Metastatic calcification of the cardiac conduction system with heart block: an under-reported entity in chronic renal failure patients.

Systemic metastatic calcification is a common complication of chronic renal failure. Cardiac involvement is particularly ominous, especially when the cardiac conduction system is affected. Conduction defects, arrhythmias, and sudden death have all been reported with conduction system calcification; however, these are relatively under-reported or unrecognized causes of cardiac morbidity and mortality. We describe a 40-year-old man with Von Hippel-Lindau disease who had been maintained on hemodialysis for two years following bilateral nephrectomies for renal cell carcinoma. The patient presented

Prognostic value of serum angiotensin-converting enzyme activity for outcome of community-acquired pneumonia.

BACKGROUND: In a previous study, a relation between decreased serum angiotensin-converting enzyme (ACE) activity and physiological parameters was observed in patients with community-acquired pneumonia. The present study aims to further assess the prognostic value of serum ACE activity for outcome of community-acquired pneumonia. METHODS: This was a prospective observational study including two cohorts of patients with community-acquired pneumonia (2004-2006; n=157 and 2007-2010; n=138). Serum ACE activity was measured at time of hospital admission. Based on reference values in healthy persons

[Nursing home-sensitive conditions and approaches to reduce hospitalization of nursing home residents].

BackgroundInterventions to reduce potentially risky hospitalizations among nursing home residents are highly relevant for patient safety and quality improvement. A catalog of nursing home-sensitive conditions (NHSCs) grounds the policy recommendations and interventions.MethodsIn two previous research phases, an expert panel developed a catalog of 58 NHSCs using an adapted Delphi-procedure (the RAND/UCLA Appropriateness Method). This procedure was developed by the North American non-profit Research and Development Organisation (RAND) and clinicians of the University of California in Los Angeles

The Influence of Psychosocial Factors on Hospital Length of Stay Among Aging Canadians.

Background: Hospital stays that are prolonged due to non-clinical factors are costly to health care systems and are likely suboptimal for patient well-being. We assessed the influence of psychosocial factors on hospital length of stay (LOS) for older Canadians in a retrospective cohort study. Data and Methods: Data from the Canadian Community Health Survey were linked with the Discharge Abstract Database. Analyses were stratified by age, 55-64 (n = 1,060) and 65 and older (n = 2,718). Main predictor variables of interest included four measures of social support, sense of belonging, and living

Palliative and Hospice Care Mobile Applications: A Comprehensive Review and Recommendations.

Palliative and hospice care, traditionally provided in hospitals, are expanding into homes and communities with an increase in the elderly population and number of patients with chronic diseases. Despite the development of various mobile applications, the characteristics and relevance of these technologies in community settings have been insufficiently analyzed, resulting in a lack of clarity regarding their effectiveness. This systematic scoping review aimed to analyze and categorize the characteristic features of mobile applications for palliative and hospice care, and propose key attributes

Hospital readmissions among older people with intellectual disability in comparison with the general population.

BACKGROUND: Older people with intellectual disability have high multimorbidity and poor physical and mental health compared with the general population. Consequently, they have a greater need for health care. Hospital readmissions may be an indicator of the quality of health care. However, so far, only a few studies have investigated this outcome in populations of people with intellectual disability. None has focused on older people. METHOD: We identified a cohort of people with intellectual disability aged 55+ years and alive at the end of 2012 (n = 7936). Moreover, we established a referenc

Community household income and resource utilization for common inpatient pediatric conditions.

BACKGROUND AND OBJECTIVE: Child health is influenced by biomedical and socioeconomic factors. Few studies have explored the relationship between community-level income and inpatient resource utilization for children. Our objective was to analyze inpatient costs for children hospitalized with common conditions in relation to zip code-based median annual household income (HHI). METHODS: Retrospective national cohort from 32 freestanding children's hospitals for asthma, diabetes, bronchiolitis and respiratory syncytial virus, pneumonia, and kidney and urinary tract infections. Standardized cost

Indicators predicting use of mental health services in Piedmont, Italy.

BACKGROUND: Since the 1978 Italian reform, an integrated network of community mental health services has been introduced. With few exceptions, research on determinants of mental health service use at the district level has focused on inpatient activities and social deprivation indicators. The European Psychiatric Care Assessment Team (EPCAT) standardized methodology allows for an evidence-based comparison of mental health systems between geographical areas. AIMS: To compare service provision and utilization between local catchment areas; to explore quantitative relationships between residenti

Hospitalization and Housing: A Qualitative Study Exploring the Perspectives of Hospitalized Patients Experiencing Housing Insecurity.

Although housing insecurity has clear negative impacts on health, little is known about how it impacts patients' experience of hospitalization. In this qualitative study, we interviewed 22 hospitalized patients experiencing housing insecurity. The following three major themes emerged: 1) adverse social and environmental factors directly contribute to hospitalization, 2) lack of tailored care during hospitalization leaves patients unprepared for discharge, and 3) patients have difficulty recuperating after a hospital stay, leading to the risk of rehospitalization. Within these themes, participa

Implementing a Multidisciplinary Post-COVID Clinic in a Small Community Environment.

The emergence of Post-Acute Sequelae of Sars Cov-2 (PASC), also known as long-COVID, has prompted response from the medical community with research in how to treat patient's symptoms, and in some places, development of post-COVID clinics. Publications about PASC clinics thus far have been in large academic research centers, which have access to many specialists, yet only treat a small amount of the US population. Our hospital system was able to develop a multidisciplinary post-COVID clinic in a small rural community using a PM&R (Physical Medicine and Rehabilitation) physician lead, and the an

Youth and Parent Perspectives on Patient-Centered Outcomes for a Pediatric Hospital-Based Violence Intervention Program.

BACKGROUND: Hospital-based violence intervention programs (HVIPs) aim to interrupt cycles of violence and support recovery among youth exposed to interpersonal violence. However, HVIP evaluations often rely on deficit-oriented outcomes (i.e., reinjury, mortality, criminal justice system involvement) that may not reflect priorities of youth and families. OBJECTIVE: To determine youth and parent perspectives on patient-centered outcomes, needs, and priorities following violent injury. METHODS: Focus groups were conducted with youth and parents connected to community-based organizations (CBOs)

Role of Next-Generation Sequencing in Excluding the Nosocomial Origin of a Case of Legionnaires' Disease Integrating Environmental Surveillance and Clinical Diagnosis.

Legionella pneumophila (Lp) remains one of the major causes of community- and hospital-acquired pneumonia, yet its diagnosis and source attribution continue to pose significant challenges. Here, we describe the case of an immunocompromised patient who developed Legionnaires' disease during hospitalization. Following activation of the hospital's internal surveillance system, Lp and Legionella anisa (L. anisa) were recovered from multiple water distribution points using a simplified culture-based protocol. Whole-genome sequencing (WGS) demonstrated that all environmental isolates belonged to a s

Antibiotic Consumption and Resistance during a 3-Year Period in Sicily, Southern Italy.

Antimicrobial resistance (AMR) is one of the biggest issues facing global public health. In 2017, Italy adopted its first National Action Plan on Antimicrobial Resistance 2017-2020, which works through the synergy between national, regional, and local levels. In the framework of a Regional Action Plan on healthcare-associated infections and AMR prevention, the Sicilian Health Authority of the Sicilian Region, Southern Italy, has implemented a surveillance system of antibiotic consumption in hospitals, in the community, and of resistance rates (RRs) in hospital settings. Data on antibiotic cons

Increasing Medicaid clients' access to care. Church, state, and private providers collaborate.

Medicaid clients often have difficulty obtaining a physician referral and thus seek treatment for nonemergent conditions in hospital emergency rooms. A committee with representatives from Alexian Brothers Health System, Inc., Elk Grove Village, IL, and Catholic Charities of the Archdiocese of Chicago, Rolling Meadows, IL, has collaborated with six other organizations to put an end to this misuse of the healthcare system in Chicago's northwest suburbs. Catholic Charities Physician Referral Service came about as the result of research conducted by the committee which indicated that persons on pu

Rethinking Palliative Care Through Three Institutional Ethnographic Stories of People Living With Homelessness and Life-Limiting Illness.

Fifteen to thirty percent of Canadians have access to palliative care, with even fewer access opportunities for people with experiences of homelessness. Existing research identifies barriers to access but rarely shows how health and social institutions actively organize exclusion. Part of a larger study, this paper examines how health and social systems shape the need for community-based palliative and end-of-life care, using 3 stories from clients of the Community Allied Mobile Palliative Partnership (CAMPP). Using institutional ethnography, data were collected between Fall 2019 and Summer 20

[Application and Significance of Wastewater-based Pathogen Monitoring in Infectious Disease Surveillance: Insights from International Case Studies].

Wastewater-based monitoring of pathogens, such as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has emerged as a highly effective tool for infectious disease surveillance systems. Wastewater surveillance systems can detect emerging infectious diseases or new viral variants in a community earlier than hospital-based clinical surveillance systems, thereby preventing the spread of infections. In particular, it provides a foundation for rapid response to variants with higher transmissibility and virulence. This study aims to examine how wastewater-based pathogen surveillance can be
