Should all babies with oesophageal atresia have routine screening for midgut malrotation anomalies? A systematic review in search of evidence.

Background/Purpose Oesophageal Atresia (OA) is associated with co-existent anomalies. There is a controversy of literature pertaining to the risk (s) of intestinal malrotation. In order to guide management we critically evaluate the incidence of IM anomalies in OA newborns. Design MEDLINE and EMBASE databases were searched using keywords "(O)Esophageal Atresia and Malrotation/Associated Abnormalities/Associated Anomalies". Full texts of articles were screened if manuscripts exclusively reported patients with OA malrotation and/or associated anomalies. Larger case series (> 10patients) were inc

The Role of Laparoscopy in the Acute Neonatal Abdomen.

IntroductionThe surgical treatment of the acute neonatal abdomen still poses a challenge in pediatric surgery. Various underlying etiologies … acute neonatal abdomen and enables immediate surgical treatment of acute volvulus, appendicitis, or intestinal atresia. In case of conversion to laparotomy, precise localization of the incision is guaranteed. Minimization of the surgical trauma and avoidance of unnecessary laparotomy are the most important benefits of the minimal-invasive approach for the critically …

[History of organ transplantation in the field of pediatric surgery in Japan].

In Japan, liver transplantation was first attempted 50 years ago, around the same time as the development of pediatric surgery. In 1989, clinical liver transplantation in Japan started with a living related-donor transplantation in a boy with biliary atresia. In the early years, the majority of recipients were children worldwide, which is why pediatric surgeons played a major role in the establishment of liver transplantation in Japan. From 1998, most of the indications for pediatric patients needing liver transplantation have been covered by governmental health insurance. Since that year, the

A nordic multicenter study on contemporary outcomes of pediatric short bowel syndrome in 208 patients.

BACKGROUND & AIMS: Despite advances in the management of short bowel syndrome related intestinal failure (SBS-IF), large-scale contemporary pediatric studies are scarce. The aim of this multicenter study was to assess key outcomes and clinical prognostic factors in a recent Nordic pediatric SBS-IF population. METHODS: Patients with SBS-IF treated during 2010-2019, whose parenteral support (PS) started at age <1 year and continued >60 consecutive days were included and retrospectively reviewed. All six participating centers followed multidisciplinary SBS-IF management. Risk factors for PS depe

A new variant of MYCN gene as a cause of Feingold syndrome.

Feingold syndrome 1 (FS1) is a rare disorder that is inherited in autosomal dominant manner with full penetrance but with variable expressivity. The most common phenotypical features described are finger and toe anomalies, microcephaly, short stature, and intestinal atresia. Dysmorphic features, intellectual disability and other organ anomalies are less frequently described. Here, we present a 7-year-old boy with severe intellectual disability who is diagnosed with FS1 syndrome caused by a new heterozygous variant of MYCN gene.

More Distally Located Duodenal Webs: A Case Series.

Duodenal atresia is a frequent cause of intestinal obstruction in the newborn. Obstruction due to duodenal web is infrequent, but its location other than second part of duodenum is rare with only a few cases reported in the literature. We are reporting three patients where we found duodenal webs at unusual locations. In one neonate the web was located at third part of duodenum and in other two patients the web was present at duodeno-jejunal junction (DJ).

Gastrointestinal stromal tumor, jejunal atresia and stenosis in a neonate.

Gastrointestinal stromal tumors could rise in different areas of the digestive tract, at any age, but very rarely in neonates. We present the case of a 5-day-old male, with intestinal stenosis and atresia (type II) operated for peritonitis. On the resected specimen, the histopathological examination revealed a small gastrointestinal tumor of 8 mm. The immunohistochemical analysis indicated a low malignant potential. He is currently at two years of oncologic follow-up with no evidence of disease.

The surgery of malrotation and midgut volvulus: a nine year experience in neonates.

Thirty four neonates presented with acute duodenal obstruction due to malrotation during a 9 year period between 1973 and the end of 1981. Of these, 20 patients (58.8%) presented in the first week of life, and 24 (70.5%) had an associated midgut volvulus. This frequent association is stressed as bowel necrosis occurs very rapidly. Massive gangrene of small bowel was present in 5 patients, extensive resection was necessary in 3 patients, of whom 2 died postoperatively (5.8%). Five patients required reoperation and 10 had additional G.I. malformations (29.4%).

Case series of selenium insufficiency among infants with intestinal failure.

BACKGROUND: Selenium (Se) is an essential trace element incorporated into 'selenoenzymes' that are necessary for immune, thyroid and antioxidant responses. Infants with intestinal failure (IF) require prolonged administration of parenteral nutrition (PN). This report presents a series of infants with IF who demonstrate biochemical Se deficiency despite PN supplementation, which normalized following dose escalation. CASE SERIES: Patient 1 was born at 25 weeks' gestation, birthweight 870 g, with intestinal malrotation and colonic atresia. Patient 2 was born at 28 weeks' gestation, birthweight 1

Congenital or acquired? Obstructive jaundice in reoperated duodenal atresia.

A 55-day-old boy was transferred to our unit with intestinal obstruction and obstructive jaundice after two neonatal operations for duodenal atresia and intestinal malrotation. Abdominal ultrasound showed dilated intrahepatic and extrahepatic ducts with cut-off at the distal common bile duct (CBD). He underwent emergency laparotomy for adhesive intestinal obstruction with a contained abscess from mid-jejunal perforation. Biliary dissection was not attempted due to poor preoperative nutritional status. Tube cholecystostomy was created for biliary decompression. Postoperative magnetic resonance

Bronchopulmonary foregut malformation in 3 infants--with special references to cases in childhood.

We encountered three infant patients with bronchopulmonary foregut malformation. Two of them were associated with oesophageal atresia, and the remaining one with jejunal atresia. Two of the patients showed total lung sequestration, and the rest showed intralobar sequestration. 37 paediatric cases of this type were reviewed from the literature. Total lung sequestration was observed in 19 of the 37 cases, showing an increase in the incidence in comparison with that reported previously. There were also 7 cases associated with oesophageal atresia.

Congenital Jejunal Web with Central Aperture in Children: Report of Two Cases of Delayed Diagnosis and Management.

Congenital intestinal web of the jejunum is an exceptionally rare condition. While most intestinal atresias present during the neonatal period, jejunal webs with central apertures can lead to delayed presentations, often causing diagnostic challenges. We report two cases of jejunal web in children diagnosed beyond the neonatal period. Both patients presented with failure to thrive and bilious vomiting. One patient, a 2-year-3-month-old girl, was diagnosed with a single jejunal web and underwent successful web excision. The other, a 2-year-8-month-old girl, was found to have double jejunal webs…
