Knee flexion test and sciatica.

Knee flexion of the leg when a patient with sciatica bends to reach the floor is objective evidence of lumbar spinal nerve root compression. Contralateral sciatic scoliotic list usually accompanies the finding, which should be known as a positive knee flexion test for sciatic spinal nerve root compression.

Further pitfalls in knee arthrography.

Peripheral separations of the posterior medial meniscus, diagnosed at arthrography, may not be recognized at arthroscopy, arthrotomy or even upon examination of the resected specimen. Arthrographic recognition of an oblique buckethandle meniscal tear may be difficult when the visualized outer fragment maintains a triangular appearance. Articular cartilage defects, out of profile to the x-ray beam, may be suspected at fluoroscopy and confirmed by additional views. There is an icreased incidence of chondromalacia in symptomatic postmeniscectomy knees. Degenerative meniscal changes without arthro…

Lymphatics filled at knee arthrography.

A review of 587 knee arthrographies revealed filling of lymph vessels in 12 cases. They occurred in connection with arthritis (4 rheumatoid), osteoarthritis, ruptured meniscus, ruptured popliteal cyst and after operation. Lymphatic filling was not related to the leakage of contrast through a ruptured popliteal cyst or to inadvertent extra-articular injection. Filling of lymphatics is more likely to occur in examinations with a positive contrast medium when a larger amount of medium is used.

Discoid knee menisci in children.

Five children were operated for discoid menisci. One of them had, in addition, an anomalous capsular attachment and an anomalous course of the popliteus tendon. Vascularisation at the free margin was a striking feature in the four discoid menisci of the primitive type. Limited or painful extension was common in these children. In four cases the diagnosis was confirmed arthrographically. Arthrography was carried out in two patients respectively four and sixteen years old, after operation. In both cases a small regenerated meniscus was found as is seen after meniscectomy in adults.

A Comparative Study of Fixed-Bearing Unicompartmental Knee Arthroplasty for Spontaneous Osteonecrosis and Osteoarthritis of the Knee.

BACKGROUND: The purpose of the study was to investigate the two-year clinical efficacy differences between fixed-bearing unicompartmental knee arthroplasty (FB-UKA) for treating spontaneous osteonecrosis of the knee (SONK) and medial knee osteoarthritis (MKOA). METHODS: A retrospective cohort study was conducted, enrolling 25 patients who had SONK and underwent FB-UKA performed by the same senior surgeon at our institution between January 2023 and December 2024. Patients who had SONK were matched 1:1 with 25 contemporaneous MKOA patients who underwent FB-UKA, based on age, sex, and body mass …

Foundations of a knee joint digital twin from qMRI biomarkers for osteoarthritis and knee replacement.

This study forms the basis of a digital twin system of the knee joint, using advanced quantitative MRI (qMRI) and machine learning to advance precision health in osteoarthritis (OA) management and knee replacement (KR) prediction. We combined deep learning-based segmentation of knee joint structures with dimensionality reduction to create an embedded feature space of imaging biomarkers. Through cross-sectional cohort analysis and statistical modeling, we identified specific biomarkers, including variations in cartilage thickness and medial meniscus shape, that are significantly associated with…

Design of a Hydraulic Prosthetic Knee With Control Moment for Adjustable Stance-Phase Knee Flexion.

Lower-limb amputees worldwide have been increasing continuously in recent years. Hydraulic knees are suitable for active transfemoral amputees in developing countries due to their adaptability to various walking speeds and greater accessibility compared to high-end prosthetics. However, most hydraulic prosthetic knees operate via ground reaction force control, which exhibits a double-peak characteristic, causing slight flexion during the early stance phase, leading to unnatural and asymmetrical gait patterns for amputees. This study proposes a novel technique that expands the concept of the tw…

[Early impact of robot-assisted total knee arthroplasty on the treatment of varus knee arthritis].

OBJECTIVE: To investigate the clinical efficacy and advantages of robot-assisted total knee arthroplasty (TKA) in patients with varus knee osteoarthritis. METHODS: Between October 2022 and June 2023, a total of 59 patients with severe knee osteoarthritis resulting in varus were treated with total knee arthroplasty, aged from 59 to 81 years with an average (70.90±4.63) years, including 19 mals and 40 females. The patients were divided into two groups based on the surgical method used:28 patients in the robot group and 31 patients in the traditional group. The robot group consisted of 8 males a…

Total Knee Arthroplasty in the Stiff and Ankylosed Knee at a Mean Follow-Up of 16 Years.

BACKGROUND: Total knee arthroplasty (TKA) in patients who have stiff or ankylosed knees (range of motion [ROM] < 20°) presents technical challenges. Outcomes of TKA in this group of patients at follow-up greater than 12 years are under-reported. This study assesses the outcomes of TKA in this cohort at a mean follow-up of 16.3 years. METHODS: This retrospective review of 109 patients (135 knees) who underwent TKA for stiff or ankylosed knees between 2000 and 2014 included cases from our previously published series, with a minimum follow-up of 10 years. There were 61 knees that were stiff in e…

Unicompartmental Knee Arthroplasty in Spontaneous Osteonecrosis of the Knee: A Narrative Review of Indications, Techniques, and Outcomes.

Spontaneous osteonecrosis of the knee (SONK) is a painful condition characterized by localized subchondral bone necrosis, predominantly affecting the medial femoral condyle in older adults. While total knee arthroplasty (TKA) is a definitive solution, unicompartmental knee arthroplasty (UKA) has emerged as a bone and ligament preserving alternative for this typically unicompartmental disease. This narrative review synthesizes current evidence on the indications, techniques, and outcomes of UKA for SONK. In well-selected patients, those with isolated condylar involvement, intact ligaments, corr…

Development of a Machine Learning Model for Determining Alignment in Knees Following Total Knee Arthroplasty.

BACKGROUND: Malalignment remains a major reason for implant failure following total knee arthroplasty (TKA). Manual analysis of images at a large scale is untenable, and machine learning (ML) models may be a useful tool for determining alignment following TKA and can help identify patients who are at risk for failure. We aimed to develop an ML model that can accurately determine TKA alignment from full-length hip-to-ankle films. METHODS: Long-leg radiographs from 550 patients who had undergone primary TKA were collected, of which 440 were utilized to train an ML model to identify landmarks us…

Determinants of Achilles tendon thickness and their influence on knee function and foot alignment in knee osteoarthritis.

Knee osteoarthritis (OA) significantly impacts the quality of life of individuals globally. However, the interconnections between Achilles tendon thickness, knee symptoms/functions, and foot alignment remain understudied in knee OA patients. This study determines the relationships between Achilles tendon thickness (ATT), knee symptoms/functions, and foot alignment in knee OA patients, considering their interconnected biomechanical nature. In a cross-sectional analysis involving 122 knee OA patients, Knee injury and Osteoarthritis Outcome Score (KOOS) assessed knee function and symptoms. Forefo…

Association of Muscle Quantity and Quality With Knee Extension Strength in Knee Osteoarthritis: The Nagahama Study.

OBJECTIVE: We aimed to examine whether lower-limb muscle quantity and quality assessed by bioelectric impedance analysis (BIA) were associated with knee extension strength and whether the association differed with knee osteoarthritis (OA) severity. METHODS: We included 1,525 participants (63.6% women; mean ± SD age 68.0 ± 5.3 years) from the Nagahama Prospective Cohort. Knee extension strength was measured during maximum voluntary isometric contraction. Lower-limb muscle mass and the extracellular water (ECW)-to-intracellular water (ICW) ratio were used as indicators of muscle quantity and qu…

Total Knee Arthroplasty for Neglected Habitual Dislocation of Patella with Knee Osteoarthritis: A Case Report.

CASE: We performed total knee arthroplasty (TKA) on a 62-year-old male patient who presented with symptomatic knee osteoarthritis resulting from habitual dislocation of the patella. The patient had a painless unassisted gait with stable patella and 5 to 95° knee range of motion, 23 months after surgery. CONCLUSION: TKA can be an effective treatment option for geriatric patients with habitual patellar instability and concomitant knee osteoarthritis, but may require surgical modifications based on individual pathoanatomy.
