Sacubitril/valsartan and cardiac injury/stress and systemic inflammatory biomarkers in acute heart failure: a prespecified secondary analysis of the PREMIER study.

AIMS: Acute heart failure (AHF) is accompanied by myocardial injury, wall stress, and systemic inflammatory activation. Whether initiating sacubitril/valsartan (Sac/Val) after haemodynamic stabilization modifies cardiac injury/stress and systemic inflammatory biomarkers is uncertain. METHODS AND RESULTS: The Program Angiotensin-Neprilysin Inhibition in Admitted Patients with Worsening Heart Failure (PREMIER) study was a multicentre, prospective, randomised, open-label trial with blinded endpoint assessment. Within 7 days of hospitalisation and after haemodynamic stabilisation, inpatients prev

Liver-Kidney-Heart Axis in MASLD: Molecular and Cellular Pathways of Renal Injury Associated with Metabolic Hepatic Steatosis. A Narrative Review.

BACKGROUND: Steatotic liver disease associated with metabolic dysfunction (MASLD) has become the most common chronic liver disease worldwide. The impact of this condition is not limited to liver dysfunction; its relationship with chronic kidney disease (CKD) is significant, as it shares pathophysiological mechanisms and an increased risk of adverse cardiovascular outcomes. In this context, the coexistence of MASLD and CKD does not appear to be solely due to shared risk factors, but rather to convergent biological mechanisms that promote liver injury, as well as glomerular and tubular damage, i

Astragali Radix-Carthami Flos alleviate brain-heart injury after cerebral ischemia/reperfusion via regulating TSPO signaling.

Ischemic stroke is often accompanied by secondary cardiac dysfunction, yet its underlying pathogenesis remains under investigation. Astragali Radix-Carthami Flos (ARCF) is a drug pair with medicine food homology, commonly used for cerebrovascular and cardiovascular diseases. This study found that in the middle cerebral artery occlusion/reperfusion rat model, ARCF management mitigated CI/R-induced brain-heart injury, as evidenced by improved neurological function, reduced cerebral and cardiac damage, restored serum biomarker levels, and enhanced overall cardiac function. Transcriptomic analysis

Heart Rate Variability as a Predictor of Region-Specific Brain Injury in Neonates with Perinatal Asphyxia: A Prospective Study in a Middle-Income Country.

Background and Objectives: Neonates with moderate-to-severe hypoxic-ischemic encephalopathy (HIE) in low- and middle-income countries (LMICs) remain at high risk of neurological sequelae despite access to therapeutic hypothermia (TH). Real-time accessible biomarkers are required to improve risk stratification and guide neuroprotective care in these settings. This study evaluated the predictive capacity of heart rate variability (HRV) metrics for brain injury detected using magnetic resonance imaging (MRI) in neonates with HIE who underwent TH at an LMIC. Materials and Methods: We conducted a p

Astragaloside IV ameliorates cardiomyocyte injury and heart failure through hif/rho/rock pathway regulation: In vitro and in vivo insights.

Heart failure represents the culmination of various cardiovascular diseases, distinguished by a spectrum of complex symptoms. Astragaloside IV (AST-IV) has shown significant cardiac protection in heart failure rats, though the mechanisms are not fully understood. This study aimed to investigate the effects of AST-IV using hypoxia-reoxygenation injury in cardiomyocytes and heart failure in rats to explore the effects of AST-IV. Experimental groups were treated with AST-IV, HIF-2α siRNA, or Y-27,632 (a ROCK inhibitor). Cell proliferation was assessed using CCK-8 and EdU assays, while mitochondri

CREG1 promotes autophagy and protects the heart against nutritional stress-induced injury and age-associated hypertrophy, fibrosis and diastolic dysfunction.

BACKGROUND: Cellular repressor of E1A-stimulated genes 1 (CREG1) is an evolutionarily conserved endolysosomal glycoprotein that enhances lysosomal biogenesis and autophagy, suppresses proliferation, and promotes differentiation. A prior gene targeting strategy that produced truncated N-terminal fragments resulted in embryonic lethality, limiting the ability to assess the physiological role of complete CREG1 loss. We hypothesized that CREG1 regulates cardiac autophagy, thereby maintaining cardiac structure and function under both physiological and stress conditions. METHODS: We generated true

Preventive impact of probiotic supplements on heart injury and inflammatory indices in a rat model of myocardial infarction: histopathological and gene expression evaluation.

Although there is a bulk of evidence on the favorable effect of probiotics on the cardiac system, their role in the management of myocardial infarction is not clear. Three viable probiotic bacterial strains, namely Lactobacillus reuteri, Bifidobacterium longum, and Bifidobacterium lactis, were gavaged to the rats daily for 28 days prior to the induction of myocardial injury. Myocardial injury was induced by the use of isoproterenol (ISO) in the probiotics, control and sham groups. The heart tissues were catheterized to evaluate the histopathological parameters and measure the expression of gen

Endotoxin induced acute kidney injury modulates expression of AQP1, P53 and P21 in rat kidney, heart, lung and small intestine.

This study was designed to explore whether aquaporin 1(AQP1), P53 and P21 can be used as diagnostic biomarkers of lipopolysaccharide (LPS)-induced acute kidney injury (AKI) and potential indicators of sepsis-induced multiple organ injury. Bioinformatics results demonstrated that AQP1, P53, P21 was dramatically elevated 6h after Cecal ligation and puncture (CLP)-AKI in rat renal tissue. The expression of AQP1, P53, P21, NGAL and KIM-1 in kidney were increased significantly at first and then decreased gradually in LPS-induced AKI rats. Histopathological sections showed swelling of tubular epithe

Heart rate reduction after genetic ablation of L-type Cav1.3 channels induces cardioprotection against ischemia-reperfusion injury.

BACKGROUND: Acute myocardial infarction (AMI) is the major cause of cardiovascular mortality worldwide. Most ischemic episodes are triggered by an increase in heart rate, which induces an imbalance between myocardial oxygen delivery and consumption. Developing drugs that selectively reduce heart rate by inhibiting ion channels involved in heart rate control could provide more clinical benefits. The Cav1.3-mediated L-type Ca2+ current (ICav1.3) play important roles in the generation of heart rate. Therefore, they can constitute relevant targets for selective control of heart rate and cardioprot

Tolvaptan's Association with Low Risk of Acute Kidney Injury in Patients with Advanced Chronic Kidney Disease and Acute Decompensated Heart Failure.

INTRODUCTION: Furosemide, a loop diuretic, is often empirically used to treat acute decompensated heart failure (ADHF) initially. Conversely, decongestion using tolvaptan, an aquaretic, is thought to maintain renal function compared to furosemide. However, it has not been investigated in patients with advanced chronic kidney disease (CKD) at high risk of developing acute kidney injury (AKI). This study aimed to investigate AKI incidence using tolvaptan add-on treatment, compared to increased furosemide treatment for patients with ADHF complicated by advanced CKD. METHODS: We retrospectively s

Ferulic Acid, Pterostilbene, and Tyrosol Protect the Heart from ER-Stress-Induced Injury by Activating SIRT1-Dependent Deacetylation of eIF2α.

Disturbances in Endoplasmic Reticulum (ER) homeostasis induce ER stress, which has been involved in the development and progression of various heart diseases, including arrhythmias, cardiac hypertrophy, ischemic heart diseases, dilated cardiomyopathy, and heart failure. A mild-to-moderate ER stress is considered beneficial and adaptative for heart functioning by engaging the pro-survival unfolded protein response (UPR) to restore normal ER function. By contrast, a severe or prolonged ER stress is detrimental by promoting cardiomyocyte apoptosis through hyperactivation of the UPR pathways. Prev

Gallic, ellagic acids and their oral combined administration induce kidney, lung, and heart injury after acute exposure in Wistar rats.

Gallic (GA) and ellagic (EA) acids are present in foods, medicinal plants, teas, and dietary supplements. An acute toxicological study was conducted by oral administration of both compounds alone (200, 1000, and 2000 mg/kg) and combined (2000 mg/kg) and their effects on the electron transport chain (ETC) and the ROS production in kidney mitochondria further evaluated. All treatments induced a dose-dependent heart, lung, and kidney injury. However, the intensity of these effects varied according to the substance, with greater cardiac and renal toxicity for EA and pulmonary injury for GA, while

Hypoxic acclimation improves cardiac redox homeostasis and protects heart against ischemia-reperfusion injury through upregulation of O-GlcNAcylation.

Ischemia-reperfusion (I/R) injury is detrimental to cardiovascular system. Alteration in glucose metabolism has been recognized as an important adaptive response under hypoxic conditions. However, the biological benefits underlying this metabolic phenotype remain to be elucidated. This study was designed to investigate the impact of hypoxic acclimation (HA) on cardiac I/R injury and the antioxidative mechanism(s). Male adult mice were acclimated in a hypoxic chamber (10% oxygen [O2]) for 8 h/day for 14 days, and then subjected to cardiac I/R injury by ligation of left anterior descending coron

The Role of Reactive Oxygen Species, Kinases, Hydrogen Sulfide, and Nitric Oxide in the Regulation of Autophagy and Their Impact on Ischemia and Reperfusion Injury in the Heart.

There is considerable evidence that autophagy in cardiomyocytes is activated by hypoxia/ reoxygenation (H/R) or in hearts by ischemia/reperfusion (I/R). Depending upon the experimental model and duration of ischemia, increases in autophagy in this setting maybe beneficial (cardioprotective) or deleterious (exacerbate I/R injury). Besides the conundrum as to whether or not autophagy is an adaptive process, it is clearly regulated by a number of diverse molecules, including reactive oxygen species (ROS), various kinases, hydrogen sulfide (H2S) and nitric oxide (NO). The purpose of this review wa

Cardioprotective effect of isorhamnetin against myocardial ischemia reperfusion (I/R) injury in isolated rat heart through attenuation of apoptosis.

In this study, we investigated the effects of isorhamnetin on myocardial ischaemia reperfusion (I/R) injury in Langendorff-perfused rat hearts. Isorhamnetin treatment (5, 10 and 20 μg/mL) significantly alleviated cardiac morphological injury, reduced myocardial infarct size, decreased the levels of marker enzymes (LDH and CK) and improved the haemodynamic parameters, reflected by the elevated levels of the left ventricular developed pressure (LVDP), coronary flow (CF) and the maximum up/down velocity of left ventricular pressure (+dp/dtmax ). Moreover, isorhamnetin reperfusion inhibited apopto
