Between culture and care: nurses' experiences of end-of-life communication in Ghana.

BACKGROUND: Communicating at the end-of-life (EOL) requires deep sensitivity and adherence to strong ethical principles that respect the dignity, rights, and values of patients and their families. However, many nurses find this task emotionally challenging, especially in settings with limited training and strong cultural taboos about death. In Ghana, little is known about how nurses experience and manage these conversations in everyday clinical practice. This study explored nurses’ experiences communicating with families at the end of life. METHODS: This study adopted an interpretivist qualita

Outcomes of Hospitalizations for Cardiogenic Shock at Left Ventricular Assist Device Versus Non-Left Ventricular Assist Device Centers.

Background Cardiogenic shock (CS) is a complex syndrome associated with high morbidity and mortality. In recent years, many US hospitals have formed multidisciplinary shock teams capable of rapid diagnosis and triage. Because of preexisting collaborative systems of care, hospitals with left ventricular assist device (LVAD) programs may also represent "centers of excellence" for CS care. However, the outcomes of patients with CS at LVAD centers have not been previously evaluated. Methods and Results Patients with CS were identified in the 2012 to 2014 National Inpatient Sample. Long-term outcomes of isolated liver transplantation for short bowel syndrome and intestinal failure-associated liver disease.

BACKGROUND AND AIM: A select group of children with short bowel syndrome (SBS) and intestinal failure-associated liver disease (IFALD) fulfill the criteria for isolated liver transplantation (iLTx). Long-term results in this group of patients have not been reported. METHODS: A retrospective study of the medical records of 8 survivors of 14 children who underwent iLTx for SBS and IFALD from 1998 to 2005, managed by a multidisciplinary intestinal rehabilitation team at our institution. RESULTS: Median follow-up is 107.5 months (range 89-153 months). Five of 8 children were weaned from parenter

Surgical Experience and Literature Review of Sellar and Suprasellar Arachnoid Cysts: Endoscopic Management and Surgical Anatomy-Based Reorganization.

BACKGROUND: The suprasellar cistern containing major neurovascular structures is accessible by various microscopic and endoscopic approaches. While literature strengthening the case for skull-base endoscopic management of neoplastic sellar-suprasellar lesions abounds, a straightforward algorithm for endoscopically managing sellar/suprasellar arachnoid cysts (SAC) has not been set forth. This is likely due to their low incidence and the wide variation in both nomenclature and surgical management described in the literature. METHODS: All (10) cases of SAC managed at our institution over the pas

Surgical management of metastatic adrenocortical carcinoma: Is there a role for multivisceral resection?

BACKGROUND: Recent data suggest that cytoreductive surgery in metastatic adrenocortical carcinoma) may improve survival. However, successful removal of locally invasive primary tumors in such patients may require complex en bloc and/or multivisceral resection, for which the survival benefits remain unclear. METHODS: We retrospectively analyzed 153 patients with metastatic adrenocortical carcinoma treated at our institution from 1998 to 2024. Patients were categorized into 3 groups based on treatment approach: multivisceral resection, adrenalectomy alone, and nonoperative management. Kaplan-Me

Pain management after pediatric minimally invasive repair of pectus excavatum: a narrative review.

BACKGROUND AND OBJECTIVE: Pectus excavatum is a common congenital chest wall abnormality characterized by a concave appearance of the chest, and minimally invasive repair of pectus excavatum (MIRPE) is the surgical treatment of choice. A rapidly growing field of research is pain management in children undergoing MIRPE, with many shifts in practice occurring over the last decade. The primary objectives of this narrative review are to describe current methods of perioperative pain management and the development of enhanced recovery after surgery (ERAS) to improve the experience of patients under

Self-Reported Obesity Medicine Competence, Knowledge, and Attitudes Among US Medical Students: An Eight-School Pre- vs. Post-Intervention Study.

BACKGROUND: Obesity contributes to many chronic diseases that internists manage. Even though there is a documented gap in obesity education, no studies have evaluated competency-based obesity education interventions from the learner perspective. AIM: To develop and implement a pilot, multi-institutional, competency-based obesity medicine curriculum intervention, with the learner perspective as the primary outcome. PROGRAM DESCRIPTION: We developed and implemented an educational intervention grounded in social constructivist learning theory and guided by the logic model framework. Students pr

Training clinicians to be organizers: Expanding professional identities through a year-long Climate Health Organizing Fellowship.

The climate crisis and its harmful effects on human health are staggering. It is motivating clinicians to transform their scope of practice. Physicians and other clinicians are on the front lines of managing a growing surge of illness and injury directly caused by increasingly extreme temperatures, weather, and ecological transformation. But the joint institutional constraints of clinician socialization through training and the labor conditions of health care delivery tend to obfuscate and disincentivize the array of possible practical strategies for meaningful, collective intervention in the

Suicidal ideation in residency: recommendations for building a supportive learning environment.

Although overall suicide risk for physicians may be decreasing from historical estimates, each physician suicide is a tragic loss of life for the affected individual and their family and has a profound impact on their colleagues, their team, and their organization. Work-related problems appear to be more common prior to instances of physician suicide relative to suicide occurring in other workers, and Nasca and colleagues' study, "One Is Too Many: Suicidality Among General Surgery Residents," elucidates several factors that were associated with increased risk for suicidal ideation in the previ
