A Surgically Resected Pancreatic Schwannoma with Obstructive Jaundice with Special Reference to Differential Diagnosis from Other Cystic Lesions in the Pancreas.

Pancreatic schwannomas are uncommon. About 60% of pancreatic schwannomas develop cystic lesions, and the differential diagnosis from other cystic pancreatic tumors is difficult. A 43-year-old man presented for evaluation of liver dysfunction detected during a medical checkup. Blood testing detected obstructive jaundice. A computed tomography scan revealed a well-defined polycystic tumor of about 5 cm at the pancreatic head. We performed surgical resection to treat the patient's symptoms and facilitate long-term management. Histopathological examination revealed spindle-shaped cells. Immunohist

One-step palliative treatment method for obstructive jaundice caused by unresectable malignancies by percutaneous transhepatic insertion of an expandable metallic stent.

AIM: To describe a simple one-step method involving percutaneous transhepatic insertion of an expandable metal stent (EMS) used in the treatment of obstructive jaundice caused by unresectable malignancies. METHODS: Fourteen patients diagnosed with obstructive jaundice due to unresectable malignancies were included in the study. The malignancies in these patients were a result of very advanced carcinoma or old age. Percutaneous transhepatic cholangiography was performed under ultrasonographic guidance. After a catheter with an inner metallic guide was advanced into the duodenum, an EMS was pla

Exploring the interplay of stigma, family care, and discharge readiness in patients with malignant obstructive jaundice undergoing percutaneous transhepatic cholangial drainage recovery.

OBJECTIVE: To investigate the current status of stigma, family care, and discharge readiness in patients with malignant obstructive jaundice undergoing percutaneous transhepatic cholangial drainage (PTCD) and analyze its influencing factors. METHODS: A cross-sectional survey was conducted on 399 patients with malignant obstructive jaundice who underwent PTCD using a general information questionnaire, the readiness for hospital discharge scale (RHDS), the social impact scale (SIS), and the family APGAR index. RESULTS: The total average stigma score in patients with malignant obstructive jaund

Acute Obstructive Jaundice Secondary to Surgical Clip Migration Into the Common Bile Duct 12 Years Following Laparoscopic Liver Resection: A Case Report.

Surgical clip migration to the common bile duct is a rare late complication, typically originating from clips placed at the cystic duct and most commonly reported after laparoscopic cholecystectomy. We present an exceptionally rare case of obstructive jaundice caused by clip migration from the liver dissection plane, rather than from the cystic duct, occurring 12 years after laparoscopic liver resection (LLR) and cholecystectomy and associated with chronic biliary inflammation. A 73-year-old man underwent LLR of segments 4a + 5 and cholecystectomy for hepatocellular carcinoma and was discharge

Preoperative biliary drainage for patients with malignant obstructive jaundice: an update on the systematic review and model-based Bayesian network meta-analysis.

BACKGROUND: Several procedures are available for preoperative biliary drainage, but the necessity and best clinical choice remain uncertain. The purpose was to compare the efficacy and safety of seven methods of preoperative biliary drainage with different surgical procedures, materials, or places of stents in patients with malignant obstructive jaundice. METHODS: This study included randomized controlled trials, prospective and retrospective trials, and case-control studies focusing on preoperative biliary drainage (PBD). Meta-analysis and Bayesian Model-Based Network Meta-Analysis were used

A single-center retrospective study comparing safety and efficacy of endoscopic biliary stenting only vs. EBS plus nasobiliary drain for obstructive jaundice.

PURPOSE: Biliary drainage is an important modality for extrahepatic obstructive jaundice both in patients with palliative and resectable. Currently, endoscopic biliary drainage is preferred in clinical practice, including endoscopic nasobiliary drainage (ENBD) and endoscopic biliary stenting (EBS), both of which have their own advantages and disadvantages. The purpose of our study was to compare the safety and efficacy of endoscopic biliary stenting (EBS) only vs. EBS plus nasobiliary drain for obstructive jaundice. METHODS: We consecutively reviewed patients with endoscopic biliary drainage

Gallstone ileus inducing obstructive jaundice at the afferent loop of Roux-en-Y hepaticojejunostomy after bile duct cancer surgery: a case report.

The diagnosis of gallstone ileus is occasionally challenging due to the variability of its presentation. We herein present a very rare case of gallstone ileus inducing obstructive jaundice at the afferent loop of Roux-en-Y hepaticojejunostomy after 10 years of bile duct cancer surgery. We describe the case of a 74-year-old Korean woman with obstructive jaundice, treated conservatively. She showed severely impaired liver function test and obstructive jaundice. The computed tomography (CT) scan led to a diagnosis of very rare type of gallstones ileus at the afferent jejunal loop. Since the clini

EUS/EUS-FNA for suspected pancreatic cancer: influence of chronic pancreatitis and clinical presentation with or without obstructive jaundice on performance characteristics.

BACKGROUND: The clinical utility of EUS-FNA is debated in patients with obstructive jaundice (ObJ) because of a very high pretest probability of pancreatobiliary malignancy (PBM) and biliary stent-induced inflammation that can potentially confound EUS-FNA diagnosis. EUS-FNA also has lower accuracy in patients with underlying chronic pancreatitis (CP). OBJECTIVE: Our purpose was to determine the clinical value of EUS-FNA for PBM diagnosis based on clinical presentation and presence of CP. DESIGN: Retrospective analysis of prospective database. SETTING: University hospital. PATIENTS: Patient

A simple procedure for intrahepatic biliary drainage in patients with obstructive jaundice owing to a recurrence of cancer following reconstructive gastric surgery.

We designed a simple and reliable method of intrahepatic biliary drainage for patients with obstructive jaundice owing to a recurrent gastric cancer. This approach can be used for selected patients treated by partial gastrectomy and reconstructive surgery, using the Billroth II procedure and antecolic anastomosis. Anastomosis between the intrahepatic bile duct and the afferent jejunal loop is performed at the anterior edge of the liver, thus facilitating the prevention of recurrence of jaundice caused by tumor infiltration. Application of this technique led to a long-term palliation, control o

[Two cases of obstructive jaundice due to extrahepatic carcinoma of the bile duct with marked response to daily oral administration of etoposide].

Two cases of obstructive jaundice due to cancer of extrahepatic bile duct were treated with a combination chemotherapy combined with etoposide. The first case was a 81-year-old male and the second case was a 62-year-old female who introduced to this hospital because of jaundice and admitted. Cholangiography was performed and diagnosed cancer of the extrahepatic bile duct. Daily oral administration of etoposide (25mg/day) added to other immunochemotherapy was carried out. The cholangiography via PTCD tube revealed a passage of obstruction of the choledochus. These data suggest that a combinatio

Successful Chemotherapy Induction Following Management of Obstructive Jaundice and Femoral Pathological Fracture in Extensive-Stage Small Cell Lung Cancer: A Case Report.

Patients with poor Eastern Cooperative Oncology Group performance status (ECOG PS) ≥ 3 may be considered for systemic chemotherapy when the decline is driven by reversible tumour-related factors, but assessing reversibility is often difficult, and some patients ultimately receive best supportive care. We report the case of a 76-year-old man with extensive-stage small cell lung cancer (SCLC) whose condition declined to PS 3 due to obstructive jaundice from pancreatic metastasis and a painful femoral fracture. Biliary stenting improved hepatic function; however, after the first cycle of carbopla

Polysplenia syndrome with agenesis of the dorsal pancreas and preduodenal portal vein presenting with obstructive jaundice--a case report and literature review.

Polysplenia, as part of the heterotaxy syndrome, is a rare embryological disorder which results from failure of development of the usual left-right asymmetry of organs. It is often associated with cardiac and biliary abnormalities, which are the usual causes of death in early neonatal life. A congenitally short pancreas and abnormalities with portal vein formation, gut malrotations and inferior vena cava anomalies are known to be associated with this rare syndrome. We report a case of polysplenia in an adult female presenting with obstructive jaundice owing to choledocholithiasis, possibly for

[Endoscopic ultrasound-guided transmural cyst drainage (EUS-CD) was effective in a case of pancreatic pseudocyst with obstructive jaundice and duodenal stenosis].

A 40-year-old man was admitted to our hospital because of epigastralgia and vomiting. His condition was diagnosed as acute pancreatitis with a pancreatic pseudocyst, obstructive jaundice, and duodenal stenosis. Because he had fever, abdominal pain, and elevated levels of C-reactive protein (CRP), endoscopic ultrasound-guided transmural cyst drainage (EUS-CD) was performed with a nasocystic tube on the 6th day. After the cyst was reduced and the patient recovered from the obstructive jaundice and duodenal stenosis, the nasal drainage tube was replaced with a plastic stent. Because a short exten

Duodenal somatostatinoma presenting as obstructive jaundice with the coexistence of a gastrointestinal stromal tumour in neurofibromatosis type 1: a case with review of the literature.

Somatostatinomas are rare neuroendocrine tumours, mostly located in the pancreas or duodenum, with an estimated incidence of 1 in 40 million. Duodenal somatostatinomas (DSs) are usually found in association with neurofibromatosis type 1 (NF1), tuberous sclerosis and Von Hippel-Lindau syndrome. Gastrointestinal stromal tumours (GIST) have also been described in NF1, but the association with somatostatinoma is very uncommon. We report the case of a patient with NF1 who presented with obstructive jaundice due to multiple firm nodules around the ampulla of Vater. A pancreaticoduodenectomy was perf

The effects of somatostatin and ursodeoxycholic acid in preventing the ischemic injury of the liver following Pringle maneuver in obstructive jaundice-rat model.

BACKGROUND/AIMS: In our study, the effects of somatostatin (SS) and ursodeoxycholic acid (UDCA) on ischemic liver injury were studied in (obstructive) jaundice-rat model. METHODOLOGY: For this purpose, jaundice was produced in the first four groups by binding of their choleducts. We performed just laparotomy to the other four groups of animals. To groups 1 and 5, SS was given 15 mcg/kg/day intraperitoneally, and to groups 2 and 6, UDCA was given 20 mg/kg/day enterally. No drugs were given to any other group. At the end of one week, a procedure with ischemia of the liver for 60 minutes followe

The effect of communication between the right and left liver on the outcome of surgical drainage for jaundice due to malignant obstruction at the hilus of the liver.

Debate continues regarding the optimal management of irresectable malignant proximal biliary obstruction. Controversy exists concerning the ability of unilateral drainage to provide adequate biliary decompression with tumors that have occluded the communication between the right and left hepatic ductal systems. Between October 1986 and October 1989, 18 patients with malignant proximal biliary obstruction were treated by an intrahepatic biliary enteric bypass. Patients were divided into two groups based on the presence or absence of a communication between the right and left biliary systems. In
