Left ventricular hypertrophy in incident dialysis patients randomized to treatment with hemofiltration or hemodialysis: results from the ProFil study.

INTRODUCTION: Left ventricular hypertrophy (LVH) is present in a majority of hemodialysis (HD) patients and is among the strongest risk factors for cardiovascular events and mortality. Hemofiltration (HF), a purely convective dialysis treatment, has been associated with enhanced hemodynamic stability compared with HD, possibly as a result of a more physiologic removal of fluid and solutes. METHODS: In a randomized controlled study conducted at ten dialysis centers in Sweden and Denmark, incident patients (HD <3 months) without clinical signs or history of cardiovascular disease were randomize

[Influence of Chrysanthemum indium on collagen accumulation and signaling transduction pathways in left ventricular tissue of cardiac hypertrophy in rats].

OBJECTIVE: To evaluate the influence of Chrysanthemum indium on collagen accumulation and signaling transduction pathways in left ventricle tissue of cardiac hypertrophy induced by abdominal aortic banding in rats. METHOD: Ventricular remodeling was induced by abdominal aortic banding (AAB) in rats. After 35 day treatment, the blood pressure was measured, then the ratios of LVW/BW and HW/BW were calculated. The histological assay was performed by HE staining for determining the myocardium cell cross section and picric acid/sirius red staining for determining collagen content. Immunohistochemi

Association of left ventricular hypertrophy and chronic atrial fibrillation with the incidence of new thromboembolic stroke in 2,384 older persons.

In a prospective study of 2,384 persons, mean age 81 years, at 44-month follow-up, new thromboembolic stroke developed in 510 of 2,384 persons (21%). The Cox regression model showed that significant independent risk factors for new thromboembolic stroke were atrial fibrillation (risk ratio 3.2), left ventricular hypertrophy (risk ratio 2.8), prior stroke (risk ratio 2.2), and male gender (risk ratio 1.2).

Left ventricular hypertrophy is more prominent in patients with primary aldosteronism than in patients with other types of secondary hypertension.

We determined functional and morphological changes of the heart by 2-dimensional and pulse Doppler echocardiography in 20 patients with primary aldosteronism and compared the results with those in 50 healthy normotensive subjects, 12 patients with Cushing's syndrome, 9 patients with pheochromocytoma, and 47 patients with essential hypertension. All hypertensive groups had greater left ventricular mass indexes than did the normotensive group (76.9 +/- 17.2 g/m2). Despite similar age distribution, blood pressure during antihypertensive treatment, and duration of hypertension, the primary aldoste

Effects of menidipine on dihydropyridines binding sites in cardiac and cerebral membranes from old rats with left ventricular hypertrophy.

AIM: To study the effects of menidipine (Men) on the affinity and density of dihydropyridines (DHP) binding sites in the cell membranes of left ventricle (LV) and brain in elderly renovascular hypertensive rats (RVHR) with LV hypertrophy. METHODS: Renovascular hypertension was produced by clipping the left renal artery in 20-month-old rats. The affinity and density of DHP binding sites in the cell membranes of LV and brain were assessed by radioligand assay. RESULTS: Men (20 mg.kg-1.d-1 ig for 9 wk) decreased markedly the systolic blood pressure and the LV weight (P < 0.01). Though not affec

Regression of left ventricular hypertrophy with 1 year of antihypertensive treatment in type 1 diabetic patients with early nephropathy.

M-mode echocardiograms were recorded in 22 Type 1 diabetic patients with microalbuminuria (n = 10) or early persistent proteinuria (n = 12). Eight (36%) had both an increased left ventricular mass (males greater than 131 g m-2; females greater than 100 g m-2) and a systolic blood pressure above the 75th centile of the normal blood pressure distribution. These eight patients were treated with antihypertensive drugs, predominantly enalapril, for 1 year. Echocardiograms were repeated after 3 and 12 months. Systolic blood pressure at recruitment was 155 +/- 14 (+/- SD) mmHg, and was significantly

A study of the subendocardial ultrastructural changes in the volume-overloaded left ventricular hypertrophy: a comparison of the effects of aortic and mitral regurgitation.

Ultrastructural changes of the subendocardial layer of the volume-overloaded left ventricle were studied in patients who underwent open heart surgery for chronic aortic valve disease (4 patients) and mitral valve disease (6 patients). Patients in these two entities were estimated to be in the same level of hemodynamic functional capacity. In aortic valve disease, the subendocardial layer of the hypertrophied left ventricle showed degeneration of cardiac muscle cells. Conversely, degeneration of cardiac muscle was rarely observed in those with mitral valve disease. Numerous small and medium siz

Normal pulmonary vascular resistance and left ventricular hypertrophy in young infants with bronchopulmonary dysplasia: an echocardiographic and pathologic study.

To evaluate the cardiac anatomy and functional hemodynamics in young infants with chronic lung disease, nine patients, aged 2 to 7 months, with a clinical diagnosis of bronchopulmonary dysplasia (BPD) underwent echocardiographic examination. All infants required supplemental O2 (mean FIO2 35%) to maintain adequate systemic oxygenation (Pao2 greater than 50 mm Hg). None of the infants had evidence of a patent ductus arteriosus at the time of examination. Echocardiographic measurements of left and right ventricular systolic time intervals revealed normal systolic time interval ratios suggesting

Association between Neutrophil-to-Lymphocyte ratio and left ventricular hypertrophy in chronic kidney disease patients: a cross-sectional study.

BACKGROUND: Left ventricular hypertrophy (LVH) is a common cardiovascular complication in chronic kidney disease (CKD), associated with increased morbidity and mortality. Systemic inflammation may contribute to LVH, but evidence remains limited in non-dialysis CKD populations. The neutrophil-to-lymphocyte ratio (NLR), a readily accessible inflammatory marker, may serve as a predictor of LVH. METHODS: In this cross-sectional study, 514 hospitalized CKD patients were enrolled. LVH was defined by echocardiographic criteria based on left ventricular mass index. NLR was calculated from complete bl

An Unusual Case of Transthyretin Cardiac Amyloidosis Presenting as Heart Failure With Improved Ejection Fraction Without Left Ventricular Hypertrophy.

Heart failure with preserved ejection fraction (EF) and left ventricular (LV) hypertrophy (LVH) are typical phenotypes of transthyretin cardiac amyloidosis (ATTR-CA). Extracardiac manifestations, such as carpal tunnel syndrome, lumbar spinal stenosis, or biceps tendon rupture, frequently precede the development of overt cardiac involvement. We report a rare case of ATTR-CA presenting as heart failure with improved EF (HFimpEF) without LVH in an elderly patient, where the diagnosis was prompted by bilateral carpal tunnel syndrome, lumbar spinal canal stenosis, and persistently elevated troponin

Bicuspid aortic valve with associated aortopathy, significant left ventricular hypertrophy or concomitant hypertrophic cardiomyopathy: A diagnostic and therapeutic challenge.

Due to its prevalence of 0.5% to 2% in the general population, with a 75% predominance among men, bicuspid aortic valve is the most common congenital heart defect. It is frequently accompanied by other cardiac congenital anomalies, and clinical presentation can vary significantly, with stenosis being the most common manifestation, often resulting in mild to moderate concentric hypertrophy of the left ventricle. Echocardiography is the primary diagnostic modality utilized for establishing the diagnosis, and it is often the sole diagnostic tool relied upon by clinicians. However, due to the hete

Nonalcoholic Fatty Liver Disease Is Related to Abnormal Corrected QT Interval and Left Ventricular Hypertrophy in Chinese Male Steelworkers.

OBJECTIVES: Nonalcoholic fatty liver disease (NAFLD) has been associated with an increased risks of corrected QT (QTc) prolongation and left ventricular hypertrophy (LVH), both of which are associated with the development of cardiovascular disease. Rotating night shift work and a higher risk of incident NAFLD have been reported in male steelworkers. This study aimed to investigate the association of the severity of NAFLD with a prolonged QTc interval and LVH in a large cohort of Chinese male steelworkers. METHODS: We examined baseline data of 2998 male steel workers aged 26 to 71 years at two

In-center Nocturnal Hemodialysis Reduced the Circulating FGF23, Left Ventricular Hypertrophy, and All-Cause Mortality: A Retrospective Cohort Study.

Fibroblast growth factor 23(FGF23) is the most important biomarker and pathogenic factor in Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD). In the moderate and severe stages of chronic renal failure, abnormally elevated circulating FGF23 can lead to some complications, including myocardial hypertrophy, which is positively correlated with all-cause mortality. However, the circulating FGF23 level of different hemodialysis modalities, the underlying essential regulatory factors, and potential clinical benefits remain to be elucidated. In this retrospective cohort study, 90 in-center n

Pressure overload by suprarenal aortic constriction in mice leads to left ventricular hypertrophy without c-Kit expression in cardiomyocytes.

Animal models of pressure overload are valuable for understanding hypertensive heart disease. We characterised a surgical model of pressure overload-induced hypertrophy in C57BL/6J mice produced by suprarenal aortic constriction (SAC). Compared to sham controls, at one week post-SAC systolic blood pressure was significantly elevated and left ventricular (LV) hypertrophy was evident by a 50% increase in the LV weight-to-tibia length ratio due to cardiomyocyte hypertrophy. As a result, LV end-diastolic wall thickness-to-chamber radius (h/R) ratio increased, consistent with the development of con

Maxacalcitol (22-Oxacalcitriol (OCT)) Retards Progression of Left Ventricular Hypertrophy with Renal Dysfunction Through Inhibition of Calcineurin-NFAT Activity.

PURPOSE: Left ventricular hypertrophy (LVH) is a cardiovascular complication highly prevalent in patients with chronic kidney disease (CKD). Previous studies analyzing 1α-hydroxylase or vitamin D receptor (Vdr) knockout mice revealed active vitamin D as a promising agent inhibiting LVH progression. Paricalcitol, an active vitamin D analog, failed to suppress the progression of LV mass index (LVMI) in pre-dialysis patients with CKD. As target genes of activated VDR differ depending on its agonists, we examined the effects of maxacalcitol (22-oxacalcitriol: OCT), a less calcemic active vitamin D

Risk factors for increased left ventricular hypertrophy in patients with chronic kidney disease: findings from the CKD-JAC study.

BACKGROUND: Although left ventricular hypertrophy (LVH) has been established as a predictor of cardiovascular events in chronic kidney disease (CKD), the relationship between the prevalence of LVH and CKD stage during the pre-dialysis period has not been fully examined. METHODS: We measured left ventricular mass index (LVMI) in a cross-sectional cohort of participants in the Chronic Kidney Disease Japan Cohort (CKD-JAC) study to identify factors that are associated with increased LVMI in patients with stage 3-5 CKD. RESULTS: We analyzed the baseline characteristics in 1088 participants (male
