Musculoskeletal and Rheumatic Diseases Induced by Immune Checkpoint Inhibitors: A Review of the Literature.

BACKGROUND: Immune checkpoint inhibitors are a new promising class of antitumor drugs that have been associated with a number of immune-related Adverse Events (AEs), including musculoskeletal and rheumatic disease. METHODS: We searched Medline reviewing reports of musculoskeletal and rheumatic AEs induced by immune checkpoint inhibitors. RESULTS: Several musculoskeletal and rheumatic AEs associated with immune checkpoint inhibitors treatment are reported in the literature. In particular, arthralgia and myalgia were the most common reported AEs, whereas the prevalence of arthritis, myositis a

The latest developments in synthetic peptides with immunoregulatory activities.

In the past few years, many researches have provided us with much data demonstrating the abilities of synthetic peptides to impact immune response in vitro and in vivo. These peptides were designed according to the structure of some important protein molecules which play a key role in immune response, so they act with specific targets. The class I and II MHC-derived peptides inhibit the TCR recognition of antigen peptide-MHC complex. Rationally designed CD80 and CD154-binding peptides block the interaction between cell surface costimulatory molecules on antigen-presenting cells (APCs) and T ce

Advances in clinical outcomes: What we have learned during the COVID-19 pandemic.

Our understanding of risk factors and interventions influencing outcomes from coronavirus disease 2019 (COVID-19) has continued to evolve, revealing advances emerging from hypotheses formed at the start of the pandemic. Epidemiologic studies have shown that asthma control, rather than a diagnosis of asthma, is a determinant of COVID-19 severity. Clinical outcomes in patients with primary immunodeficiencies, even in those with impaired cellular immunity, are variable. IL-6 has emerged as a reliable biomarker of COVID-19 severity, and large clinical trials have shown the potential for improving

Gut microbiota-host interactions and juvenile idiopathic arthritis.

BACKGROUND: Juvenile idiopathic arthritis is the most common form of chronic arthritis in children. There is mounting evidence that the microbiota may influence the disease. MAIN BODY: Recent observations in several systemic inflammatory diseases including JIA have indicated that abnormalities in the contents of the microbiota may be factors in disease pathogenesis, while other studies in turn have shown that environmental factors impacting the composition of the microbiota, such as delivery mode and early exposure to antibiotics, affect the risk of chronic inflammatory diseases including JIA

Effect of age at onset on disease characteristics in vitiligo.

BACKGROUND: Vitiligo is a multifactorial disease in which genetic, immunologic, and environmental factors play an important part. Late-onset vitiligo is a poorly defined entity. MATERIALS AND METHODS: Case records of patients who attended the pigmentary clinic at our institute from January 2001 to December 2010 were reviewed. Patients with a diagnosis of vitiligo were analyzed with respect to their demographic characteristics with special reference to their age at onset. RESULTS: Patients with disease onset after 30 years had a significantly higher association with precipitating factors such

[Complications of dental infections].

Infectious dental complications are quite frequent given the high incidence of tooth decay. Other pathologies not related to dental decay can cause mouth infectious such as periodontal infections, alveolitis, peri-coronitis of impacted wisdom tooth and secondary infection of dental fragments. Dental infection can be localised to apex (top of the root) or progress to soft and bony tissues surrounding the teeth. The most frequent germs involved in these infections are generally the same as saprophyte buccal flora but are often associated to other anaerobic germs. The following complications will

[Food allergies/prevention and diagnosis].

Both immunological (antibody-mediated) and non-immunological mechanisms can trigger allergic reactions and intolerance phenomena to food. A decisive contribution to the genesis of a food allergy is the interaction of "external" factors such as early ingestion of antigens, no breast-feeding, enteritis, and "internal" ones such as genetic influences, a family history of allergies. Correct diagnosis is guided and determined by anamnestic information, identification of characteristic signs and symptoms after ingestion of a particular foodstuff (allergen elimination or provocation), skin test, rele

[Preliminary data on the study of various immunologic parameters in hemophilic children].

Treatment of coagulation deficiencies with high dosages of the missing factor is a source of continuous diffusion of homologous proteins that could modify the normal immunological profile. We have performed immunologic studies on 24 children with ages ranging from 1 - 16 years with severe classical hemophilia and 1 child with von Willebrand's and on a control of 19 age-matched healthy children. Our preliminary data show no statistically significant alterations in the basic immunological profile between normal children and those with hemophilia. Nor were there any differences among the hemophil

Thyroiditis: current views of pathogenesis.

From indirect and circumstantial evidence it would appear that subacute thyroiditis is probably a viral infection of the thyroid gland. The thyroid may respond in the stereotyped manner to any one of a number of viruses. Despite the available evidence, however, the viral theory has yet to be finally proven. Autoimmunity does not appear to be a primary factor in the initiation of subacute thyroiditis, although transient immunologic responses occur as a secondary phenomenon. Autoimmune thyroiditis, on the other hand, is almost certainly a primary immunologic disease, and likely is due to a speci

Application of Topical Immunotherapy in the Treatment of Alopecia Areata: A Review and Update.

Treatment of extensive or recalcitrant alopecia areata (AA) is a major clinical challenge. Even after thorough investigation of several medications, its treatment outcomes have remained unsatisfactory. While there is no US Food and Drug Administration-approved medication for AA yet, topical immunotherapy has been a well-documented treatment option. Dinitrochlorobenzene, squaric acid dibutylester, and diphenylcyclopropenone are three substances that have demonstrated efficacy in the treatment of extensive or recalcitrant AA. Despite being commonly used, the mechanism underlying topical immunoth

Heat-labile enterotoxins as adjuvants or anti-inflammatory agents.

Escherichia coli and Vibrio cholerae produce structurally related AB5-type heat-labile enterotoxins, which are classified into two major types. The Type I subfamily includes cholera toxin and E. coli LT-I, whereas the Type II subfamily comprises LT-IIa and LT-IIb. In addition to their roles in microbial pathogenesis, the enterotoxins are widely and intensively studied for their exceptionally strong adjuvant and immunomodulatory activities, which are not necessarily dependent upon their abilities to elevate intracellular cAMP levels. Despite general structural similarities, these molecules, in

The patient: Emerging clinical applications of intravenous immunoglobulin.

Intravenous immunoglobulin (IGIV) originally was used as prophylactic treatment of infections in patients with primary immunodeficiency disease. Today, administration of IGIV, due in large part to its immunomodulatory activity, has expanded to include a number of other disorders. Available data suggest that the accepted indications for IGIV will continue to expand. As the number of clinical applications for this therapy grows, so will market opportunities; current preparations will be modified and improved and new products introduced. Intravenous immunoglobulin therapy has improved the lives o

Complete Spontaneous Regression of Squamous-Cell Lung Cancer: A Case Report.

Spontaneous regression of cancer is an extremely rare phenomenon, and it has been described in only a few cases of pulmonary cancer. We report a case of complete spontaneous regression of squamous-cell lung cancer (SCLC) following a core needle biopsy in a 67-year-old female patient with two previous lung cancers and concomitant follicular lymphoma. The patient was diagnosed with SCLC after 4 core needle biopsies from a nodule in the left upper lobe and at the same time suspected of having follicular lymphoma. Treatment for the lung cancer was delayed by approximately 8 weeks because the diagn

Immunological Aspects of Chronic Rhinosinusitis.

Chronic rhinosinusitis (CRS) is related to persistent inflammation with a dysfunctional relationship between environmental agents and the host immune system. Disturbances in the functioning of the sinus mucosa lead to common clinical symptoms. The major processes involved in the pathogenesis of CRS include airway epithelial dysfunctions that are influenced by external and host-derived factors which activate multiple immunological mechanisms. The molecular bases for CRS remain unclear, although some factors commonly correspond to the disease: bacterial, fungal and viral infections, comorbidity

Constitutive production of interleukin-6 and immunologic features in cardiac myxomas.

The constitutive production of interleukin-6 (IL-6), a potent hepatocyte-stimulating factor and B cell-differentiating factor, was demonstrated in 3 patients with cardiac myxomas. Tumor cells from the only patient who presented with immunologic features produced 14-23-fold higher levels of IL-6 than those from the 2 patients who lacked such features. A significant serum IL-6 level (56 pg/ml), greater than that observed in patients with active rheumatoid arthritis, was also observed only in this patient, with a subsequent return to an undetectable level after surgical removal of the tumor. This

Elective conversion from cyclosporin to azathioprine in living related donor kidney transplants.

Thirty-five patients with living related donor kidney transplants who were initially immunosuppressed with cyclosporin and a small dose of prednisolone were electively converted to receive azathioprine. Conversion was carried out 6 months after initiation of cyclosporin therapy. Cyclosporin was initially given as part of a random study in 13 cases (group I), and due to the presence of immunological high-risk factor(s) or to control steroid-resistant rejection episodes in 22 cases (group II). In group I, three cases developed acute rejection symptoms which were easily controlled. In group II, c

Immunological basis and immunotherapy of nasopharyngeal carcinoma.

Clinically, the prognosis of nasopharyngeal carcinoma is very poor. It is considered that this depends on three factors. The first is the difficulty of early detection of this disease, because the symptoms of this cancer are latent. The second factor is the specificity of the histological character. The third is most important. This cancer cases fall into the extreme failure category of the immuno-surveillance mechanism. Immuno-responsiveness is extremely depressed. Immunological status has been examined and the clinical evaluation of immunotherapy with OK-432 (streptococcal preparation) and l

[Herpes gestationis: immunologic findings].

The treatment of herpes gestationis has been considerably changed in recent years by progress in electron microscopy and immunology. Herpes gestationis, a rare polymorphic vesiculo-bullous pruriginous dermatosis, occurs during pregnancy or the post-partum period. It evolves by acute periods of several weeks, and relapses during later pregnancies or on taking oestro-progestagens. Histologically, the bulla is situated at the dermo-epidermic junction. The epidermal oedema is both intra and extra-cellular. The ultrastructural studies show the importance of the oedema and the changes in the plasma

The Protective Effect of Abortion on Preeclampsia: An Analysis of Current Research.

A review of the current literature on preeclampsia (PE) confirms that this pregnancy complication remains a common cause of maternal mortality. Within the last several decades, obstetric and gynecological researchers worldwide have indicated an association between prior abortions and the development of PE. Different studies have debated whether abortion is a protective or risk factor for PE. However, the most current literature demonstrates a stronger likelihood that a positive history of abortions will offer a protective effect against PE. This association has been supported by advancements i

Neuroinflammation in Alzheimer's disease.

Increasing evidence suggests that Alzheimer's disease pathogenesis is not restricted to the neuronal compartment, but includes strong interactions with immunological mechanisms in the brain. Misfolded and aggregated proteins bind to pattern recognition receptors on microglia and astroglia, and trigger an innate immune response characterised by release of inflammatory mediators, which contribute to disease progression and severity. Genome-wide analysis suggests that several genes that increase the risk for sporadic Alzheimer's disease encode factors that regulate glial clearance of misfolded pr
