Characteristics of research volunteers for inpatient cocaine studies: focus on selection bias.

In order to investigate the selection bias of subjects for inpatient human cocaine studies, characteristics of 859 potential subjects were examined. Excluded subjects compared with accepted group were more likely to be single and male, currently use drugs other than cocaine, have a history of intravenous cocaine use, and have medical or mental health problems or physical complaints. Subjects who were accepted but did not participate, compared with participants, were likely to spend more money on cocaine. These results suggest that potential subjects who were accepted to our research studies ma

Countersymbiotic suicide.

In the course of studying the outcome of treatment for severely disturbed schizophrenics on a special ward, the authors encountered three instances of suicide with many features in common. All three cases involved young Hispanic male patients who demonstrated pathologically symbiotic relationships with their mothers. None had a history of overt suicide attempts prior to their actual death by jumping. These incidents occurred while the patients were en route to, or at, the maternal home on an unauthorized leave from the hospital. We present the case histories and offer a dynamic interpretation.

[Medico-sociologic analyses of the effectiveness of psychiatric consultation (author's transl)].

Certain observations led us to test the validity of the general assumption that hysterical syndromes occur more frequently among those inpatients who are refered to us from other departments of our faculty than among the psychiatric outpatients referred by private physicians. Such an assumption was verified in both hysterical neurotics and hysterical personality structures. Moreover, phobic neurotics, and depressive as well as infantile personality structures were underrepresented among the nonpsychiatric inpatients. These findings revealed a selection in psychiatric consultation. The social e

Variables associated with low, moderate and high emergency department use among patients with substance-related disorders.

AIMS: This study identified factors associated with frequency of emergency department (ED) use for medical reasons among patients with substance-related disorders (SRD) in Quebec (Canada) for 2014-15. METHODS: Participants (n = 4731) were categorized as: 1) low (1 visit/year), 2) moderate (2 visits/year), and 3) high (3+ visits/year) ED users. Independent variables included predisposing, enabling and needs factors based on the Andersen Behavioral Model. Multinomial logistic regression identified associated variables. RESULTS: Factors positively associated with moderate and high ED use includ

Postdischarge follow-up of psychiatric inpatients and readmission in an HMO setting.

This study examined the relationship between follow-up and rehospitalization of inpatients discharged from treatment in two divisions of a health maintenance organization (HMO). Among 580 patients discharged, two-thirds made a follow-up visit within 30 days. Slightly less than a third were readmitted within six months. Readmission was less likely for patients who made a follow-up visit and for men. Patients who had a preadmission relationship with a mental health practitioner were more likely to make a follow-up visit and were more likely to be readmitted. Follow-up was also associated with di

Customer-driven outcomes: a patient and family perspective.

Experiencing the healthcare system during an acute surgical event highlighted factors that contributed to customer-driven outcomes. Communicating intentions of and rationale for interventions increased the patient and family's confidence, and engaged the whole mind-body connection into the healing process. Utilizing the family as a repository of patient information incorporated their perspective, knowledge, and wisdom into the delivery and evaluation of patient care. Lastly, fostering the relationship between the nurse and the patient and family strengthened the therapeutic process, thus provi

Leveraging health information exchange for clinical research: Extreme underreporting of hospital service utilization among patients with substance use disorders.

BACKGROUND: Substance use disorders are associated with inefficient and fragmented use of healthcare services. The Chesapeake Regional Information System for Our Patients, Inc. (CRISP) is a Health Information Exchange (HIE) linking disparate systems of care in the mid-Atlantic region. METHODS: This article describes applications of HIE for tracking hospital service utilization in substance use disorder clinical and services research, drawing upon data from one of the first studies approved to access the CRISP HIE. Participants were 200 medical/surgical inpatients with comorbid opioid, cocaine

Identifying the Relation between Catheter-Associated Urinary Tract Infections and the National Distribution of Urologists and Urology Residencies.

Background: Catheter-associated urinary tract infections (CAUTIs) are associated with urinary catheterization. Up to 25% of hospitalized patients may be catheterized during their stay. Urethral catheters are necessary as part of many urologic procedures, but the removal of unnecessary catheters has become more important to reduce infections, healthcare costs, and patient morbidity. Methods: Open access data from the U.S. Centers for Disease Control and Prevention (CDC), United States Census, and the American Urological Association (AUA) Census was obtained, and a linear correlation used to det

Time, space and opportunity in the outpatient consultation: 'the doctor's story'.

Using excerpts from videotaped consultations of physicians meeting with Inflammatory Bowel Disease (IBD) patients in a UK hospital, this article pays attention to biographic or narrative time over the course of three consultations. On the basis of this approach an important role emerges for what we term proto-stories that doctors may tell to their patients. This shift from patients' to doctors' stories is linked to visual evidence regarding the embodiment of doctor and patient. Taken together, these topics indicate differences in the way that space and time are constructed in the consultation,

Association of homelessness and psychiatric hospital readmission-a retrospective cohort study 2016-2020.

BACKGROUND: A large proportion of adult psychiatric inpatients experience homelessness and are often discharged to unstable accommodation or the street. It is unclear whether homelessness impacts psychiatric hospital readmission. Our primary objective was to examine the association between homelessness and risk for 30-day and 90-day readmission following discharge from a psychiatric unit at a single urban hospital. METHODS: A retrospective cohort study involving health administrative data among individuals (n = 3907) in Vancouver, Canada with an acute psychiatric admission between January 201

Hospital Initiated Injectable Opioid Agonist Therapy for the Treatment of Severe Opioid Use Disorder: A Case Series.

BACKGROUND: Across North America, there is an unprecedented opioid overdose epidemic. Approximately 15% of individuals with severe opioid use disorder (OUD) do not benefit from opioid agonist therapy (OAT) such as buprenorphine/naloxone or methadone and are considered treatment refractory. Of those who inject, injectable OAT (iOAT), with hydromorphone or diacetylmorphine, offered in community settings has demonstrated improved retention to treatment and decreased nonprescription opioid use. This case series seeks to describe iOAT initiation and titration in a hospital setting for treatment ref

"You don't want anyone who hasn't been through anything telling you what to do, because how do they know?": Qualitative analysis of case managers in a hospital-based violence intervention program.

STATEMENT OF PURPOSE: Intentional violent injury is a leading cause of disability and death among young adults in the United States. Hospital-based violence intervention programs (HVIPs), which strive to prevent re-injury through intensive case management, have emerged as a successful and cost-effective strategy to address this issue. Despite the importance of strong therapeutic relationships between clients and their case managers, specific case manager behaviors and attributes that drive the formation of these relationships have not been elucidated. METHODS: A qualitative analysis with a mo

A perspective on therapeutic work with in-patient adolescents.

It is generally difficult to explain the complex practice of psychotherapeutic work with in-patient adolescents. This is because the adolescent workers' relatively "relaxed" enterprise of promoting the process of self-understanding within the adolescents is overshadowed by the concurrent high profile activity of their managing these troubled young people. This paper attempts to address what is indeed a perennial difficulty by way of a close study or phenomenology of the workers' on-going therapeutic relationships with in-patient adolescents. For the purposes of the investigation the author uti
