The concept of a blood brain or blood tumour barrier blocking the passage of lipid insoluble cytoreductive drugs from blood into brain-tumor, leads to a protocol of intraarterial injection of high dose methotrexate (MTX) during reversible, osmotic blood brain barrier disruption (BBBD). Malignant Gliomas of grades III and IV in 9 patients and one primary CNS-lymphoma in one patient have been treated in 2 to 5 sessions per patient with BBBD plus polychemotherapy (MTX, cyclophosphamide, procarbazine). Acute cranial neuropathy followed intra-arterial chemotherapy with Cisplatin for a squamous cell cancer of the mouth. The favourable course, histological studies and local trophic impairment suggested local neurotoxicity due to Cisplatin accumulation in cranial nerves. The patient was the first to develop such a toxic effect in a series of 35 consecutive patients treated with the same therapy.

The intra-arterial (IA) model by microcatheter administration was an effective way to deliver drugs or cells to the brain. All of these models were carried out introduced in rat rather than mice for the difficult and technically challenging due to their small caliber. In 2014, Alejandro Santillan first introduced this model in mice and we found that most of the operational steps were similar with the middle cerebral artery occlusion and reperfusion (MCAO/R) model. A new microcatheter-arterial port system for transarterial regional therapy was evaluated in a swine model. Subclavian or femoral access was followed by superselective catheterization of target arteries (hepatic, n = 6; splenic, n = 3). A hemostatic sleeve was placed at the arterial puncture site. Implantation into the target vessel succeeded in all nine placements (100%). After animal movement, no microcatheter tip moved by more than 10 mm. A small hematoma developed at one of the subclavian insertion sites. This system holds promise for transarterial therapies for regional cancer therapy.

Targeting tumors with proteosome inhibitors has demonstrated antitumor activity and has been successfully translated to the clinic for patients with multiple myeloma. However, in patients with solid tumors, treatment with proteosome inhibitors as single agents has consistently failed to yield meaningful responses. In our study, we investigate the potential of hepatic artery infusion pump delivery of carfilzomib, to continuously direct a large dose to the tumor with least hepatic toxicity.
