The cerebrospinal fluid (CSF) pressure patterns have been reported as one of the most relevant indexes for the diagnosis and treatment of idiopathic normal-pressure hydrocephalus (INPH). Forty consecutive patients coming from our observations with the classic Hakim's triad underwent continuous CSF pressure monitoring via lumbar puncture for at least 12 hours. Twenty-eight patients were diagnosed as having INPH and underwent CSF shunt. A multi-layer neural network (perceptron) was employed to study the pressure patterns in order to try an alternative classification to the "expert" neurosurgeon

FROM THE RECORDS of approximately 1500 shunt operations performed between 1987 and 1992, we identified 37 adults between ages 38 and 86 years (mean, 70 yr) with the normal-pressure hydrocephalus (NPH) syndrome who underwent surgery by a single surgeon. Since 1990, we have routinely used a flow-regulated shunt system (Orbis-Sigma valve [OSV]; Cordis Corporation, Miami, FL) in these patients. In this study, we compared the OSV system with conventional differential-pressure (DP) shunt systems uniformly used before 1990. This series (n = 37) consisted of 62% men (n = 23) and 38% women (n = 14). We

BACKGROUND: Disproportionately enlarged subarachnoid-space hydrocephalus (DESH) is a key feature of Hakim's disease (synonymous with idiopathic normal pressure hydrocephalus; iNPH). However, it previously had been only subjectively evaluated.
PURPOSE: This study aims to evaluate the usefulness of MRI indices, derived from deep learning segmentation of cerebrospinal fluid (CSF) spaces, for DESH detection and to establish their optimal thresholds.
MATERIALS AND METHODS: This study retrospectively enrolled a total of 1009 participants, including 77 patients diagnosed with Hakim's disease, 380 h

PURPOSE: To investigate differences in cerebrospinal fluid (CSF) flow through the aqueduct and to determine whether there is a relationship between CSF flow and ventricular volume parameters in idiopathic normal pressure hydrocephalus (iNPH) patients, elderly acquired hydrocephalus patients and age-matched healthy volunteers by phase-contrast MR (PC-MR).
METHODS: A total of 40 iNPH patients and 41 elderly acquired hydrocephalus patients and 26 age-matched healthy volunteers in the normal control (NC) group were included between November 2017 and October 2019 in this retrospective study. The f

Herpes simplex virus type 1 (HSV-1) encephalitis can mimic neurodegenerative or structural disorders, particularly in older adults presenting with cognitive or behavioral symptoms. We report the case of a 73-year-old woman with progressive gait instability, urinary incontinence, and cognitive decline, which was initially attributed to normal pressure hydrocephalus (NPH). Despite transient improvement after two lumbar punctures and neuroimaging suggestive of significant ventriculomegaly, the patient's symptoms progressed with increasing paranoia, hallucinations, and agitation. Psychiatric consu

BACKGROUND: The etiology of NPH is unknown, but might be related to altered craniospinal compliance caused by spinal canal narrowing, as has been suggested by in-vitro simulation studies. However, clinical evidence is lacking. We explored associations between cervical (CS) and lumbar (LS) spinal stenoses, and NPH-imaging characteristics.
METHODS: Spinal MRI of NPH-patients scheduled for shunt-surgery was retrospectively evaluated by measuring the minimal dural sac cross-sectional area (DSCSA), and anterior-posterior diameter (AP) in CS and LS. Cranial-CT was evaluated regarding Evans-Index, c

BACKGROUND: Limited data are available regarding the long-term functional outcomes and associated factors in patients with idiopathic normal pressure hydrocephalus (iNPH) undergoing ventriculoperitoneal shunt (VPS) placement. This study aimed to retrospectively evaluate the long-term outcomes of iNPH patients treated with VPS.
METHODS: Functional outcomes were assessed preoperatively and at 1-year, 2-year, and 3-year intervals postoperatively using the modified Rankin Scale (mRS), the iNPH grading scale (iNPHGS), and the Mini-Mental State Examination (MMSE).
RESULTS: Significant improvements

Capillary malformations in the central nervous system, especially in the cauda equina, are rare. Although the typical symptoms are lower back pain and motor weakness in the lower extremities, we report here a rare case in which the initial symptom of a capillary malformation in the cauda equina was normal-pressure hydrocephalus. A 67-year-old woman was admitted to our hospital for close examination of hydrocephalus. Imaging studies showed no abnormalities other than an 8-mm intradural extramedullary tumor at the lumbar 2 level. Given lumbar puncture revealed a high protein level and the pressu

PURPOSE: Degenerative cervical myelopathy (DCM) and idiopathic normal pressure hydrocephalus (iNPH) share symptoms such as gait disturbance and urinary dysfunction, especially in the elderly. While the incidence of DCM in patients with iNPH is reported, the prevalence of iNPH in patients with DCM remains unclear. This study investigates the prevalence of iNPH in DCM patients.
METHODS: This retrospective cohort study included 83 patients undergoing decompression surgery for DCM between April 2018 and March 2023. Brain MRI performed within three years prior to surgery was analyzed. Patients wer

Normal Pressure Hydrocephalus (NPH) is a syndrome in predominantly older adults, differential diagnoses include primary neurodegenerative diseases. The key radiological finding is ventricular enlargement, different from increased ventricular size associated with cerebral atrophy. After definitive surgical cerebrospinal fluid (CSF) shunting there appears to be increased brain tissue volume in patients' follow-up images which could indicate that imaging findings usually interpreted as cerebral atrophy may be reversible with appropriate treatment. A cross-sectional study was performed comparing p

INTRODUCTION: Gait disturbances are a key symptom in normal pressure hydrocephalus (NPH) and Parkinson's disease (PD). We employed instrument-supported gait analysis to characterize commonalities and differences in gait patterns among PD and NPH patients, as well as gait changes following disease-specific interventions (i.e., spinal tap and dopaminergic medication).
METHODS: Gait and balance analyses were conducted i) before and after NPH patients (n = 19) received a spinal tap, and ii) before and after PD patients (n = 19) received soluble dopaminergic medication. Change scores were calculat

BACKGROUND: The pathophysiology of idiopathic normal pressure hydrocephalus (iNPH) remains poorly understood. While it is commonly accepted that iNPH has an insidious onset, little is known about its preclinical and early stages and its development over time.
OBJECTIVES: To gain more insight into how iNPH becomes manifest clinically and radiologically, and how its major clinical symptoms evolve in non-shunted patients.
METHODS: For this critical review a literature search was performed using specific search terms concerning the evolution of iNPH. Manuscripts were categorized according to the

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a progressive yet potentially reversible neurological disorder affecting older adults, characterized by gait disturbance, urinary incontinence, and cognitive impairment. Unlike most dementias, iNPH responds to cerebrospinal fluid shunting, with early intervention yielding optimal outcomes.
OBJECTIVES: To examine the ethical obligations of emergency physicians (EPs) in recognizing iNPH and to review practical screening methods applicable to emergency department (ED) settings.
DISCUSSION: Despite affecting approximately 2% to 5% of

BACKGROUND: Amyloid-β (Aβ) is one of the hallmark lesions of Alzheimer's disease (AD). During the disease process, Aβ undergoes biochemical changes, producing toxic Aβ variants, proposed to be detected within the neurons. Idiopathic normal pressure hydrocephalus (iNPH) causes cognitive impairment, gait, and urinary symptoms in elderly, that can be reversed by a ventriculo-peritoneal shunt. Majority of iNPH subjects display different Aβ variants in their brain biopsies, obtained during shunting.
OBJECTIVE: To study the cellular compartmentalization of different Aβ variants in brain biopsies fr

Hodgkin's lymphoma is a malignancy of the lymphatic system that can rarely present with advanced-stage features such as spinal cord involvement and hypercalcemia. We present the case of a 63-year-old Caucasian male with advanced Hodgkin's lymphoma, presenting with hypercalcemia and symptoms resembling normal pressure hydrocephalus. The patient exhibited gait impairment, altered mental status, and urinary incontinence, forming the classic triad seen in normal pressure hydrocephalus. Laboratory findings revealed leukocytosis, anemia, hypercalcemia, and acute kidney injury. Imaging showed extensi

BACKGROUND AND PURPOSE: Differentiating idiopathic normal pressure hydrocephalus (iNPH) from neurodegenerative disorders such as progressive supranuclear palsy (PSP), Multiple System Atrophy-parkinsonian type (MSA-P), and vascular dementia (VaD) is challenging due to overlapping clinical and neuroimaging findings. This study assesses if quantitative brain stem and cerebellum metrics can aid in this differentiation.
METHODS: We retrospectively compared the sagittal midbrain area, midbrain to pons ratio, MR parkinsonism index (MRPI), and cerebellar atrophy in 30 PSP patients, 31 iNPH patients,

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2 virus) has been reported to cause significant injury to the central nervous system (CNS). Herein, we describe the case of a 48-year-old male with a past medical history of attention-deficit/hyperactivity disorder (ADHD), hypertension, and hyperlipidemia who developed typical symptomatology of normal pressure hydrocephalus (NPH) with cognitive impairment, gait dysfunction, and urinary incontinence after a mild coronavirus disease (COVID-19) infection. The diagnosis was confirmed by imaging and lumbar puncture (LP). The patient was trea
