Psychiatric hospitalization and rehospitalization among children and adolescents: evidence from 10-years electronic records in Beijing, China.

BACKGROUND: Enhancing inpatient services for children and adolescent mental illness is a primary goal of medical systems, yet knowledge of child and adolescent psychiatric hospitalization utilization is limited. This study aimed to characterize hospitalizations and rehospitalizations over a decade and to identify factors associated with rehospitalizations. METHODS: In this retrospective analysis, we obtained data from the Beijing Hospital Electronic Record Database from January 1, 2013, to December 31, 2022. We analyzed an admission-level dataset (n = 20215 admissions) to describe overall tre

A Qualitative Assessment of an Electronic Health Record-Embedded Intervention to Increase In-Hospital Opioid Use Disorder Treatment Initiation.

BACKGROUND: In-hospital initiation of medications for opioid use disorder (MOUD) remains low despite their effectiveness to reduce opioid-related mortality. We implemented an in-hospital OUD intervention across 12 hospitals to increase OUD treatment provision. OBJECTIVE: Understand factors impacting adoption and implementation of a non-interruptive electronic health record-embedded intervention which provided OUD treatment guidance. DESIGN AND PARTICIPANTS: We conducted focus groups and key informant interviews involving hospital-based physicians and advanced practice professionals (APPs), n

Clinical profiles of patients with antibiotic-resistant infections at select tertiary hospitals in Uganda: a retrospective study of clinical records.

BACKGROUND: Antibiotic resistance (ABR) remains a critical global health threat, with disproportionate burden in low-resource settings. METHODS: This retrospective study analyzed the clinical records of 599 patients across nine Uganda tertiary hospitals between March 2023 and April 2024. We described ABR rates and used multivariable mixed effects modified Poisson regression to determine the factors associated with ABR. We defined non-multidrug-resistance (non-MDR) as resistance to less than three antibiotic classes, MDR as resistance to at least three antibiotic classes, extremely drug-resist

Readmission and survival of hospitalized pulmonary tuberculosis patients: a nationwide record-based cohort analysis in Thailand (2017-2022).

BACKGROUND: Assessing the readmitted diseases and survivors in patients hospitalized with pulmonary tuberculosis (TB) is crucial for healthcare strategies tailored to TB survivors. This study aims to compare readmissions and survival of hospitalized pulmonary TB patients with those of their matched controls in Thailand. METHODS: We retrieved inpatient drug-sensitive pulmonary TB cases from the Thai Health Information Portal database (2017-2022), which included all hospitalized admissions in Thai hospitals. Non-TB controls were selected using 1∶1 propensity score matching by admission date, lo

From Steps to Mobility Levels: Validating a Consumer-Grade Activity Monitor for Automated Recording of Patient Mobility in Hospitals.

Patient mobility during hospitalization is essential for high-quality healthcare as mobility is linked to physical function and quality of life. The Johns Hopkins Highest Level of Mobility (JH-HLM) scale is a validated method to assess mobility in hospitalized patients. Although the JH-HLM is widely utilized, it has limitations including ceiling effects, unobserved mobility events going unrecorded, and the staff time needed to observe and document. We explored the feasibility of using a consumer-grade activity monitor (Fitbit) to predict JH-HLM scores and address these limitations.JH-HLM scores

Increased exacerbations and hospitalizations among PI*MZ compared to PI*MM individuals: an electronic health record analysis.

BACKGROUND: The best described endotype of COPD is alpha-1 antitrypsin (AAT) deficiency, due to a genetic abnormality in the SERPINA1 gene. Common deficient PI variants are the Z and S variants. Homozygotes for the Z allele (PI*ZZ individuals) carry the genotype most commonly associated with severe AAT deficiency (AATD), but a highly prevalent endotype is the heterozygous state (PI*MZ individuals). The effect of PI*MZ status on exacerbations and health care utilization is unknown. STUDY DESIGN AND METHODS: Cleveland electronic health record data was examined to compare healthcare utilization

Impact of Measles, Mumps, and Rubella Vaccination on Hospitalizations and Human Capital: Evidence from Copenhagen School Health Records.

Background: This paper investigates the long-term effects of the measles, mumps, and rubella (MMR) vaccine on healthcare, education, and economic outcomes using a novel dataset from the Copenhagen School Health Records Register. Methods: To address potential endogeneity, we use within-sibling variation in vaccination status induced by different periods of vaccine availability in Denmark. Results: Our findings reveal that, prior to the establishment of herd immunity, vaccinated cohorts experienced substantial protection against hospitalizations related to MMR. During the same period, we also ob

A neural network approach to predict opioid misuse among previously hospitalized patients using electronic health records.

Can Electronic Health Records (EHR) predict opioid misuse in general patient populations? This research trained three backpropagation neural networks to explore EHR predictors using existing patient data. Model 1 used patient diagnosis codes and was 75.5% accurate. Model 2 used patient prescriptions and was 64.9% accurate. Model 3 used both patient diagnosis codes and patient prescriptions and was 74.5% accurate. This suggests patient diagnosis codes are best able to predict opioid misuse. Opioid misusers have higher rates of drug abuse/mental health disorders than the general population, whic

The quality and the accuracy of codes for terminations of pregnancy for fetal anomalies recorded in hospital databases in three countries in northern Europe.

BACKGROUND: The number of terminations of pregnancy for fetal anomalies in Europe (TOPFA) has increased over recent decades. Therefore, it is important that TOPFAs, in addition to all other birth outcomes, are included in the surveillance of congenital anomalies and in studies on possible teratogenic risks of pregnancy exposures. The aim of this study was to evaluate the quality and the accuracy of codes identifying TOPFA cases in hospital databases. METHODS: TOPFA cases recorded in three EUROCAT congenital anomaly registries (Finland, 2010-2014; Funen in Denmark, 2005-2014; and northern Neth
