Bilateral congenital dislocation of the patella associated with synostosis of proximal tibiofibular and proximal radioulnar joints: A case report.

Congenital dislocation of the patella is a rare and difficult pathology to treat. We present a case of bilateral congenital dislocation of the patella with synostosis of proximal tibiofibular and proximal radioulnar joints without genu valgum deformity of both knees in a 30-year-old man. To our knowledge, congenital dislocation of the patella associated with synostosis of proximal tibiofibular and proximal radioulnar joints has not been reported in the literature yet.

Hemarthrosis of the clinically stable knee due to sports and military training in young recruits: an arthroscopic analysis.

Findings of an early arthroscopy performed in 108 consecutive hemarthrotic and clinically stable knees were analyzed. The patients were young conscripts, the mean age being 19.9 years. Fifty-seven of the knees were injured during military training, 36 in different sports activities, 9 at falls, and 6 in motorbike accidents. The lesions were divided into "primary" (lesions needing operative treatment or the supposed most common lesions in incidence) and "secondary" (other lesions). An average of 1.6 lesions were observed per knee. Dislocation of the patella was the most common diagnosis in 38 (

Management of knee deformities in children with arthrogryposis.

Arthrogryposis includes heterogeneous disorders, characterized by congenital contractures of multiple joints. Knee involvement is very commo … adriceps tenotomy, open quadricepsplasty, and femoral shortening osteotomy with limited arthrotomy. Knee dislocations usually require surgery and should be reduced early.

[Current therapy progress on the acute patellar dislocation].

The treatment of acute patellar dislocation remains a topic of debate among orthopedic surgeons. While conservative treatment has traditionally been favored, recent studies have highlighted the high redislocation rate, prompting a reevaluation of treatment strategies. Current approaches recognize the importance of addressing not only the knee joint but also associated factors contributing to instability. Surgical intervention, particularly focusing on restoring patellar stability through medial patella-femoral ligament (MPFL), repair or reconstruction, has gained popularity. MPFL reconstructio

The results of an extensive soft-tissue procedure in the treatment of obligatory patellar dislocation in children with ligamentous laxity: a post-operative isokinetic study.

In 11 paediatric patients (seven girls and four boys, from 12 to 15 years old) with unilateral obligatory patellar dislocation and ligamentous laxity vastus medialis advancement, lateral release, partial patellar ligament transposition and Galeazzi semitendinosus tenodesis was undertaken to stabilise the patella. The diagnostic criterion for ligamentous laxity was based on the Beighton scale. Outcomes were evaluated radiologically and functionally by measurement of the range of knee movement and isokinetic testing. The evaluation also included the Lysholm knee scale. Follow-up studies took pla

[Results of the Elmslie-Trillat procedure in cases of patella(sub)luxation related to chondral pathology].

Between May 1981 and December 1993, 120 patients (126 knee joints) were operated using the Elsmlie-Trillat procedure because of patella(sub)luxation. 79 patients were reviewed at an average follow up of 4.1 (1-13.6) years. 46 women and 33 men were operated with an average age at operation of 26.9 (14-59) years. The examination included Bentley's Score, radiological parameters (e. g. grading of patella dislocation), grading of chondromalacia and patient characteristics (e. g. age, pre-operative time). 64 knee joints (70. 3 %) were evaluated 'very good' and 'good', using the score of Bentley. Superior dislocation of the patella with early onset patellofemoral arthritis: a case report and literature review.

Among the traumatic patellar dislocations, superior dislocation of patella without patellar ligament injury is very rare. We present in this article, a case of superior dislocation of the patella trapped by interlocked osteophytes in a 38-year-old female who had concurrent bilateral knee arthrosis. Successful reduction was achieved by closed manipulation without anesthesia. Neither redislocation nor symptoms of instability was seen after 36 months of follow-up, although some progression of arthritis was observed. A new classification including all traumatic patellar dislocation was also propos

[A painful knee that cannot bend].

A 59-year-old woman was seen at the ER with a painful right knee locked in extension. This was caused by unloaded hyperextension in bed. Interlocking patellofemoral osteophytes and a superior patella dislocation tilted away from the femur were seen on the X-ray, which are both pathognomonic signs of a superior dislocation of the patella.

The Use of Synthetic Graft for MPFL Reconstruction Surgery: A Systematic Review of the Clinical Outcomes.

(1) Background: Acute patella dislocation (APD) is a prevalent knee injury, with rates between 5.8-77.8 per 100,000. APD often results in repeat lateral patella dislocations due to the instability of the medial patellofemoral ligament (MPFL). Non-operative treatments have a 50% recurrence rate. While autologous grafting for MPFL has been favored, surgeons are now exploring synthetic grafts. We aimed to assess the effectiveness of synthetic grafts in MPFL reconstruction surgeries for repeated patellar dislocations; (2) Methods: Our research was based on a thorough search from the National Insti

Tendon transfers for peroneal nerve injuries in the multiple ligament injured knee.

Although knee dislocations are relatively rare injuries; associated drop foot complication as a consequence of common peroneal nerve palsy (CPN) is substantially high. Even after successful ligament constructions; unresolved CPN palsy is a factor contributing to bad outcome after knee dislocations. CPN palsy is seen more after open dislocations, rotatory dislocations, and especially in patients with posterolateral corner injuries. CPN palsy can readily be diagnosed clinically, although high index of suspicion is needed. Surgical exploration in the acute setting is still debatable. Conservative…

Proximal tibiofibular joint dislocation treated using flexible and permanent syndesmosis fixation.

We present the case of a 40-year-old man who suffered an isolated proximal tibiofibular dislocation of the left knee after a trauma during a soccer game. Physical examination and radiographic imaging revealed an anterolateral dislocation of the proximal fibula. The diagnosis was confirmed by MRI. The treatment choice was open reduction and internal fixation under direct visualisation using flexible and permanent internal fixation. Postoperative treatment includes knee immobilisation during the first week, and partial weight was allowed for 2 weeks progressing to full weight bearing over 4 week

Hindlimb torsional alignment changes in growing rabbits after patellar dislocation.

BACKGROUND: Torsional malalignment has been considered as a risk factor for patellar dislocation. But the influence of patellar dislocation for torsional alignment development remains unknown. The present study aims to investigate whether the torsional alteration of the hindlimb occurs after patellar dislocation in growing rabbits. METHODS: In the present study, 30 one-month-old rabbits were included. The experimental group consisted of 30 left knees of rabbits which underwent patellar lateral dislocation. The control group consisted of 30 right knees of the rabbits which no surgical procedur

Treatment options for patellofemoral instability in sports traumatology.

Patellofemoral instability not only involves lateral patellar dislocation, patellar mal-tracking or subluxation but can also cause a limiting disability for sports activities. Its underlying causes are known as morphological anomalies of the patellofemoral joint or the mechanical axis, femorotibial malrotation, variants of the knee extensor apparatus, and ligamentous insufficiencies often accompanied by poor proprioception. Athletes with such predisposing factors are either suffering from unspecific anterior knee pain or from slightly traumatic or recurrent lateral patellar dislocation Treatme

MRI but not arthroscopy accurately diagnoses femoral MPFL injury in first-time patellar dislocations.

PURPOSE: The purpose of this study was to investigate whether the femoral part of the medial patellofemoral ligament (MPFL) and its injury can be accurately assessed by standard knee arthroscopy in first-time patellar dislocations or whether preoperative MRI is required to determine injury location in patients where primary MPFL repair is attempted. METHODS: Twelve patients with acute first-time dislocations and MRI-based injury of the femoral MPFL and ten patients with recurrent patellar dislocations underwent knee arthroscopy with the use of a 30-degree optic and standard antero-medial and

Traumatic bilateral medial patellar instability in a 15-year-old girl.

INTRODUCTION: Patellar dislocation encompasses the 2-3% of the knee joint injuries, and lateral patellar luxation is by far more common than the medial one. Medial patellar dislocation is described only by few reports and generally as a consequence of previous surgeries. The purpose of this case report is to describe the surgical management of a rare case of traumatic bilateral medial patellar dislocation in a 15-year-old girl with no previous patellofemoral surgeries. CASE REPORT: The patient underwent a traumatic medial patellar dislocation on the left knee, and 18 months later also on the

Management of haemarthrosis induced anterior shoulder dislocation with arthroscopic washout and haemostasis.

Spontaneous heamarthrosis is a known and previously described complication of anticoagulant therapy. The knee and the shoulder joints are amongst the most commonly affected joints [1,2]. Subsequent dislocation/subluxation of the shoulder is also previously described reporting acute anterior shoulder dislocation complicating a non-traumatic haemarthrosis [3], and a further report of a severely subluxed glenohumeral joint that was initially mis-diagnosed as anterior atraumatic dislocation [4]. Both cases were treated by aspiration of the joint, with supplementary manipulation required in the for
