Despite significant progress in our understanding of the pathogenesis and etiology of periodontal diseases, the nature and contribution of the immune system to this disorder remains unclear. Several studies provide evidence for either a protective or destructive rôle. These conflicting findings are difficult to reconcile, since most interpretations tend to argue for a static contributory rôle (i.e., either protective or destructive) of the immune system. Current theories on the rôle of the immune response do not address these conflicting findings as well as the contradictory observation of a d

Immunological tolerance loss is fundamental to the development of autoimmunity; however, the underlying mechanisms remain elusive. Immune tolerance consists of central and peripheral tolerance. Central tolerance, which occurs in the thymus for T cells and bone marrow for B cells, is the primary way that the immune system discriminates self from non-self. Peripheral tolerance, which occurs in tissues and lymph nodes after lymphocyte maturation, controls self-reactive immune cells and prevents over-reactive immune responses to various environment factors. Loss of tolerance results in autoimmune …

PURPOSE: This review aims to describe the key mechanisms behind ocular immune privilege and how disruption of this immunological balance contributes to ocular disease.
METHODS: A narrative review of the literature was performed focusing on ocular immune privilege and diseases in which these mechanisms are altered. The review considered evidence related to corneal transplantation, peripheral ulcerative keratitis, age-related macular degeneration, autoimmune uveitis, diabetic retinopathy, ocular tumors, and allergic conjunctivitis.
RESULTS: The reviewed literature demonstrates how ocular immun

This paper reviews effects of various components of cigarette smoke on the immune system. Nicotine is a major immunosupressive component in cigarette smoke. While acute effects of cigarette smoke on the immune system are less clear, chronic exposure to cigarette smoke causes alterations in humoral and cellular immunity. This study has revealed a communication between the immune system and the central nervous system (CNS) through nicotinic receptors in the brain. In clinical practice there is a great interest in effects of smoking on the immune system in patients with tuberculosis. Studies have

Endotoxin tolerance represents a safeguard mechanism for preventing detrimental prolonged inflammation and exaggerated immune/inflammatory responses from innate immune cells to recurrent harmless pathogens. On the other hand, excessive immune tolerance can contribute to pathological immunosuppression, e.g., as present in sepsis. Monocyte activation is accompanied by intracellular metabolic rearrangements that are reportedly orchestrated by the metabolic signaling node mTORC1. mTORC1-dependent metabolic re-wiring plays a major role in monocyte/macrophage polarization, but whether mTORC1 partici…

Central and peripheral tolerance are key to maintain immune homeostasis. Imbalance of these processes often leads to diseases such as allerg … and generating allergen-neutralizing antibodies. However, the precise mechanisms of how peripheral immune tolerance is effectively maintained in healthy people, but not in allergic patients are still not well understood. Immune checkpoints have recently been proposed as critical molecular pathways across diseases for understanding how the immune system maintains …

Rheumatoid arthritis (RA) involves a breakdown of immune tolerance to citrullinated proteins, leading to chronic inflammation and joint damage. Despite advances in treatment, achieving long-term remission remains a major challenge. Restoring immune tolerance to citrullinated proteins represents a promising strategy to halt disease progression and establish lasting remission. This review examines the potential of using citrullinated proteins or peptides to reestablish immune tolerance in RA. It explores the potential role of anti-citrullinated protein antibodies (ACPAs) in disease pathology and…

Oral tolerance is a method of downregulating an immune response by feeding antigens. The use of oral tolerance toward adenoviruses and colitis-extracted proteins for long term gene therapy and alleviation of experimental colitis, and the mechanisms of tolerance induction are presented. Adenoviruses are efficient vectors in liver-directed gene therapy; however, the antiviral immune response precludes the ability to achieve long term gene expression and prohibits the ability to reinject the recombinant virus. Oral tolerance induction via feeding of viral-extracted proteins prevented the antiaden

Autoimmune diseases arise from a breakdown in immune tolerance. This leads to self-reactive immune responses and chronic inflammation. Induction of antigen-specific tolerance offers a targeted approach that restores immune balance without global immunosuppression. We discuss current approaches to therapies that seek to induce tolerance. These include peptide-, immunoglobulin-, carrier-, and cell-based approaches. We review clinical trials and preclinical studies that explore novel strategies for the induction of durable (antigen-specific) tolerance as possible interventions in autoimmune disea…

Pregnancy represents an immunological paradox where the maternal immune system must tolerate the semi-allogeneic fetus expressing paternally-derived Ags. Accumulating evidence over decades has revealed that successful pregnancy requires the active development of robust immune tolerance mechanisms. This review outlines the multi-layered processes that establish fetomaternal tolerance, including the physical barrier of the placenta, restricted chemokine-mediated leukocyte trafficking, lack of sufficient alloantigen presentation, the presence of immunosuppressive regulatory T cells and tolerogeni…

PURPOSE OF REVIEW: To describe the most recent progress towards tolerance in xenotransplantation.
RECENT FINDINGS: Mixed chimerism and thymic transplantation have been used to promote tolerance in xenotransplantation models. Intra-bone bone marrow transplantation is a recent advance for mixed chimerism, which promotes longer lasting chimerism and early graft function of subsequent organ transplantation. The hybrid thymus, an advancement to the vascularized thymokidney and vascularized thymic lobe, is being developed to allow for both donor and recipient T-cell selection in the chimeric thymus

The liver is a central immunological organ with a high exposure to circulating antigens and endotoxins from the gut microbiota, particularly enriched for innate immune cells (macrophages, innate lymphoid cells, mucosal-associated invariant T (MAIT) cells). In homeostasis, many mechanisms ensure suppression of immune responses, resulting in tolerance. Tolerance is also relevant for chronic persistence of hepatotropic viruses or allograft acceptance after liver transplantation. The liver can rapidly activate immunity in response to infections or tissue damage. Depending on the underlying liver d

Rheumatoid arthritis (RA) is an autoimmune-related disorder characterized by chronic inflammation. Although the etiopathogenesis of RA still remains to be clarified, it is supposed that the breakdown of immune self-tolerance may contribute to the development of RA. Thus, restoring of immune tolerance at the site of inflammation is the ultimate goal of RA treatment. Regulatory T cells (Treg cells) are the main suppressive cells that maintain tolerance and inhibit immunity against auto-antigen. Of note, recent studies demonstrated the efficacy of adoptive transfer of Treg cells in the modulation…

Minimal change nephropathy (MCN) has been associated with a wide variety of abnormalities in the immune response of affected individuals. Many of these may be a consequence of the renal lesion, rather than a cause thereof. This article reviews some of these immunological phenomena and concentrates in particular on the imbalance between type 1 and type 2 cytokine responses, suggesting that there are plausible mechanisms whereby a type 2 cytokine bias could directly or indirectly cause MCN.
