Actinomycosis: an unusual cause of unresectable rectal cancer.

We present a very unusual case of a woman with an intrauterine contraceptive device (IUCD) who developed pelvic actinomycosis during long course chemoradiotherapy for rectal cancer, which presented a significant diagnostic challenge and eventually rendered the cancer unresectable. IUCDs are often implicated in the development of pelvic actinomycosis but there is no current evidence or guideline to suggest that they should be removed prior to oncological treatment. Owing to the devastating consequences of this combination of disease, we suggest that it may be prudent to remove IUCDs in this set

Clinical features and antimicrobial susceptibility trends in Citrobacter freundii bacteremia.

This retrospective study investigated the clinical characteristics and antimicrobial susceptibilities of 36 cases of Citrobacter freundii bacteremia treated at the Taipei Veterans General Hospital from 1996 through 1999. The results showed that the predominant infection site was the intraabdominal region and the mortality was 22%. The resistance of C. freundii to most third-generation cephalosporins and broad-spectrum penicillins increased in both nosocomial and community-acquired C. freundii bacteremia during the study period, and the strategy of using a combination of antimicrobial agents to

Antibiotic de-escalation in severe intra-abdominal infections: a single-center retrospective cohort study on safety and prognostic factors.

BACKGROUND: Antibiotic de-escalation is a key antimicrobial stewardship strategy aimed at reducing the ecological and adverse effects of broad-spectrum therapy without compromising efficacy. However, its real-world safety and impact on clinical outcomes in patients with severe intra-abdominal infections (IAIs) remain underexplored. METHODS: A single-center, retrospective cohort study was conducted from June 2022 to June 2025. Adults with severe IAIs who received initial broad-spectrum antibiotics and achieved source control within 48 h were included. Patients were stratified into de-escalatio

Analytical performance of species-targeted quantitative PCR for intra-abdominal candidiasis in critically ill patients: a proof-of-concept post hoc analysis from the pBDG2 multicenter study.

BACKGROUND: Intra-abdominal candidiasis (IAC) is a severe form of invasive candidiasis that affects critically ill patients and is associated with a high mortality rate and persistent diagnostic delays. Conventional fungal culture from peritoneal fluid remains slow and has insufficient sensitivity. Molecular assays promote early diagnosis, but data from critically ill populations remain scarce. OBJECTIVES: We evaluated the diagnostic performance of the OLM CandID Real-Time PCR assay directly applied to peritoneal fluid samples obtained from a previously established multicenter cohort of critic

Imipenem and relebactam pharmacokinetics in peritoneal fluid: microdialysis reveals infection-independent penetration.

Imipenem/relebactam is a promising therapy for complicated intra-abdominal infections caused by carbapenem-resistant Enterobacterales species. However, data on tissue-site pharmacokinetics are limited. We evaluated the peritoneal penetration of imipenem and relebactam in healthy and infected rats by using microdialysis. The animals received a single intravenous infusion of imipenem (45 mg/kg) and relebactam (22.5 mg/kg), and unbound concentrations were dynamically monitored in the plasma and peritoneal fluid. Imipenem exposure in the peritoneal fluid was consistently lower than that in the pla

Establishment and Validation of a Machine-Learning Prediction Nomogram Based on Lymphocyte Subtyping for Intra-Abdominal Candidiasis in Septic Patients.

This study aimed to develop and validate a nomogram based on lymphocyte subtyping and clinical factors for the early and rapid prediction of Intra-abdominal candidiasis (IAC) in septic patients. A prospective cohort study of 633 consecutive patients diagnosed with sepsis and intra-abdominal infection (IAI) was performed. We assessed the clinical characteristics and lymphocyte subsets at the onset of IAI. A machine-learning random forest model was used to select important variables, and multivariate logistic regression was used to analyze the factors influencing IAC. A nomogram model was constr

A Systematic Review of Pericolic Extraluminal Air in Left-Sided Acute Colonic Diverticulitis.

BACKGROUND: Although approximately 15% of all patients with acute colonic diverticulitis have pericolic extraluminal air found on computed tomography (CT), studies on the natural course and treatment of these patients are scarce. It is not clear whether these patients behave as mild uncomplicated diverticulitis or as severe diverticulitis needing more aggressive treatment. Consequently, there is no consensus about the treatment, guidelines do not discuss this group of patients, and physicians treat these patients based on their own experiences and opinion. An evidence-based approach is needed

Recent developments in the surgical management of complex intra-abdominal infection.

BACKGROUND: Current guidance on the management of sepsis often applies to infection originating from abdominal or pelvic sources, which presents specific challenges and opportunities for efficient and rapid source control. Advances made in the past decade are presented in this article. METHODS: A qualitative systematic review was undertaken by searching standard literature databases for English-language studies presenting original data on the clinical management of abdominal and pelvic complex infection in adults over the past 10 years. High-quality studies relevant to five topical themes tha

Efficacy and safety of intravenous azithromycin followed by oral azithromycin for the treatment of acute pelvic inflammatory disease and perihepatitis in Japanese women.

UNLABELLED: Pelvic inflammatory disease (PID) is mainly caused by ascending infection from the vaginal flora including the sexually transmitted organisms, Neisseria gonorrhoeae and Chlamydia trachomatis, and lower genital tract endogenous anaerobes, leading to serious consequences including infertility and ectopic pregnancy. To evaluate the efficacy and safety of azithromycin in the treatment of PID that requires initial intravenous therapy, we conducted a multicenter, unblinded, non-comparative phase 3 trial. Intravenous azithromycin (500 mg, once daily) for 1 or 2 days followed by oral azith

[Epidemiological and microbiological evaluation of nosocomial infections caused by Candida species].

Nosocomial infections due to candida species are increasingly gaining importance. In this study, we aimed to determine the risk factors associated with nosocomial candida infections and also to determine the species and antifungal sensitivity of candida strains. Candida spp. strains isolated from the clinical specimens of 92 patients who were hospitalized in Ankara Research and Training Hospital between May 2008 and September 2009 were prospectively included in the study. The demographic features, clinical and laboratory findings of the patients were recorded. Identification and antifungal sen

Financial and Clinical Toxicity of Empiric Vancomycin for Intra-Abdominal Infections: A Cohort Study.

Background: Vancomycin for intra-abdominal infections (IAI) should be reserved for healthcare-acquired infections, history of multiple interventions, or methicillin-resistant Staphylococcus aureus (MRSA). The MRSA incidence is low; however, fear of missing MRSA leads to overutilization. Methods: This single-center retrospective cohort study evaluated the cost and risks of empiric vancomycin for IAI. The primary objective was to determine the incidence of MRSA-positive culture and surveillance testing. Secondary outcomes included acute kidney injury (AKI) incidence, progression to dialysis, dir

The aetiologic agents and resistance rates in community-acquired complicated intra-abdominal infections: Where are we locally?

BACKGROUND: Community-acquired complicated intra-abdominal infections (CA-cIAIs) are a significant global health concern, associated with considerable morbidity and mortality. In regions lacking local epidemiological data, obtaining intraoperative cultures is recommended to guide empirical antibiotic selection. This study aimed to describe the local microbiological profile and resistance patterns in CA-cIAIs. METHODS: We retrospectively reviewed adult patients operated for CA-cIAIs at Ankara University Hospital between January 2018 and December 2023. Inclusion criteria included intraoperative

Reduced peripheral CD8+ T cells are associated with sepsis-associated acute kidney injury in intra-abdominal infection: a multicenter cohort and multi-omics analysis.

BACKGROUND: Sepsis-associated acute kidney injury (SA-AKI) is a severe complication of sepsis characterized by high mortality and limited early immune-related biomarkers. Increasing evidence suggests that immune dysregulation contributes substantially to the pathogenesis of SA-AKI; however, the immunological characteristics underlying bacterial intra-abdominal infection (IAI)-associated SA-AKI remain incompletely understood. METHODS: In this study, we investigated alterations in lymphocyte subsets and immune mediators in patients with bacterial IAI-associated sepsis. Clinical and immunologica

Differential Impact of Polymyxin B Hemadsorption on Long-Term Mortality in Septic Shock: A Retrospective Analysis of Intra-Abdominal Versus Extra-Abdominal Infections.

BACKGROUND: The long-term benefit of polymyxin B hemadsorption (PMX-HA) in septic shock patients remains unclear and may depend on the site of infection. We evaluated the association between PMX-HA use and 1-year mortality in patients with intra-abdominal infection (IAI) and extra-abdominal infection (EAI). METHODS: We retrospectively analyzed adult patients with septic shock (Sepsis-3) admitted to an ICU between January 2017 and July 2023. Patients were categorized as IAI or EAI and further stratified by PMX-HA use. One-year mortality was assessed using Kaplan-Meier analysis, multivariable C

Improving Empiric Antibiotic Selection for Patients Hospitalized With Abdominal Infection: The INSPIRE 4 Cluster Randomized Clinical Trial.

IMPORTANCE: Empiric extended-spectrum antibiotics are routinely prescribed for over a million patients hospitalized annually with abdominal infection despite low likelihoods of infection with multidrug-resistant organisms (MDROs). OBJECTIVE: To evaluate whether computerized provider order entry (CPOE) prompts providing patient- and pathogen-specific MDRO infection risk estimates can reduce empiric extended-spectrum antibiotics for non-critically ill patients admitted with abdominal infection. DESIGN, SETTING, AND PARTICIPANTS: This 92-hospital cluster randomized clinical trial assessed the e

Clinical Pharmacokinetics and Pharmacodynamics of Eravacycline.

On 27 August, 2018, the US Food and Drug Administration approved eravacycline, a fluorocycline antimicrobial agent within the tetracycline class of antibacterial drugs, for the treatment of complicated intra-abdominal infections in patients aged 18 years and older. This decision was based on substantial clinical and pre-clinical data, including rigorous pharmacokinetic and pharmacodynamic work. This paper examines the in-vivo pharmacokinetic/pharmacodynamic work that led to the approval of eravacycline and explores how this important new antibiotic may be used to treat aggressive multidrug-res
