Clinical presentation of hypernatremic dehydration in exclusively breast-fed neonates.

OBJECTIVE: To identify the clinical presentation of dehydration related to failure of lactation in exclusively breast-fed term infants. METHOD: A prospective study was performed between January 2000 and June 2003 in Al Qassimi Hospital in the Emirate of Sharjah. Enrollment criteria included term neonates whose birth weight of > 2000 g with no underlying organic illness causing poor feeding admitted for clinical manifestations of dehydration with weight loss of > 10% during the first 2 weeks of life. The control group, a non-randomized sample included healthy full term neonates, seen in Sharja

[Accidental saline poisoning in newborn infants. Morphologic findings and pathogenetic discussion].

In the nursery of an Austrian hospital, five newborns accidentally received 10% natrium chloride solution by infusion or oral feeding. Because of wrong labelling by the hospital pharmacy the solution was taken for 10% glucose. In all five newborns, bradycardia, edemas, cyanosis and cramps occurred. In all cases, autopsy revealed a fatal haemorrhagic encephalopathy and more or less extended haemorrhages of the lungs, the kidneys and the liver. There was no suppurative pneumonia and no arteritis. The intracerebral bleedings, thromboses and necroses may be provoked by tissue shrinking caused by h

Sodium poisoning: a lethal case of a burned child.

An unusual case of lethal salt poisoning of a burned child is presented. Excessive hypernatraemia and hyperosmolality resulted in an irreversible cascade of events which lead to death despite vigorous attempts to rescue the victim.

[Electrolyte abnormalities associated with cancer: a review].

There are a variety of water and electrolyte disorders in patients with cancer. These disorders occur during the growth of tumors, generally as a consequence of inadequate intake and absorption of electrolytes, renal failure secondary to tumor or rapid tumor destruction and production of metabolically active substances by the tumor. In this paper, the electrolyte abnormalities associated with cancer were reviewed. Hyponatremia is one of the most common clinical electrolyte abnormalities in advanced cancer. Some patients may have hyponatremia, in spite of increased total body sodium and absence

Clinical outcomes associated with dynamic changes in serum sodium amongst adult patients with spontaneous subarachnoid hemorrhage and other critical illnesses: an exploratory scoping review.

Incident dysnatremia has been reported amongst varied intensive care unit disease populations and is associated with worse clinical outcomes, but less is known about the impacts of dynamic changes in serum sodium during intensive care unit admission. The primary objective of this study was to conduct a scoping review of the published literature on patient outcomes associated with dynamic changes in serum sodium in critically ill adults, to inform future research priorities. A scoping review was conducted according to the Joanna Briggs Institute method. PubMed®, Embase, CINAHL, Scopus, and Web

The natural history of untreated X-linked nephrogenic diabetes insipidus with mutation in the vasopressin V2 receptor gene.

Nephrogenic diabetes insipidus (NDI) results from the kidneys' inability to concentrate urine. We describe a 6-month-old male with a history of poor weight gain who presented with an incidental finding of hypernatremia (155 mEq/L) during an episode of acute gastroenteritis. The arginine vasopressin (AVP) test, along with molecular analysis revealing the M272R mutation in the AVP receptor 2 (AVPR2) gene, confirmed the diagnosis of congenital NDI. Interestingly, this mutation was also identified in the patient's maternal grandfather, who had never been diagnosed or treated for NDI despite a hist…

Acute onset of central diabetes insipidus during adolescent idiopathic scoliosis surgery.

This is a case report of a 12-year-old female with idiopathic scoliosis undergoing posterior scoliosis reconstruction that developed an acute onset of central diabetes insipidus (CDI) during surgery. The diagnosis was made as a result of an intraoperative increase in urine output to 6.82 ml/kg/hour (550 ml), a serum sodium increase from 139 to 147 mmol/L, a serum osmolality of 299 mOsmol/kg, a rapid response to vasopressin administration and a low vasopressin level of < 0.5 picograms/ml (normal, 1-5 picograms/ml). This is the first reported case of CDI with a documented low vasopressin level t

Afferent mechanisms of sodium retention in cirrhosis and hepatorenal syndrome.

Cirrhosis induces extra-cellular fluid volume expansion, which when the disease is advanced can be severe and poorly responsive to therapy. Prevention and/or effective therapy for cirrhotic edema requires understanding the stimulus that initiates and maintains sodium retention. Despite much study, this stimulus remains unknown. Work over the last several years has shown that signals originating in the liver can influence a variety of systemic functions, including extra-cellular fluid volume control. We review work on the afferent mechanisms triggering sodium retention in cirrhosis and suggest

Hypernatraemic dehydration in a neonate: brain MRI findings.

Severe hypernatremic dehydration can cause serious neurological complications in neonates. The most significant problems include brain oedema, intracranial haemorrhage, sinus thrombosis, haemorrhagic infarcts and permanent brain damage. The symptoms of many of these complications are similar. With respect to brain MRI findings in hypernatremic neonates, this is a report that describes linear lesions that represent intracranial haemorrhage at the grey-white matter junction. These MRI findings may be helpful for diagnosing hypernatremic dehydration, and for ruling out differential diagnoses for
