce.
OBJECTIVES: The current analysis reports the results of the I Brazilian Heart Failure Registry (BREATHE) assessing baseline characteristics and clinical outcomes of patients with acute heart failure due to CCC.
METHODS: BREATHE enrolled a total of 3,013 adult patients hospitalized with acute heart failure. We analyzed comparatively 261 (8.7%) patients with chronic CCC and 2,752 (91.3%) patients with other etiologies. Clinical and demograp…

BACKGROUND: Patients with heart failure (HF) overestimate survival compared with model-predicted estimates, but the reasons for this discrepancy are poorly understood. We characterized how patients with end-stage HF and their care partners understand prognosis and elicited their preferences around prognosis communication.
METHODS: We conducted in-depth, semistructured interviews with patients with end-stage HF and their care partners between 2021 and 2022 at a tertiary care center in Michigan. Participants were asked to describe barriers they faced to understanding prognosis. All interviews w…
