Strangulated appendix within a trocar site incisional hernia following laparoscopic low anterior rectal resection. A case report.

Appendix strangulation caused by an incisional trocar site hernia following laparoscopy is an extremely rare entity. A 63-year-old man was admitted to our department with continuous abdominal pain and irreducible minimal swelling in the right abdomen. His surgical history was significant for rectal cancer one year previously. The patient had undergone a laparoscopic low anterior rectal resection with construction of a protective temporary end ileostomy (pT1, pN0, G2, M0). The ileostomy was reversed 3 months after initial surgery. An incarcerated herniation was diagnosed. Subsequent surgery sur…

Colonic atresia and Hirschsprung's disease in a neonate: A case report.

INTRODUCTION: Colonic atresia is a rare form of intestinal atresia that can be encountered in neonates. Although uncommon, other disease processes can be found simultaneously including malrotation, additional atresias, gastroschisis, and Hirschsprung's disease. CASE PRESENTATION: A 2-day-old female neonate with known maternal polysubstance use was found to have colonic atresia on contrast enema after emesis and failure to pass meconium. Abdominal exploration revealed a blind ending cecum with evidence of ischemia along with an atretic transverse colon. An ileocecectomy with end ileostomy and …

Metabolic acidosis during urinary retention in a patient with an enterovesical fistula.

We report a patient known to have an enterovesical fistula who presented severe acute metabolic acidosis during an episode of urinary retention. The enterovesical fistula which had been intermittently symptomatic for 4 years, had developed after several intestinal surgical procedures and related intraperitoneal sepsis following resection of colon cancer 21 years previously. The patient who had a total colectomy and ileostomy, was admitted for hip replacement with the routine placement of a Foley bladder catheter. Three weeks post-operatively, the patient developed acute urinary retention follo…

Traveling with an Ostomy.

People undergoing ostomy surgery face an extended physiologic and psychosocial recovery. It is not uncommon to experience interruptions in work, pursuing life goals, relationships, physical activity, and social life. Even those with previous travel experience may feel inhibited by their new ostomy. Nurses, including those experienced in wound and ostomy continence, provide essential care to patients recovering from surgery and adjusting to their stoma. With counseling, education, and experience, patients can develop increasing competence and autonomy. This, in turn, helps an ostomate look forw

Association Between Radiotherapy (±Chemotherapy) and the Severity of Low Anterior Resection Syndrome After Rectal Cancer Surgery: Does Radiotherapy Separate Risk?

Background: Low anterior resection syndrome (LARS) is a frequent survivorship problem after sphincter-preserving rectal cancer surgery. Pelv … gression assessed associations with major LARS, adjusting a priori for tumor location and diverting ileostomy; furthermore, extended sensitivity models incorporated technical/pathway variables. Discrimination was explored using 5-fold cross-validated ROC/AUC. Item-level LARS responses were analyzed to characterize symptom phenotype. Results: Overall, 182 pa …

Risk Factors for Stoma Outlet Obstruction after Proctocolectomy for Ulcerative Colitis.

OBJECTIVES: Stoma outlet obstruction (SOO) occurs with an incidence of approximately 40% after proctocolectomy for Ulcerative colitis (UC) with diverting ileostomy. This study aimed to identify the risk factors for SOO after proctocolectomy with diverting ileostomy for patients with UC. METHODS: We reviewed the data of 68 patients with UC who underwent proctocolectomy and diverting ileostomy between April 2006 and September 2021. These cases were analyzed on the basis of clinicopathological and anatomical factors. SOO was defined as small bowel obstruction displaying symptoms of intestinal ob…

Aryl hydrocarbon Receptor activation during in vitro and in vivo digestion of raw and cooked broccoli (brassica oleracea var. Italica).

Broccoli is rich in glucosinolates, which can be converted upon chewing and processing into Aryl hydrocarbon Receptor (AhR) ligands. Activation of AhR plays an important role in overall gut homeostasis but the role of broccoli processing on the generation of AhR ligands is still largely unknown. In this study, the effects of temperature, cooking method (steaming versus boiling), gastric pH and further digestion of broccoli on AhR activation were investigated in vitro and in ileostomy subjects. For the in vitro study, raw, steamed (t = 3 min and t = 6 min) and boiled (t = 3 min and t = 6 min) b…

Colectomy and endorectal ileal pull-through with lateral ileal reservoir for ulcerative colitis and polyposis in children.

Nineteen children with chronic ulcerative colitis refractory to medical therapy and one with multiple polyposis, all under 20 years of age, underwent total colectomy, mucosal proctectomy, and endorectal ileal pull-through with ileoanal anastomosis at the UCLA Medical Center during a 4 1/2-year period (mean age, 14.4 years). Seventeen patients underwent second-stage closure of the ileostomy with construction of a side-to-side isoperistaltic ileal reservoir (mean, 6 months) after the ileal pull-through operation. The anastomosis extended over a 20- to 30-cm distance and the lower end was placed …

Protective defunctioning stoma in low anterior resection for rectal carcinoma.

BACKGROUND: Anastomotic leak is a serious complication of resection for low rectal carcinoma. METHODS: Data from a prospective multicentre study conducted between January 2000 and December 2001 were analysed to determine the early outcome after low anterior resection in patients with and without a protective stoma. The morbidity and mortality rates associated with ileostomy and colostomy closure were compared. RESULTS: Eight hundred and eighty-one (32.3 per cent) of 2729 patients received a protective stoma after low anterior resection. Overall anastomotic leak rates were similar in patients…

[Can continence function after rectal resection be prognostically estimated?].

AIM: To determine clinical and physiologic parameters enabling the prognosis of continence after protective ileostomy closure secondary to rectal resection for rectal cancer. METHODS: Patients who had undergone rectal resection (n = 65, of whom 24 had had radiochemotherapy) were evaluated by clinical examination, anorectal manometry and orthograde contrast enema before ileostomy closure. Continence was evaluated by clinical findings 91 +/- 52 weeks after stoma closure with the help of standardized questionnaires and classified according to the Wexner continence score. The relationship between…

Completion Proctectomy for Crohn's Colitis: Lessons Learned.

Surgical management of Crohn's disease is reserved for patients refractory to medical therapy and those who develop complications alleviated by surgery. Surgical resection may be the most efficient way to restore health in patients with stricturing and or fistulizing disease of the terminal ileum / small bowel. However, decision-making in patients with Crohn's colitis is more difficult. The merits of segmental resection versus subtotal/total colectomy versus total proctocolectomy with end ileostomy are affected by a myriad of factors, including extent of colon involvement, the patient's age, a…

The Appendix and Aganglionosis. A Note of Caution-How the Histology Can Mislead the Surgeon in Total Colonic Hirschsprung Disease.

We present the case of a child with presumed total colonic Hirschsprung disease (HD) to highlight the problems the surgeon is likely to encounter if he/she relies on the appendix alone for histopathologic diagnosis. A newborn male infant, who was presumed to have total colonic aganglionosis when the appendix was found to be aganglionic at the time of initial exploratory laparoscopy, was managed with an ileostomy in the newborn period; however, at the time of his planned pull-through procedure, the rectal biopsy revealed normal ganglion cells. The child was subsequently managed with ileostomy c…

Use of a long-acting somatostatin analogue (SMS 201-995) in controlling a significant ileal output in a 5-year-old child.

We report the efficacy of a long-acting somatostatin analogue, associated with conventional therapy, in controlling profuse ileostomy losses in a child with short bowel syndrome following a volvulus. The first therapeutic effects of the treatment [50 to 100 micrograms/day of SMS 201-995 (Sandoz Ltd.) subcutaneously] appeared 48 h after institution. Ileal output was reduced on an average from 1,800 to 600 ml. The transit time to the ileostomy was prolonged from 20 to 360 min. The loss of chloride and sodium was reduced. Clinical tolerance was good. This treatment allowed rapid weaning of parent…

Postoperative intra-abdominal and pelvic sepsis complicating ileal pouch-anal anastomosis.

In a series of 500 patients who underwent ileal pouch-anal anastomosis for ulcerative colitis or polyposis coli, significant intra-abdominal or pelvic sepsis developed in 30 (6%). Among the patients who did not require laparotomy because they responded to treatment with antibiotics or local drainage (surgical or radiologically guided) or both, no pouches were excised and the ileostomy closure rate (92%) was similar to that for the patients who did not have sepsis. The 17 patients whose sepsis did require laparotomy had a high rate of pouch excision (41%) (p less than 0.0001) and a low rate of …
